While cutting cable tray with a battery powered sawsall, the individual's left hand slipped past the tool grip, crushing his finger in the mechanism driving the saw blade. Stitches were required.
Employee was assisting with threading a drill steel and got his hand wrapped up in the steel.
While using a floor mounted grinder to smooth up a cut on a thin piece of metal the object and the employees right hand were pulled into the wire brush wheel. The table and guards were all in place and properly adjusted. The employee sustained an amputation on the tip of the little finger and a laceration on the ring finger which required 12 stitches to close.
Conditions unknown. Minor right eye irritation.
He was attempting to swing the boom on 3018 bolter. When he activated the lever, the boom swung and caught his thumb on his right hand between the base lift jack and the boom causing a fracture to the right thumb.
He was using a grinder on a D-9 dozer, when the grinder kicked back and struck him on the left hand, causing a lacertion that required 4 sutures.
He was operating the longwall shearer, when a piece of rock fell deflecting water from the machine sprays going into his left eye, causing a traumatic cataract of the left eye. He continued to work at his regular duties until 4/22/09, when he was taken off work to schedule surgery to repair the traumatic cataract.
Injured was raising the drill head to put steels in drill chuck. Steels bent toward mast jack catching right middle finger. The finger was fractured and lacerated.
Victim was directing miner operator who was slabbing a new longwall face. The miner bits contacted the wood of a fold out bridge mat constructed of six inch timbers. Injured was thrown against the mine roof and struck by a timber. He sustained multiple contusions of his chest, trunk, legs, and two fractured ribs.
While employee was hanging the bolter cable on the rib, the operator took the cable reel up and the cable pulled his legs from under him twisting his knee.
He was assisting the center bolter while installing bolts. He reached across the bolter head to grab a stuck steel, his left foot was under the mast when lowered, causing a crushing injury to the left foot.
Employee was operating the longwall sheraer when he placed his right hand on the hand rail a piece of rock flew from the drum and struck his right ring finger causing a fracture.
Cutting a broken beam when beam broke loose from side sheet (bolts were cut off) and pinched his right index finger on bottom deck stringer that was above the beam being cut. The laceration required five sutures. No time from work was lost.
He was drilling a bolt hole, steels became jammed in roof, he put his right hand on top of the drill head to check the suction, a drill steel came out of the roof and struck him on the right hand causing a fracture and laceration to the right middle finger.
Mechanic was straightening sheet metal on a Terex MT5500 haul truck with a small porta-power. The porta-power ram slipped and pinched thumb against sheet metal.
He was standing near the suction line of the C Mains centrifugal pump while the line was being filled. The line came apart at a coupler and struck him on the right side of the head causing a laceration to the left side of the head, concussion and skull fracture.
A welder was attempting to attach a 3/16 steel sheet. The steel slipped 14 inches resulting in a laceration requiring stitches. The welder was wearing leather gloves at the time of the incident.
Employee was dropping the drill pot and was going to remove the drill wrench when a piece of 5 foot drill steel that was stuck in the drilled hole came out of the hole hitting his little finger against the wrench. Fractured the tip end of his little finger.
Employee was using the wash down hose on the water pump. The end blew off cutting his right hand at the base of the thumb. He received stitches.
The employee was installing a 12 foot rope bolt. The bolt bent and the employee hit the rotation causing it to hit him in the back of the left hand fracturing it.
Grinding particulate entered left eye sometime during process of welding, grinding, switching PPE, and moving place to place.
While applying torque to a bolt, the drill chuck and motor fell off and hit him in the left knee, causing contusions to the left knee and left rib cage. He continued to work at his regular duties and eventually went to a physician. He was removed from work on 9/10/2008.
Victum was positioning the Alpine miner shovel extension jack on the CM and it slipped. EE received a laceration on the left ring finger that required 5 sutures to close.
EE was in the process of placing miner cable and water hose on the Alpine miner head. The pump motor was running on the miner and he had the remote on his person. He was standing beside shovel and trammed the miner forward to put cable on the head, when the shovel extension caught him against the rib. EE received contusions to the spine, groin and abdomen.
The employee states he was putting up a pope belt and it was slugging mud into his face and he put and he put up his hand up to block the mud, and the plate hit him in the left hand cutting it.
