EE was operating the company #24-43 Wagner ST 2DS Can-setter at x-cut #11 in B Canyon,#2 entry in the 23rd East intake. When the victim was found, he was unconscious & unresponsive, his mid torso was pinned in the articulation area of the machine. His feet were in the cab & he was seated in the seat. CPR was administered. The victim was pronounced dead upon arriving on the surface.

Employee was laying on the shop floor underneath the #10 Wagner LST 25X removing the bolts from the axle support plate. As the last bolt was removed, the plate fell to the ground and smashed EE's left index and middle fingers. 7 stitches were required on the left index finger and 6 stitches were required on the left middle finger.

Employee was exiting the porta potty at the mine site headed back to her truck that was parked 15 feet away, when she fell forward hitting the ground. She was taken to the Hospital where x-rays showed her right wrist to be broken. She returned that day on light duty.
The employee was at the #5 drive area at #38 X-cut. He was removing a drive. He was on the in by side of the 25-X. The 25-X oper was backing up, EE reached for the winch hook that was on the tip of the fork. The fork was also on top of the drive arm. He didn't realize the forks were on top of the arm. The forks fell and hit him on top of the right foot on the steel toe
Person was removing an inner race off of a Pickup truck #120 Isuzu. He was removing the bearing race with a heel bar and a hammer. As he struck the heel-bar a piece of the race broke off and it penetrated his outer left index finger between the hand and the first knuckle. After the doctor removed the small piece of metal, it resulting in 4 stitches.
Two employees were in the process of setting a simplex jack to support roof in the belt line. When they applied pressure to the jack a piece of the roof broke and struck the employee in the upper left part of the back. Resulting in a partial collapsed lung.
The employee was preparing to help make a belt splice. He grabbed the template for the splice. He had it resting on the splice board when it slipped the pin slid through the template and caught his finger. Resulting in a laceration to his left ring finger on the tip.
The employee was applying whip checks to the hoses at the head-gate area of the LW. When he pulled the staple the back pressure popped the #13 valve out of the connection and struck him on the left ring finger. Resulting in a fracture to that finger.
Employee was using a 3/4 ton come-a-long to remove a 40' roll of 60"" mine belt from a 975. The belt was lifted high enough for clearance, and then was being lower to the ground when the come-a-long released & free wheeled until the belt hit the ground. EE was holding onto the slack end of the come-a-long and his arm was jerked upward thus staining his left shoulder.
The employee was attempting to remove a plastic plug from a valve bank he was replacing. He grabbed his razor knife and tried to pry the cap out. The knife blade cut through the cap and also cut his left thumb.

The employee was letting down a track jack that was holding up a take-up truck. As he applied pressure to the jack handle it slipped hitting him in the left side of the head resulting in a small laceration.
The employee was dropping miner cable from the mine roof, the miner cable had tension on it. As it came down it bent the employee over backwards and to the side. Resulting in a strain in his mid to lower back on the left side.
The employee was removing cables from the #1 shield in prep. for extended push. He was cutting down zip ties and as he was performing this duty he cut through his left arm just above his wrist. Received 5 stitches
While shoveling silt onto the #2 main line belt, employee's left shoulder started hurting. After approximately another hour shoveling and the pain worsening, EE was brought outside and transported to the clinic where he was diagnosed with an acute muscle stain in left shoulder. He was given a cortisone shot and a prescription for a muscle relaxer & released to return to work.
Dropping a Kennedy overcast panel down. The first course of block had been removed. They were attempting to move the panel when it dislodged a piece of the wall(approx. 4 block) that fell striking employee on the right hand. Resulting in a fracture to the little finger and a laceration to the right wrist.
The employee was preparing to swap base lift hoses. He was waiting for the shearer to pass and he had his hand on top of the base lift. When the shearer came passed. They pushed the pan over and the base lift moved on the relay bar moving the base lift forward pinching his finger between the bolt and the base lift bracket.
The employee had a drill steel stuck in the roof. When he pulled on it to get it out the steel came down on his finger smashing it between the steel and the drill box. Resulting in stitches and a small fracture to his right little finger.
The employee was lifting a Kennedy panel off of the top of a damaged overcast. When he put his hand up and over the top of the panel, there was a metal shard on the top side of the Kennedy. He placed his hand on the piece of metal and it penetrated his leather glove and caused a deep puncture to the palm of his left hand below his thumb.
EE and his partner were putting up a metal mat. The mat was bent and they were trying to get it to stay on the TRS. The mat fell and struck EE in the right middle finger. Resulting in a 1"" laceration. He was transported to Castle Dale Medical Clinic were he received 4 stitches. He was released to return to work on his next available shift.
Two employees were pulling a SC into the mine with a 25X. They had 2 3/4"" chains between the machines. They went around a corner to backup. When they started forward the chains had slack in them an came apart. The SC rolled back and the hoses pulled the steering valve off and it hit the SC operator in the right forearm.
An employee was installing a compressor for a 25x shield hauler. He had the compressor in place an was preparing to mount it to the machine, when it rolled on it side and smashed his finger against the frame of the engine. Resulting in a laceration to the right index finger on the pad. Taken to medical clinic where they glued the laceration.

While setting a roof bolt in the roof, the employee was using a drill steel wrench to finish pushing it to the roof. With his hand on the wrench, he attempted to set the bolt. The wrench slipped off the bolt and kicked over smashing his hand between the wrench and the drill steel clamps, resulting in a Laceration which required 5 stitches.
