While lowering the cage from one deck to another deck, the hoist tripped out and dropped approximately 2-3 feet, this caused the employee to lose his balance and fall on his butt.

The crew of the production shaft were getting into the bucket for transportation to surface at the end of their shift. The last miner in the bucket entered facing inward, with his left arm rested on the rim of the bucket. The bucket belled up and before he could move his arm and his sleeve caught on the guard rail pulling his arm into a space between the rail and bucket.
