Employee was struck in the eye with a self tapper screw while attaching metal sheeting to door stopping causing a small laceration to right eye. EE has prescription eye drops until it heals.

Mechanic grabbed a piece of all-thread which had a piece of sharp metal attached from a previous cut and cut finger requiring stitches.

Employee working underneath equipment removing clutch/brake had small debris fall into eye. Safety glasses was worn at the time.

Miner tripped on hose while climbing into basket of bucket truck causing EE's left knee to become sore. Miner saw a doctor on 4/17/2023 and has been put on light duty by EE's doctor. The miner has been doing EE's regular job duties with no lost time since the time of accident therefore the accident didn't become reportable until 4/17/2023.

Miller was attempting to bar a wedged rock and grip slipped off of the pry bar causing them to bang a hand against some metal framework and it cut a finger.

Employee was operating a box stapler. While trying to move the slack in the air hose they struck the top right side of their head with the staple gun which imbedded the staple into their skull. MSHA was notified when doctors realized that staple had penetrated a portion of their brain. Doctors removed staple and employee is fine as of 4/22/2019

Struck against a moving object

Employee parked truck in blind spot of loader operator to get lunch money. Loader backed into the truck with employee in it and it jarred employee's neck.

Miner was washing off the outside window of the 775 hual truck and slipped off of the bumper of the truck and fell backwards to the ground (aprox 4 feet)

Employee was cutting stretch wrap off of pallet and slipped with the knife cutting left hand above the thumb. Employee received 4 stitches and returned to normal work duties that day.

Employee was chipping slag off of a weld and had a piece of slag fly back and strike EE in the eye causing a globe lacerating.

While using a hand grinder, employee lost control and the grinder ""kicked back"" injuring the top of his left hand. Employee had to get stitches and returned to work later that day.

Foreign debris blew into employees eye while performing maintenance work on a very windy day. Employee was wearing safety glasses. The object was not imbedded but he did receive a antibiotic prescription.

Employee extended his arm out to grab a hold of a draw string chest high and felt a sharp pain in his left forearm. Employee went to doc 10/7/14 for diagnostic and was release to come back to work the following day (10/08/14).

Mechanic was opening a new welding rod can and had it half way opened when his hand slipped and cut his right hand (between thumb and pointer finger) on the can lid. He went to the doctors office and received stitches.

Grinding metal in a dark location and took his glasses off and a small piece of metal got in his left eye.... it wasn't imbedded but the doctor had to remove it.

While using a jackhammer to bust a rock down to size the jack hammer slipped off and smashed employees finger tip between jack hammer handle and metal guard.

Left foot became lodged between frame & tire when crawling out of machine causing employee to fall onto concrete on left side should & arm.

While in the process of removing a 4X8-3/16"" steel plate from a vertical rack, employee was using a welding clamp as a handle. The clamp slipped off the sheet and employee lost his balance, rolling his ankle and breaking the fibula just above the ankle joint.

While making an adjustment on the pick up conveyor, the robot was inadvertently energized, causing it to begin in a stacking cycle. The arm of the robot struck the victim before he could react. Two lacerations were sustained on the face. One, vertical, along the bridge of the nose, the second was a horizontal laceration below the left eye.

Associate was loading chunks of rock salt (avg. wt. 35lbs) from the bucket of a 966 Cat loader into a large (48""x48""x48"") shipping bag which was hanging, via support loops, on the raised forks of a forklift. Associate picked up a rock, turned and accidentally stepped off the edge of the bucket, then landing on the raised fork of the forklift with his back.

Associate was prying belting with a prybar. The prybar slipped off of the belting and hit him above the right eye. Assoiate received 6 Stitches above eye at the emergency room and then retured to work to finish his shift.

EMPLOYEE WAS BRAKING SALT ROCKS WITH AN AIR POWERED CEMENT BRAKER. THE POINT OF THE GAD BROKE THROUGH THE SALT ROCK SUDDENLY AND SLASHED OPEN THE SIDE OF HIS RIGHT FOOT.