While building a Kennedy stopping the employee had just raised one of the panels, he remembered that he needed a drain pipe under the panel. When lowering the jack the panel stayed up. While removing the jack the panel collapsed, falling against his left hand, cutting his left small finger. He was diagnosed to have received a Laceration which required 7 stitches.
Employee was attempting to change the wench on the #9 Wagner 25X in the surface shop. As he removed the last bolt, the wench slid back and smashed his left hand between the wench and the frame and cut his hand. 10 stitches were required to close the wound. EE was released to return to work.
while pulling the shuttle car slack in 20th East, Employee had secured a loop of shuttle car cable to the Mini Trac and walked away when he noticed that the nip was falling off of the machine. As he walked back to the machine, another employee signaled the operator to go, and the machine ran over employees right foot. EE was released with restricted duty thru 10/24/13.
Employee was initially injured on 10/5/13 at the 23rd West drive area when he was standing on a cinder block to operate a come-a-long and his foot slipped off of the block thus spraining his left ankle. EE was examined and released by Dr. on this date. On 10/14/13 he returned to the doctor and was given a modified work release.
Employee was attempting to set a simplex jack, he was holding the jack with his left hand & had slide the top section of the jack up to the roof. As he was jacking the jack upwards, the top section of the jack released & slide down inside the bottom section of the jack & smashed his left index finger between the top and bottom sections of the jack. Laceration required 9 sutures.
As employee was preparing to enter the operator cab of a Wagner 25X in 22nd West, rock and coal sloughed off from the roof/rib and hit his right shoulder area. EE went to the ER, x-ray were negative, diagnosed abrasion right shoulder, released for work. The next day his neck was stiff, went back to ER and Doctor has him off from work until 9/3/2013.
2 employees were changing a top roller in the tail piece in 22nd West. While attempting to install the new roller, the roller slipped and smashed injured miners right hand between the roller and the frame structure of the tailpiece, resulting in a fractured Scaphoid bone in the right wrist.
After drilling a hole, the employee placed a roofbolt on the drill head and as he attempted to push the bolt into the hole, the bolt missed the hole & hit the mine roof causing the roofbolt to bow out and smash his left index finger between the bolt and the drill steel clamps resuting in a laceration which required 7 sutures.
Employee was setting up the #3 face in 22nd West. He was preparing to install a ventilation tube (that was laying on the ground) into the existing ventilation tube that was hung from the mine roof. He noticed that the vent tube needed to be turned end for end and as he reached & lifted the tube and spun it around, he felt a sharp pain in his lower back. Released for modified work.
Employee strained his right shoulder while attempting to tighten a hydraulic fitting on the Eimco 975 in the surface shop. EE was taken to the Doctor, examined & a MRI was ordered & he was released to return to work, EE continued working until 7/23/2013, at which time he underwent surgery to repair torn muscles in his right shoulder.
Employee was building a metal cross over on the 2nd West Mains belt conveyor at cross cut #8. As he would tack weld the pieces together, he neglected to lower his welding hood and when he started to weld is when he would lower his welding hook. Employee sustained flash burns to both eyes.
Employee was taking the angle iron rails off of a Kennedy stopping at x-cut 25 in 20th West, so the stopping could be removed. As he went to throw a rail aside, the rail caught his right leg and cut his leg. 10 stitches were required to close the wound.
The employee and 2 other employees were attempting to place a monorail dolly onto the monorail system at x-cut 26 in 20th West. When the dolly slid into place, it smashed his right thumb between another dolly already installed. 6 sutures were required to close the laceration on the right thumb. 2 prescriptions were also prescribed.
Employee picked up a 4"" aluminum pipe that was approximately 4 foot long. As he attempted to toss the pipe along the rib at x-ct 75 in the left mains, his glove snagged a sharp burr on the end of the pipe resulting in a laceration to his little finger on left hand. 8 sutures were required to close the wound.
Injured employee and another employee were carrying a 60"" bottom belt roller on their shoulder, up the slope on the 2nd West belt entry at x-cut #2. As the employee stepped over a small roll in the mine floor, he felt a sharp pain in the middle of his back.
Employee was lifting a 5 gallon bucket of hydraulic oil into the bed of an Isuzu pickup truck when he felt a sharp pain in his lower back. He was initially examined by a doctor on 2/26/2013 and diagnosed with a back strain and released to return to work. He returned to the Dr. on 4/29/2013 for a follow-up and was placed on light duty pending an MRI.
Employee was operating a Fletcher double boom roof bolter in the face of the 21st West development section. As he went to drop his canopy, his right hand was placed on the material tray and as the collar on the canopy boom went past the cable tray it smashed his little finger on his right hand thus resulting in the last 1/4 inch of his little finger being cut off.
On 2/11/13, Employee strained his lower back as he was attempting to lift the end of a vent tube into place in the #2 entry of 21st West. EE reported the incident to his foreman and continued working all of his shifts until 2/25/13, at which time he went to the Dr. and was instructed to be off from work until a MRI could be conducted and evaluated.
Employee was attempting to remove the cover from the exhaust filter housing on the #9 25X located at x-c 37 on #5 road when the cover slipped out of his grip and smashed his left thumb resulting in the thumb nail being removed, a laceration (4 stitches and a fracture to the bone in the tip of the thumb.
As the employee exited the forklift on the surface, he slipped on the ice and twisted his left knee, EE was taken to the hospital and examined and released to return to work. EE had a follow-up visit on 2/20/2013 and a MRI was conducted on 2/26/2013. On 2/27/13 the EE was referred to Orthopedic surgeon and instructed to be off from work until after surgery.
