The employee was repairing a broken shuttle car conveyor chain. He was using a shop hammer to dislodge the chain and struck his left thumb with the hammer.

The individual was preparing to install the 1st row of roof bolts in 2 left face of unit 3. He was visually examining the mine roof as he set the floor jack on the roof bolting machine. The floor jack contacted his right little toe, causing a fracture and laceration. He was positioned on his knees at the time of the injury.

The belt drive take-up was fouled on a shieve. The injured was attempting to free the rope by striking a hammer held against the rope with another hammer. A metal fragment hit his right eye above his cheek and below his lower eyelash.

The employee was replacing a coupling on the feeder. He was tightening a bolt with a socket & ratchet when the ratchet slipped off the bolt head causing his right index finger to strike the bed plate resulting in a fracture to the end of his finger.

THE INJURED WAS IN PROCESS OF MAKING A BELT SPLICE. WHILE CUTTING THE BELT WITH A UTILITY KNIFE, THE BLADE OF THE KNIFE BROKE CAUSING HIM TO CUT HIS FINGER.

The employee was using a pry bar to position a water valve. The pry bar slipped and struck him on his right cheek. This resulted in a laceration which required sutures. AS A RESULT OF THIS INJURY, THE INJURED EMPLOYEE STRTED MISSING WORK ON 8/31/05.

The injured was rolling a cutter drum, for a continous mining machine with a floor jack. The top of the jack kiced out of place and struck the individual on the left elbow resulting in a bruise.

The injured was securing a rock duster in the bucket of the scoop. He was securing the duster with 1 of 2 chain binders when the binder twisted, slipped out of his hand and the handle of the binder struck him on the right side. He continued to work his regular job. However, an MRI on 6-17-05 indicated a fractured rib. He is now missing work.

A piece of rock had the cable to the roof bolting machine fouled under the deck of the machine. While trying to break the rock, the injured was holding a hammer against the rock while another person was using a hammer to strike a blow. A piece of metal flew off the hammer and imbedded into the injured's right leg.

EE WAS DRIVING A BIT INSERT INTO A BIT LINE ON THE RIPPER HEAD WITH A SLEDGE HAMMER. THE EE STRUCK GLANCING BLOW AND THE HAMMER STRUCK HIM ON TIP OF HIS LEFT FOOT.

EE WAS IN THE PROCESS OF TERMINATING A NEW HIGH VOLTAGE CABLE. THE END WAS TIED UP AND HE WAS CUTTING THE "ROPE" STRANDS FROM THE SKINNED OUT CABLE. AS HE CUT THE STRANDS, HE STRUCK THE BACK O F HIS LEFT HAND WITH THE UTILITY KNIFE APPROX 3 INCHES LONG. THIS REQUIRED SUTURES.

EMPLOYEE WAS AT #3 DRIVE DUMPING POINT CLEARING UP SPILL AT THE CHUTE CAUSED BY A PIECE OF DRAW ROCK THAT LODGED IN THE DUMP. HE USED A PIECE OF ANGLE IRON FOR A PRY BAR TO DISLODGE THE ROCK. AS THE MATERIAL CLEARED, THE BELT RESTARTED AND ANOTHER PIECE OF ROCK HIT THE END OF THE ANGLE IRON CAUSING IT TO CATCH HIM UNDER THE CHIN.
