Employee operating hoist had their right ring and pinky finger smashed after running the hoist block onto their fingers. Injury was not a result of lack of training.

EE and a coworker were on the industrial elevator at Enlow Mine. EE's fingers were pinched between the door of the elevator and the door frame for the elevator door. EE lost the fingernail on and fractured a phalanx in left ring finger.

While exiting mine on elevator it came to a sudden stop then dropped a short distance causing injured to lose balance and fall resulting in contusions and sprain to right ankle.

Employee fell due to a sudden stop of the Bailey elevator and injured his left shoulder. Diagnosed on 7/19/07 with a torn rotator cuff.

Employee was firebossing for a construction company. He and three other workers were coming out of the shaft in a construction bucket when the bucket suddenly dropped aprox. 10 to 30 feet and then immediately stopped causing his legs to be driven against the bottom of the bucket and broke both his legs below the knee.

An unknown mechanical failure occurred in the Clyde Frame 10 hoist being used for shaft steel construction that allowed the work platform in the shaft to briefly freefall prior to being stopped by the overspeed protection on hoist.

An unknown mechanical failure occurred in the Clyde frame 10 hoist being used for shaft steel construction that allowed the work platform in the shaft to briefly freefall prior to being stopped by the overspeed protection on the hoist. Employee suffered 2 cracked vertebrae and cracked sternum from impact of sudden stop.

EMPLOYEE WAS RIDING THE EAST FINLEY #2 PORTAL ELEVATOR OUT OF THE MINE WHEN HIS LEFT GLOVE CAUGHT BETWEEN THE ELEVATOR DOOR AND DOOR FRAME AS DOOR WAS OPENING, CAUSING A LACERATION TO THE TIP OF THE LEFT INDEX FINGER, REQUIRING 12 SUTURES TO CLOSE.

INDIVIDUAL WAS WORKING OUT OF A BASKET ON A BOOM TRUCK. THE BOOM WAS BEING LOWERED TO THE GROUND AND TELESCOPED IN. INDIVIDUAL HAD HIS LEFT HAND LYING ON THE EDGE OF THE JIB WHERE THE BOOM PIN S TOGETHER. AS THE BOOM PINNED TOGETHER IT AMPUTATED THE INDIVIDUALS RING AND INDEX FINGER ON THE LEFT HAND.