Employee was repairing a damaged overcast when another employee working upwind of him dumped a bag of mortar mix and some of the mortar mix was suspended into the air current and blew into the employees left eye.
While retrieving vent tubes from the face area of the #1 entry in 21st West, the employee twisted his left ankle. EE was examined by a physician and the results of the x-ray were negative and the EE was released to return to work. The next day he return to the physician and was given a work release excusing him from work for that day.
On 1/11/13 the employee claims that as he was walking down the slope in 2nd West intake from the last open x=cut towards the transformed, he suddenly felt a sharp pain in his right groin. The employee continued working his scheduled shifts until 1/29/2013 at which time he had surgery to repair the hernia. This injury became a lost time injury as of 1/29/20123.

The employee and two others were installing 60"" top roller clusters in the new belt line in the 1st West Mains section. As they lifted the cluster, it slipped from the injured employees hand and as he tried to catch the roller cluster, his right ring finger was smashed between the roller cluster and another roller cluster that was laying on the ground.
Employee was working on the 17-2 battery scoop in the 1st West Raises section. As he attempted to place the cover on top of the machine into place, when he lifted the cover and twisted to slide it into place, he felt a sharp pain in his left shoulder.
After the employee exited from the Isuzu mantrip # 23-22, as he slammed the door shut on the mantrip, he smashed his index finger on his left hand in the door resulting in a laceration to the finger which required 10 stitches and also fractured the tip of the bone on his index finger.
While conducting an on shift examination in the tailgate area of the longwall in 17th East, The examiner lost his balance while traveling thru an area where the shields had been pulled in and when he reached down with his right hand to catch himself, he felt a pop in his right wrist. X-rays indicated a hairline fracture to a small bone in the back of the wrist.
Employee was holding a shuttle car cable in his left hand while preparing the cable for a splice when his razor knife slipped and cut the palm of his left hand. 7 stitches were required to close the wound.
The injured employee was roof bolting in the face of the #1 entry of 10th East section. After drilling a hole, he was attempting to insert a tube of resin into the drilled hole and as he reached above his head he heard a pop and felt a sharp pain in his left shoulder.
The employee was in the process of retrieving 6"" plastic aqua pipe from the tailgate entry of the long wall (16th East). He hit the plastic couple with an ax in an effort to break the coupler when a small piece of the plastic coupler broke off and struck him in the forehead just above the left eye. 3 stitches were required to close the wound.
Employee was at xc 22 in the #2 entry in the 10th East section working on the 1"" water tee when the pillar yielded and the employee was struck in the left knee by a piece of coal that was expelled from the yield pillar.
Employee was retrieving 4"" pipe from the belt line in16th East and stacking the pipe in xc #16 when he stepped on a board and a nail penetrated thru his rubber boot and the nail punctured the bottom of his left foot. The ER doctor bandaged the wound and advised that he take 1 day off from work.
Employee was using a sledge hammer to remove a solid block stopping in 14th east return, when he felt a sharp pain in his lower back. He reported the incident and 5 days later (8-5-12) he went to the doctor who kept him off from wok for 4 days.
Employee was cleaning the area in the dog leg of 15th East in preparations of a seal to be installed. Employee attempted to move a rather large piece of rock from the area when he felt a sharp pain in his lower back. He finished his shift and 5 days later, on 7/30/2012 he claimed to not be getting better and asked to see a doctor.
Employee was assisting the electrician to make a splice in a pump cable. Employee was holding one end of the cable in his left hand while prepping the end of the cable. As the razor knife cut thru the outer jacket if the insulation, it also cut thru his glove and cut the index finger on his left hand. 5 stitches were required to close the wound.
The employee was loading mono-rail trucks onto a trailer. He had loaded 14 trucks onto the trailer and as he went to load the 15th mono rail truck, he felt a sharp pain in his lower back.
Employee was originally injured on 6/26/12, when he tripped over a relay bar on the longwall face in 15th East and jammed his left shoulder into the shield leg. EE was taken to the local hospital for observation and released to return to work. On 9/24/2012, the employee went in for surgery on his shoulder, therefore becoming a lost time injury.
Employee was tending the miner cable and as the miner was backing up, he bent over to grab the cable to move it, when the face expelled a lump of coal that struck his thumb on the left hand resulting in an open fracture of the left thumb. 6 sutures were required to close the wound.
Employee was hand loading roof bolts from the bucket of the section battery scoop onto the roof bolting machine. As he bent over to pick up the 3rd bundle of 3 bolts, he felt a sharp pain in his lower back. Employee was diagnosed to have sustained a lower back strain.
Employee was attempting to slide the forks on the Mini Trac together. He placed his right hand on top of the left fork at the slide bar area and as he lifted the front of the fork up with his left hand, he pinched his middle finger on his right hand between the fork and the slide bar. 3 stitches were required.
Employee was attempting to recover a 3' X 3' ventilation regulator from an unused overcast. He was using a razor knife to trim away the foam mine sealant when the knife slipped and contacted his right forearm, resulting in a laceration which required 5 stitches.
Employee was attempting to carry a bad belt roller from the belt line into the intake, as he attempted to go thru the walk thru mandoor in the stopping, he tripped over his own feet and hit his left shoulder on the frame work of the mandoor.
An inundation of methane and hydrogen sulfide gases occurred in the Mains section, in the #93 x-cut between entries #3& #4 while the miner was cutting in the face and encountered a fault/void. A CMI was on the section and immediately issued a 103K order an began his investigation.
Employee was advancing a shield at mid face when there was an outburst of coal from the face area and the employee was struck in the left elbow and left shoulder by coal which was expelled from the face. 2 stitches were required to close the laceration on the left elbow.
