EE WAS TWISTING A 4' PIECE OF GLUE TO MAKE A 2' PIECE. AS HE CUT THROUGH GLUE, KNIFE SLIPPED, CAUSING A LACERATION TO HIS LT THUMB. EE WAS TRANSPORTED TO WHS-GREENE, WHERE HE RECIEVED 5 SUTURES TO CLOSE THE LACERATION.

EE was lifting a pump out of sump with chain. When pulling pub to set on cart, EE slipped and felt pain on right side abdomen. EE saw Dr later that day and was diagnosed with an umbilical hernia. EE has a history of hernias and operator would like to file this under occupational illness.

While knocking block out of wall with sledge hammer to adjust vent, sledge hammer pinched right ring finger between handle and wall. EE broke tip of finger and received 8 sutures. EE will not miss any work and this is being reported as a medical treatment.

EE was making splice on S/C. Attempting to open cable flowing pilot wire, got off track, went to adjust, knife slipped and point punctured Lt hand on outside of palm. EE received five (5) sutures. EE will not have any lost days and return to work Monday, 9/22/14.

Helping unload spool of 27 pair phone cable. As reel was hoisted with a come-a-long above the rail, his foot caught between the spool and the tight side rail causing a severe contusion. He began missing work on 9/3/13.

Employee was using a wrench to tighten a belt block nut on a conveyor system. The wrench slipped as he was pulling it towards him causing him to fall to the ground. As he fell to the ground he landed on his right wrist and fracturing it.

Employee was in the process of changing bits on a continuous miner. While striking one of the bits with a hammer a piece of the hammer flew out, striking the employee on the bridge of the nose and on the face below the right eye. (employee was wearing Safety glasses)

Removing old chuck from bolter head. EE was holding chuck with channelocks as co-worker struck them with a small sledge. A piece of metal few off & struck EE on left side of face on the cheek. He continued to work but required surgery on 3/14/11 to remove the piece of metal. 4 sutures were used to close the wound. No time was missed from work, this is a med treatment report.

While pulling on trolley chain to move overhead trolley the trolley stopped suddenly causing the injured to feel something pop in his left hand. The injured suffered a fractured left hand. The injured continued to work and went off work on 10/30/2010 when an MRI showed he had a fracture in his left hand.

He was pulling belt with a pair of channel locks to make a belt splice and felt pain in his left upper arm. Injury happened 4/18/10 but ee continued to work until 11/14/10 when he came out of the mine due to pain. His first missed work day was 11/15/10.

The injured was cutting a piece of rock dust hose with a pocket knife. He was cutting with his right hand when the knife slipped off the hose and cut his left hand causing a laceration at the third knuckle of his left hand. This is not a lost time accident. This is a MSHA reportable accident because the injured received 6 sutures.

Using a chisel as a spacer in a chain breaker while re-assembling a continuous miner conveyor chain. He had his hand on top of the chisel to remove it and the chain jumped causing the end of the chisel to strike against his right hand between his thumb and index finger. The laceration needed five sutures to close. No time from work was lost. This is a medical treatment incident.

Using a slate bar to pry a rock off the top of a continuous miner, the rock slid back and his left thumb caught between the rock and the bar. His thumb was fractured. Injured did not miss any work. This is a medical treatment incident.

Victim was assisting workers who were re-railing an off the track 9 ton motor. He was placing wood blocking material under the locomotive when the jack that was set under it kicked out and the motor dropped. His right forearm became caught under the motor resulting in a severe contusion.

He was using a sledge hammer to knock out the blocks that remained in a stopping after a scoop had been used to knock the stopping out when several blocks fell from the corner and struck him on the top of his right foot, fracturing it.

While trying to disassemble an air pump, the injured was holding the pump steady with his right foot and turning a wrench to loosen pump part. When the injured jerked the wrench he felt pain in his groin. The injured was diangosed with a hernia. He continued to work until 7/28/07 the date of his surgery. Because the mine was on vacation his first day misseed was 8/6/07.

Removing bits from shearer cutting drum and a piece of the bit tool removal tool broke off and struck him in the left cheek, becoming imbedded. The piece was removed and one suture was necessary to close the wound. The employee did not miss any scheduled work due to the injury.

The injured had hold of a bottom belt roller while changing it. The roller was stuck in a belt chain. A co-worker tried to strike the belt chain with a hammer to free the stuck roller and missed, striking the injured in his left index finger. This accident resulted in a fracture of his left index finger.

The shearer helper and shearer operator were changing bits on the shearer. A bit was lodged in the bit keeper and the operator was using an 8 lb hammer to hit the bit wrench with. As he struck the wrench with the sledge the wrench became airborne, traveled through the face piece of helmet and struck EE above & below left eye, fracturing orbit socket and cut face and eye lid.

THE EMPLOYEE WAS CUTTING OUT THE PILOT WIRE INSIDE THE REEL COMPARTMENT DURING AN INSTALLATION OF A NEW SHUTTLE CAR CABLE. THE INJURED WAS WORKING IN TIGHT QUARTERS AND CUT HIS FOREARM, RESULTING IN SUTURES. NO WORK WAS MISSED AS A RESULT OF THIS INJURY.

HIS RIGHT HAND WAS PULLED INTO OR THRU THE BOTTOM ROLLER AND HE BECAME CAUGHT IN IT. HEJERKED HIS HAND OUT AND SUSTAINED AN AMPUTATION OF THE LAST JOINT OF THE RIGHT INDEX FINGER. INJURED IS LEFT HANDED.

WHILE ATTEMPTING TO PULL APPROX. 20'OF SLA CK BELT WITH CHAIN HOIST THE BEAM BECAME DISLODGED AND TOPPLED OVER STRIKING THE INJURED MAN IN THE LT. SHOULDER AND LT. SIDE OF THE HEAD.
