Showing all 8EE was taking a forklift tire apart while it was still infalted the rim popped up and hit EE's left hand and wrist broke left thumb and wrist.
Employee was operating a box stapler. While trying to move the slack in the air hose they struck the top right side of their head with the staple gun which imbedded the staple into their skull. MSHA was notified when doctors realized that staple had penetrated a portion of their brain. Doctors removed staple and employee is fine as of 4/22/2019
While using a hand grinder, employee lost control and the grinder "kicked back" injuring the top of his left hand. Employee had to get stitches and returned to work later that day.
While using a jackhammer to bust a rock down to size the jack hammer slipped off and smashed employees finger tip between jack hammer handle and metal guard.
While making an adjustment on the pick up conveyor, the robot was inadvertently energized, causing it to begin in a stacking cycle. The arm of the robot struck the victim before he could react. Two lacerations were sustained on the face. One, vertical, along the bridge of the nose, the second was a horizontal laceration below the left eye.
Associate inflicted small punctured on top of right foot with a 50lb pneumatic jack hammer, while gadding salt rocks. Associate received 3 stitches at the emergency room and retured to work to finish his shift.