At approx 5:55 PM an inundation of methane occurred at the tailgate of the longwall face and at the MPL's in the back of the panel, in the panel #20 longwall section as a result of a cave in the gob. The mine was evacuated and MSHA notified when methane concentrations reached 4.98% at the MPL's. Ventilation was not interrupted and the methane began to go down almost immediately.
While operating the Wagner 2DS scoop, the cab hit a belt chain that was hanging down from the mine roof and rock dust and rust from the chain flew into right eye. Went to the optometrist on 3/20/12 & removed the floating particles from the eye and kept employee off from work for one shift. Was also given a prescription pain medication.
Employee was operating the taigate drum of the shear in the 17th West Longwall at shield #141 when a bounce occurred. He was struck either by the rubber bounce guard or by a piece of coal in the right shoulder and right side of the airstreams helmet. Employee claims to have been unconscious for a few seconds. Employee finished working his shift.
Employee was operating miner in the mains. Water was dripping from the roof and running into his eyes. Later in the shift they became irritated and sore.
Employee sustained chemical burns to both eyes, while operating the roof bolter in the #1 Entry face. While drilling holes to install roof bolts, excessive water was gushing out of each of the holes. Employee would try to shove the tubes of resin and roof bolts into the hole, the gushing water would splash up onto his face and into his eyes.
Employee was attempting to place the duct tube into place on the scrubber of the longwall crusher in the 15th East set up section, when the duct tube slipped and smashed his right hand resulting in a laceration on the right pointer finger which required 6 stitches.
Employee was working on the surface in the upper yard connecting 20 foot lengths of 6"" plastic pipe together. As the employee bent over, he felt a pain in his lower back. Employee was diagnosed at the ER as to have sustained a back strain and instructed by the doctor to be off from work for 72 hours.
Employee was changing a damaged hose on shield #61 on the idle face when there was a bounce/outburst and the employee was hit in the back by a piece of coal that was expelled from the face.Emplyoee sustained 1 broken rib and 3 cracked ribs on the left side of his rib cage.
Employee initially injured his right shoulder while emptying a trash can in the surface shop on 1/12/2012. EE was examined by a doctor, diagnosed to have sustained a strain shoulder and released to return to work. On 10/9/12 he went to the doctor to be re-examined. Was referred to an orthopedic surgeon, and then had surgery on 1/25/2013. Became a lost time as of 1/25/2013.
Employee was injured while unloading 8""X8""X7' square set from a material trailer. As the employee reached for a square set that was on the deck of the trailer, another square set fell from the pile and smashed his left thumb between the 2 square sets. 5 sutures were required to close the laceration.

Employee had finished drill a hole and had inserted a tube of resin and a roof bolt into the hole. Employee pushed the bolt up into the hole with the drill head and then lowered the drill head and inserted the extension wrench. As he pushed the bolt in, the bolt bent and the wrench pinched his left hand between the clamps. 3 stitches were required
Employee was walking down the stairs out side of the con spec building and as she went to step off from the last step of the stairs, onto the ground, her foot slipped and she stained her right hip as she struggled to regain her balance.
EE was attempting to unplug a 5hp flight pump, as he went to lift a shovel full of silt, he felt a sharp pain in his back between his shoulder blades. EE was diagnosed as to have sustained a possible pinched nerve.
EE was attempting to install a new tram motor on a shuttle car when the motor shifted and smashed his right index thumb and severed the thumb between the fingertip and the first knuckle. 16 stitches were rquired to re-attached the tip of the thumb to the thumb.
Employee was attempting to hook a bridle from the feeder breaker to the scoop so that the feeder could be moved to another location. After stopping the scoop, he positioned himself between the scoop and the feeder. The scoop operator decided to exit the machine, so he set the park brake and when he removed his foot from the foot brake, the scooped rolled a couple of inches.
As the ATRS on the Shummar Pan Bolter was set, a slam of rock measuring 2'x4'x 6"" fell from between the 2 rows of roof mats and stuck the employee in the back of the head thus knocking him to the ground. 6 stitches were required for the laceration to the forehead and 5 stitches for the laceration on the pinky finger on the left hand
After setting the ATRS on the Shummar Pan Bolter, a slab of rock measuring 2"" x4'x 6"" fell from between the two rows of roof mats and grazed him on the chest.
Employee was tightening the lug nuts on the section battery scoop by using a 1/2 inch drive ratchet with a cheater pipe when the lug nut broke off and the employee fell to the ground thus spraining his right ankle.
Employee was standing along side of a shuttle car looking into the cable reel compartment of the car when the rib sloughed off and the employee was hit in the back of the right knee by a piece of coal.
Employee was standing between two 6 1/2 foot omega cans that were lying on the mine floor and as the can setter operator went to retrieve another can, the cans were rolled together thus smashing both of the employee's leg.
As the employee was entering the cab of the battery powered section scoop, he snagged his left forearm on a piece of tie wire that was attached to the cab, resulting in a laceration which required 10 stitches.
Employee was changing out bad rollers along the #3 min line belt. EE was dragging a roller along the belt line while walking backwards and he tripped over another roller which had just been removed from the belt line, resulting in the emplyoee spraining his right ankle.
Employee was moving crib blocks from a cross cut. As he lifted a crib block and threw it up on the rib, he felt a pain in his lower abdomen. He continued working for a couple of hours but the pain kept getting stronger.
The contract labor employee was shoveling coal onto the 16th West conveyor belt. As he threw a shovel load of coal up onto the belt, he mis-judged the height of the belt and his shovel hit the bottom side of the top belt which resulted in the shovel being caught in a top roller. EE injured his right shoulder trying to pull the shovel away from the roller.