At the time of the incident the technician was removing the wheel bolts from a CAT 793 truck. When the technician engaged his impact wrench, the socket came free from the wrench and struck him in the upper lip.
He was operating the longwall shearer when a piece of rock fell from between the #12 shield, struck the tailgate drum, flew from the tailgate drum and struck him on the left hand, causing contusions to the hand and amputation of the tip of the small finger.-per call-loss of finger to back end of fingernail-per call W. Dennison.
Using a battery powered reciprocating saw to cut electrial conduit and, while holding it with both hands, his left hand slipped and his left small finger became caught between the saw blade chuck and the saw frame. The laceration required two sutures. No time was lost due to this incident.
He was troubleshooting a longwall shield when the shield pulled in on his left foot causing a crushing injury to the left foot.
After installing a roof bolt, victim was dropping the mast jack to remove the wrench. He had placed his left hand on the stabilizing cross member on the 2nd stage of the mast jack assembly. As the 3rd stage of the jack lowered, the bar on the 3rd stage caught the end of his middle finger and glove between 3rd stage and bar, severing tip of middle finger between 1st and 2nd joint.
While employee was operating the miner, he backed up and caught his upper right leg between cable arm and rib.
He was advancing the #3 longwall shield when a chain became caught in the relay bar. A piece of chain broke off and struck him in the right chin causing a laceration requiring 3 sutures.
While cutting conduit with a battery saws-all, the tool slipped and injured his right thumb causing a minor crushing injury to his thumb.
He was a torch to burn off a shaft and the shaft fell striking him on the left side of his head. This resulted in a contusion to the head and laceration of the left ear. The laceration required five sutures. No time was missed from work.
Using a rock dust hose to apply rock dust in a high roof area, the dust hose was being held overhead and there was a quick surge of dust in it. The hose recoiled and he strained his right shoulder.
Using a two part steel to drill a hole & install a roof bolt. The lead steel stuck in the hole & he placed his right hand over the drill head to check for suction. As he did so, the steel fell out of the hole & struck his hand fracturing his right middle finger. THIS WAS NOT A LOST TIME INJURY.
Walking longwall face from the tail toward the head & was struck on the chin by a piece of coal that flew out from the shearer drum. The resulting laceration required two sutures to close. Injured did not miss any scheduled shifts due to the injury.
He was standing beside the continuous miner when a strap extension fell from the strap jacks and struck him on the left forearm causing a laceration requiring 5 sutures.
Truck operator sustained a neck and shoulder strain when the bottom of an electric shovel door contacted the truck bed.**It was originally believed that this injury was minor and 1st aid only. Injured had ongoing discomfort and saw a Dr. and on 8/142007 was restricted from returning to work.**
He crossed in front of the #23 shield as it was activated to move. The shield moved forward and caught his right leg below the knee between the shield pontoon and the conveyor causing a crushing injury to the right leg. The lower part of his leg was eventually amputated.
EE was bolting on the tube side of the miner. He was attempting to raise the tube for clearance when he felt a pop in his right shoulder. He was diagnosed with a right shoulder strain and possible rotator cuff damage.
While cutting angle iron on guard, grinding wheel caught on steel, kicked back & cut finger with cutting disc.
Putting drill pipe on the head, while shouldering up, drill pipe grabbed the rope and started spinning, wrapping him up with rope.
Employee was loading roof bolt plates on bolter; plates slipped and fell behind boom. Employee reached in to get plates; operator dropped boom mashing fingers on both hands.
After drilling a hole while installing a roof bolt, injured was lowering auger and they sprung apart between the starter and extension auger, striking his left thumb. He suffered a laceration and dislocation that required surgery. He began losing time his next scheduled shift.
While bolting top, drill steel became stuck in the top and came loose hitting finger between drill steel and drill pot; cutting right ring finger.
Unit #1 - Employee was rib bolting off of the scoop with a rib drill gun. The auger hit a rib bolt on the other side, causing the gun to twist resulting in a strain to the left hand, wrist, and forearm area.
He was pushing an 8 foot combination bolt into the hole, as he was pushing the bolt, the bolt caught a separation in the roof and he felt pain in his back.
Employee had a drill steel resting against the mast, when the mast was lowered the mast struck the drill steel. This caused the steel to strike him on the right foot causing two broken bones in his right foot.
Employee's right leg was caught between crane and coupler.
