The employee was attempting to install a backer beam in the 16' shaft. Upon raising the skip the employee did not realize that the spear guide was coming down. The spear guide came into contact with the employee's right arm causing tissue and muscle damage.

TWO EMPLOYEES WERE STANDING ON A WORK PLATFORM EXAMINING THE 16' HOIST ROPES. THE HOISTMAN DECELERATED THE HOIST FROM 300' A MN TO 50' A MIN. HE INADVERTANTLY DOUBLED CLICK THE 5 WHICH CAUSED THE SKIP TO COME UP STRIKING THE PLATFORM. ONE EE WAS PUSHED BACK, SUFFERED BROKEN THUMB, RIBS, AND COLLAR BONE.