The contract employee was walking thru the box check stopping just inby the belt portal, As he stepped thru the 5'X4' mandoor and reached back to close the door, the door slammed shut on his left wrist resulting in contusion of the wrist.
The employee was standing on the relay bar of shield #50 washing the shear, when his foot slipped off of the relay bar and he fell to the ground and hit his head on the toe of the #50 shield. 9 stitches were required to close the wound to the back of the head.
The employee was operating the right side drill head of the Fletcher roof bolter in the rock room of 15th East when the pillar yielded and the employee was struck in the mouth with a small piece of coal, resulting in a small cut to his upper lip, which required 3 stitches.
While walking along side of the refuge chamber at x-cut 31 in 15 1/2 West, the employee cut his left arm on a piece of tie wire that was being used to tie the chain link fence to a square set. 9 stitches were required.
There was a non injury coal dust ignition on 7/13/2011 at approximately 10:40 on the left hand side of the cutter head on the miner located in the #1 face area 17th West. The miner was grading bottom approximately 12 feet out-by the face. The flames were extinguished in approximately 3 minutes.
Employee was using a weed eater to cut down weeds when he lost his footing and as he almost fell, he reached out for a jersy barrier to catch his fall. As a result he injured his left shoulder. EE continued working until he went in for surgery on 9/15/2011.
Employee was securing chain link fence to square sets with tie wire. While attempting to tighten the tie wire, his dykes slipped off of the tie wire and hit him in the mouth, requiring 6 stitches.
Prior to the start of the employees shift, while getting dressed in the change room, after the employee had put on his boots, he stood up and struck his head on the bottom edge of his top locker door which he had left open resulting in a laceration to the top of his head. 5 stitches were required to close the wound.
EE was bending a roof bolt to install in a low spot along the rib, when he felt a sharp pain in his right elbow.
A sight string was hung from the roof screens so that the cans in the tailgate could be set in a straight line. After wedging in a can, the mini-trac was tramming out-by to get another can when the sight string got caught on the machine and was jerked down resulting in the ""J"" hook hitting the injured employee in the right cheek. 7 stitches were required.
While helping 2 mechanics remove the radiator from a Wagner LST5 diesel scoop, the radiator slipped and smashed the injured employee's right index finger requiring 10 stitches.
The contractor employee was kneeling on the section conveyor belt at the tailpiece to foam between the feeder breaker and the tailpiece when the belt was turned on from an outby location. The employee was knocked face down onto the belt as he went thru the box check and then thru the belt transfer point. EE sustained 3 fractured ribs.
Employee was attempting to move a 5hp Flyght pump from a water hole that had been pumped down. The employee lifted the pump about waist high when he lost his grip on the pump handle and the pump landed on the top of his left foot resulting in the fracture of a small bone in his foot.
The injured employee was assisting another employee to hang a 5 hp flight face pump from the roof screens along the rib line in preparation for mining in the face. As the pump was hung from the edge of the screen, the screen sagged down away from the rib and a piece of coal sloughed off from the top of the rib and struck him in the right ankle.
The injured employee was watching as another employee was using an ax to break a 6"" plastic aqua pipe coupler when a small piece of plastic broke off of the coupler and hit the injured employee in the upper lip. 3 stitches were required to close the wound.
EE strained his right wrist while hanging the roof bolter cable in the mains section. EE was re-evaluated by a doctor on 4/30/2011, at which time he was referred to an orthopedic surgeon and ordered to be kept off of work.
EE was lifting a bottom roller into the hanger bracket on the #2 main line belt, he felt a pop in his right shoulder. Employee was taken to the ER and released to return to work. As a result of a follow-up visit with an orthopedic surgeon on 4/27/11, the EE was placed on restricted duty by the Dr awaiting an MRI. There is no work available to comply with the restrictions.
Employee was attempting to place a roof mat onto the ATRS of the roof bolter and stepped too far, and as he stepped off of the cat walk of the roof bolter, he caught himself with outstretched arm on top of the canopy and ATRS frame work resulting in a strain of the left rotator cuff.
Employees were retrieving 20 foot sections of dust pipe. As one of the employees dropped his end of the pipe onto the ground, the other end of the pipe flipped up and stuck the victim in the mouth thus creating a laceration to the lower lip which required 8 stitches.
On 3/12/11 employee reported to his supervisor that his eyes were irritated and that the night before when he got home after work and removed his contact lenes, there were coal fines and rock dust under his contacts. The next day the employees eyes were swollen shut so he was taken to the hospital where his eyes were flushed out and he was advised to get prescription glasses.
Person was removing a chain from a belt top roller the chain sliped and hit him in the left leg just below the knee
Employee was injured on 3/8/11 when he was struck in the left knee by the end of a 20' piece of 6 inch aqua pipe that was being pulled around a corner. Employee continued working until 6/15/2011, at which time he had surgery on his knee.
Employee was using a utility knife to cut a hole in a piece of conveyor belting, when his hand slipped and he cut his left index finger. 7 stitches were required to close the wound.
Employee was working on shield #71 and using a 4 pound hammer to clean off the areas to be worked on when a small metal fragment became embedded in his right eye. Employee was wearing prescription safety glasses. The foreign object was surgically removed.
EE was walking at x-cut 72 in #2 entry of 14th west when he caught his left ear on a damaged roof screen that was hanging down. 6 stitches were required to close the wound.
