Employee was removing bolts from equipment. While removing, he was using an impact gun in one hand and with the other hand was applying pressure to the bolt head. Bolt came free from the employee's grip and cut the knuckle of the ring finger on the left hand, requiring stitches.

A HUNG STAGELOADER CHAIN FLIGHT WAS BEING REMOVED BY CUTTING INTO THE FLIGHT WITH A CUTTING TORCH. WHEN THE FLIGHT WAS CUT INTO A PIECE STRUCK INJURED ON THE LEFT BACK SIDE ON HIS HEAD. INJURED WAS APPARENTLY UNCONSCIOUS FOR APPROX 2 MINUTES. INJURED WAS TRANSPORTED TO HOPSITAL AND RELEASED SAME NIGHT WITH NO INJURIES.
Injured was removing material from a stopped up air pump. He had turned the pump off and stuck his hand down inside of the pump; as he was doing this his knee struck the cut off valve for the air and turned the air on to the pump. This caused the pump piston to move and strike his hand and pinch it between the pump housing and the piston.
Injured was removing a bent bolt from a hole. Injured accidently hit the wrong lever on the roof bolter causing the bolt to spin which struck the injured resulting in a broken bone in his arm.

injured was advancing a shield manually. injured was sitting on pontoon. when shield was advanced a rock fell from between the shields striking injured employee on his right foot and ankle.
Employee had installed a leg for a truss bolt. As he was pushing the end of the leg up against the roof with a piece of drill steel in order to secure it against the top, out of the walkway the leg slipped off, striking him across the nose.
Employee was drilling shot holes in the face fault on the 8k plow. A brow broke off and struck the injured on the left shoulder, neck, and his right ankle.
While attempting to drill a 5' hole a piece of rock broke loose over top of the TRS and fell down hitting the drill pot and canopy. The rock struck employee above the back of his right leg cutting through his rubber boot and cutting his leg. Laceration was about 3/4 inches long.
The slack in the longwall belt monorail trolleys was being pulled up. A sheave wheel and wire rope was being used. The injured employee walked along he belt placing himself in the "vee" of the wire rope. The sheave wheel came loose from it's mounting and flew across the belt striking the injured employee in the jaw. This resulted in a broken jaw.
THE INJURED EE WAS CARRYING A HOSE TO #91 SHIELD. THE SHIELDS WERE PULLED UP. A PUSH PRIME WAS PUT IN TO ADVANCE THE PAN LINE OVER. WHEN THE PAN LINE WAS ADVANCED, THE INJURED'S RIGHT FOOT SHI PPED OFF THE PONTOON. #92 SHIELD ADVANCED TO THE FACE ONTO INJURED RIGHT FOOT. THE INJURED PUSHED A STOP BUTTON AND PUSHED THE SHIELD OFF HIS FOOT.