EE was roof bolting and had finished drilling his next hole. Roof conditions were lower than normal and when he tried to install the 6' bolt, he could not get the bolt far enough in the hole to push the bolt up with the drill head. He attempted to push the bolt up by using the roof bolt plate, the plate slipped and struck him in the right knee.

EE was walking up the longwall face, as he stepped over the relay bar at shield #43, he stepped on a lump of coal and strained his left ankle.
EE was attempting to place a spare drill box in the parts car when the drill box slipped out of his hands and smashed his left middle finger between the drill box and the shop car causing a laceration that required 6 stitches.
EE was attempting to put a tailshaft slide plate on the feeder breaker when the slide plate slid off and smashed his left hand between the slide plate and the ground resulting in a fractured left middle finger
While retrieving the 2 piece drill steels from the hole that had been drilled, the 2 sections of steel became stuck together. EE lifted the steels up off of the drill head and dropped the steels back onto the drill head. THE 2 SECTIONS SEPERATED AND THE TOP SECTION FELL FROM THE HOLE AND STRUCK THE BACK SIDE OF RIGHT HAND AND CUTTING THE HAND. 4 STITCHES WERE REQUIRED.
MSHA inspector was preparing to do in evaluation of the scrubber system on the miner. He had the cover lifted into the air and placed his hand in the area from which the cover had been lifted. Inspector turned his head diverting his attention from the elevated cover which fell upon the inspector's hand.
EE was using 2 prybars to reposition the engine cover so that the mounting brackets could be rewelded. As EE hit the cover with a hammer, 1 of the pry bars flipped out and stuck him in the face. Resulting in 3 stitches to close the wound.
EE was using a 6.5 foot long drill steel to try to pry the keeper pins into place on the battery scoop. The drill steel slipped and hit the employee on the right side of his neck and on the right jaw. EE was knocked unconscious for approximately 15 seconds.
EE was attempting to remove a broken fitting from a spray block on the miner. As he attempted to hit a pick hammer into the fitting to extract the broken fitting, he missed the pick hammer and struck his left hand with the ax. Resulting in a hairline fracture of the left thumb.
EE was unloading mono-rail trucks out of the bed of an Izusu pickup and putting them into a cable skid. As EE tossed the last mono-rail truck onto the pile, it started to fall off of the pile and as the EE attempted to catch the mono-rail truck he smashed his #5 finger on his right hand. The tip of the bone was fractured and 8 stitches were required.
As EE exited the Izusu mantrip at the mouth of the 13th west section, she inadvertently slammed the door on her right hand and smashed her right ring finger. 5 stitches were requred to close the wound.
As EE was unloading seal blocks from a pallet to put them on a roller conveyor, the block slipped out of his hand and as he attempted to catch the block, his little finger on his right hand was smashed beteen the block and the pallet resulting in a laceration which required 3 stitches.
EE was dropping a welder cable which was hung with insulated hooks. As EE reached up and unhooked one of the hooks, the hook dug into his little finger on his right hand and the weight of the cable caused the hook to penetrate into the finger resulting in 5 stitches to close the wound.
As EE was shoveling the mine floor between the RA and the stopping, a piece of coal fell from the rib and struck him on the left shoulder.l
EE was using his right hand to guide a 2 piece sectional drill steel into a drill hole with the aid of the drill head when the steel got stuck in the hole had bowed the steel resulting in his hand being smashed between the steel and the clamps. Resulting in 3 stitches to close the wound.
EE was using a sledge hammer to knock out an old stopping. As he knocked out an Omega block out from the top row of the stopping, against the right rib, a small piece of rock fell from the roof/rib area and smashed his little finger on his left hand between the rock and the sledge hammer handle . 3 stitches were used to close the wound
As EE was stepping off of the roof bolter, his right hand slipped on the wet surface of the machine resulting in a dislocated #5 finger on his right hand.
EE had completed his shift & scaned out from the mine. While on his motorcycle traveling down the county road leading off of the mine property, EE states that he hit an obstuction laying in the road. EE lost control wrecking his bike 70' inby the gate to mine. ECPD investigated the incident and found no debris on the road & concluded that excessive speed caused the incident.
As EE was getting out of a mantrip at x-cut 65 in the left mains, his left foot got stuck in the mud and he strained his left knee as he lifted his leg. EE continued to work until 6/16/2010, at which time he had surgery on his left knee.
As EE was operating the tailgate drum of the shields, he was standing at shield # 97 when a small rock fell from between shields 96-97 and struck him in the right ear. 4 stitches were required.
EE was using a water hose to wash out a plugged belt tranfer point. When the plug was cleared, a chuck of coal bounced off of the belt and struck the EE on the right side of his face requiring 3 stitches.
A rock was dumped onto the belt conveyor from the stageloader conveyor and as the rock landed on the belt, it flipped up and struck the employee on the back of his right hand thus fracturing his right ring finger.
As EE was operating a Gehl skid loader the Gehl started to buck and his feet came off of where they were positioned at the same time as he lowered the forks, thus pinning his feet between the forks and the frame of the machine.
As EE was attempting to pull a come-a-long out of the tool box on a 975, the come-a-long struck him in the upper lip which required 2 stitches.
EE was shoveling the ribs in the face as part of the clean up cycle, when a piece of rock sloughed off the rib near the roof and struck the EE in the right ankle.
EE was working approximatly 60 feet inby the #3 portal when he slipped on some ice and fell to the ground, landing on his belt wearable CSE 100 self rescuer, resulting in the fracturing of 2 ribs on his right side.
EE was using a 1 1/2 ton come-a-long and a belt chain to secure a load of omega cans on an open trailer. As EE tightened the come-a-long, the belt chain broke and recoiled stiking the EE in the face. Resulting in a lacerated upper lip that required 4 stitches.
vendor delivering rock dust to the mine site and had arrived at approx. 2:45 pm. He checked in at the warehouse and was working removing straps that secured the load. Employees coming to the warehouse noticed this person lying beside the rear duals on the trailer. They immediately began emergency lifesaving procedures and activated the EMS system
As EE lifted the miner cable over his head, he heard a pop and felt a sharp pain in his right shoulder. EE was examined at the hospital and released to return to work. After working 1 shift, he returned to the hospital ER and is currently off from work pending a MRI.
EE was conducting a weekly examination, as he traveled thru the mandoor at x-cut 56 in the right mains from the return to the intake, he shut the door and as he turned to walk away, on his second step he stepped on a hoe and it swung upward and struck him in the mouth.

On 12/16/2009 EE as he was setting a timber and the rib sloughed off and knocked the timber and EE to the ground. As EE reached towards the round while falling, he injured his right shoulder. EE worked untill he had surgery on 4/1/2010.
EE was using a razor knife to round off the edges of a piece of 2"" plastic pipe so that the pipe could be coupled to another piece of pipe, when the knife slipped and cut his wrist which required 4 stitches
As EE was loading bolting supplies into the scoop bucket, the rib sloughed off and knocked out a rib timber and the timber struck the EE in the right shoulder. Thus fracturing his collar bone.
EE was retieving the section drill steels out of a drilled hole. As he seperated the upper steel from the lower steel, the upper steel dropped and smashed his hand between the 2 sections thus cutting his hand which required 5 stitches.
2 EE's were attemping to change an impact roller on the ash analizer when the 6' pry bar slipped out from under the roller and struck one of the EE's in the left side of the face.
EE was stocking the roof bolter with supplies. As EE went to grab some roof bolts from out of the bucket of the scoop, he cut his right forearm on the band that was used to band the bundle of bolts together. Required 4 stitches.
As the shear was heading towards the tailgate, the headgate drum was grading bottom. EE had placed his left hand on top of the panline to maintain his balance, when a piece of rock measuring 12"" by 6"" was flipped up from the cutter drum and struck him on top of his left hand thus resulting in a fracured bone on the back of his hand and 2 lacerations which required stitches.
EE was tending miner cable in the #1 entry of 11th west, when a piece of the rib was dislodged and struck the EE on the right side of his body and knocked him into the shuttle car with the left side of his body.
EE was walking across the road at x-cut 54 in the right mains when his foot slipped out from underneath him and as he fell to the ground he injured his left shoulder. EE continued to work and visit his doctor until 11/17/09 at which time he underwent surgery on his shoulder.
EE was carrying a timber from a timber pile to a pick-up truck, when he tripped over another timber, resulting in a twisted ankle.
EE bent over to pick up some trash, when a small piece of coal sloughed off from the rib and stuck him in the right knee.
EE was changing out a tram motor on a shuttle car. As he was rolling the new tram motor into place, he felt a pain in his lower back. EE was treated for a back strain and released to return to work untill 10/18/2009, at which time he had a MRI and back surgery for a herniated disk
As EE was walking down the belt line, he struck his head on a fire outlet that was hung from the water line, thus jamming his neck. EE continued working until 7/3/2009, at which time he went to the doctor. He was put on lost time on 7/20/2009.
As EE was tramming the shuttle car, the tram pedal stuck and the shuttle car hit the rib causing the operator to hit his head on the cab of the shuttle car resulting in the operator jamming his neck.
EE was bent over under the monorail to retrieve belt structure, as EE lifted upwards, he struck the back of his head ( below the hard hat ) on the monorail truck dolly. EE finished his shift and later went to the ER. EE was diagnosed with a mild concussion and kept off work for 1 day.
As EE was lowering the tip of shield #7, a small piece of rock rolled off of the shield and struck him on the left hand resulting in the middle finger and the index finger being cut. EE was positioned on shield #6.
EE was unloading a belt roller out of the bed of a pickup truck and as he lifted and twisted, he felt a sharp pain in his lower back.
EE was attempting to slide a 20' section of 8"" drisco plastic pipi thru the pipe bonding machine when the pipe got stuck. As EE stuck his hand in between sections of pipe, another ee shoved the pipe into the other pipe thus smashing his ring finger of the left hand.
EE was kneeling down on one knee putting his tools back into his tool box whem a shuttle car trammed past him and ran over his left leg.
EE was cleaning out the leg pockets of the shields in preparation for extraction. As EE went to crawl out of the shield, he twisted his back.
EE was using the sucker truck to pump silt out of the #6 sump when the suction hose surged and the EE felt a sharp pain in his right shoulder. EE continued to work and visit with his Dr. until 11/22/09 at which time he had surgery on his shoulder.
As EE lifted one end of a 8"" plastic aqua line section of pipe and turned to push the pipe thru a mandoor, he felt a sharp pain in his lower back. EE continued to work until 3/21/2009 at which time he went to the doctor.
At approx. 7:00 AM there was a bounce in the tailgate area of the longwall located in 7th west which occurred as the shear was cutting into the tailgate area. 1 person was injured and mining was expected to be delayed more than 1 hour due to mechanical repairs.
At approx. 5:30 AM there was a bounce in the tailgate of the longwall located in 7th west which occured as the shear was cutting thru the tailgate. Due to mechanical repairs, the shear was down for more than 1 hour.
The shearer machine had just started out of the tailgate and was grading bottom when there was a bounce at shield #145---142. The bounce moved the tailgate end of the shearer machine towards the spill plates, contacting 2 of the spill plates bending, breaking or shearing off some of the spill plate attachment bolts.
EE was attempting to lift the bottom belt to change out a bad bottom roller, when he felt a pop and sharp pain in his lower back
EE was standing on the relay bar of shied #3, washing the shields when his foot slipped off of the relay bar and as he fell, he landed on his right leg which had folded up under him resulting in a stained knee.
EE tripped over a cable on the shop floor. EE was treated at the ER for a back strain and was released to return to work with no restrictions. On 2/2/09 EE went back to the Dr. and was treated for a herniated disk in his lower back.

EE was operating a shuttle car when he felt an irritation in his right eye. When he stopped, he took off his glasses and gloves and rubbed his eye. He then flushed his eye with water. After his shift he went home and went to bed. When he woke to go to work his eye was still irritated and swollen.
EE was sitting in the shuttle car behind the miner getting loaded, when he was struck on the right side of his face by a piece of coal that was ejected from the end of the conveyor boom of the miner.
As EE was walking around the battery scoop, he stepped on a lump of coal and fell to the ground. As he fell he stuck out his left hand to catch his fall thus injuring his left wrist and hand.
EE was hanging a high voltage cable when he felt a sharp pain in his abdomen. The ER Dr treated for a hernia and pushed the herniation back in the abdomin and referred EE to an orthopedic surgeon and released him to return to work. EE had follow-up with surgeon and went in for surgery on 12/2/2008.
EE was attemting to change out an isolation valve on the longwall pump skid. After he pulled the staple out, as he pulled the valve out, it fell and landed on his left hand and smashed his finger between the valve and the decking of the pump skid. EE received 3 stitches to the tip of his middle finger.
EE was using a pry bar to separate two pieces of mono rail when the pry bar slipped and flipped around and stuck him in the face resulting in a small laceration to the right eye lid.
EE and a co-worker were attempting to unplug a plugged up 5 hp pump. As they lifted the pump up out of the mud, EE felt a sharp pain in the middle of his lower back.
EE was assigned the duty of washing the overland belt stucture and the structure under the belt. During the course of the day the EE was on his feet and knees repeatedly. Towards the end of his shift he noticed that his lower back was bothering him. The next day he went to the Dr. and was treated for a back strain.
EE was attempting to put a 36"" wodden crib block on top of the battery scoop, as he threw the crib block, he smashed his little finger on his left hand between the crib block and the light housing on the scoop.
EE was attempting to load a section of vent tube into the bucket of the section scoop, when it slipped out of his hand and smashed his left hand between the bucket of the scoop and the vent tube resulting in a laceration of the hand.
EE was attempting to install a section of vent tubing, as he lifted the tube up he stepped on a small piece of coal and twisted his right knee.
As EE was walking along the up-hill side of the roof bolter, the rib sloughed off and struck him in the right ankle, resulting in a broken ankle.
EE was attempting to advance the mobile tail piece and was kneeling along side of the control; at the same time the face crew was pushing the head gate and the stage loader got bound up on the mobile tail piece and shoved the tail piece sideways striking the EE in the left thigh.
EE was attempting to remove a kennedy overcast panel from an overcast wall, when one end of the panel fell to the ground (5 feet) and bounced up and struck the EE under the chin, resulting in a small laceration which required stitches.
EE was attempting to align a horizontal pin and when he pulled the relay bar, his middle finger on his left hand was smashed between the horizonal pin and the base lifter.
EE was standing beside the main cotroller in the set-up face when a cable that was hung from the mine roof, fell and struck the EE in the head & shoulder resulting in a laceration to the side of his face.
EE was walking down the entry to use the phone, when he stepped on a lump of coal and twisted his right knee.
EE was standing along the stage loader that was being installed for the new longwall panel. The stage loader was sitting on crib blocks about 1.5 feet off of the floor. While attempting to hook another piece of the stage loader together, it fell off of the blocks and knocked the EE to the ground. 12 days later the EE went to the Dr.
EE was bent over, lifting a belt roller, when he felt a pain in his back.
As the EE was loading square set into the bucket of a scoop, another square set rolled off of the pile and struck him on top of his left foot.
After EE had drained the oil from the tailgate haulage and was attempting to tighten the drain plug, the socket slipped off of the drain plug and the ratchet stuck him in the forhead, resulting in a laceration which required 6 stiches.
Employee allegedly has been diagnosed with carpal tunnel. Employee stated that he had seen a doctor and the doctor told him that this condition is a result of his mining activity. Employee scheduled surgery without authorization or notification to employer. Employee has never notified operator until now that this alleged condition existed.
While the EE was attempting to remove a section of mono rail, he had removed the roll pin from the out-by end of the mon-rail. As he cut the links of the belt chain with a pair of bolt cutters, the section of mono-rail dropped and smashed his ring finger on his right hand, which required stitches.
EE was attempting to bend a 6 foot roof bolt so that he could install it. He was using the scissor area of the machine to hold the bolt and as he pushed on the bolt to bend it, the bolt slipped and the EE fell and struck his elbow on the drill box and hit his knee on the ground. EE work the next 2 days and went to the doctor on 4/24/2008.
There was a rib roll along the stage loader that blocked the travelway on the head gate of the longwall in 4th west. The brib roll was approximately 15.5 feet long and 3 feet deep.
There was a bounce on the logwall face at the tailgate at shield 123. EE was operating the tailgate cutter drum and was pinned between the bounce guard and the sheild resulting in ee being bru ise and put on light duty for 3 weeks
