The injured was operating the bolter to install a fly board for the face of #1. Curtain was hung across the intersection to keep air in the face. After he drilled his hole and was pulling the steel down he used his hand to help bring it out and the air blew the curtain into the driller station catching the steel and wrapping his arm into the steel.

Employee was installing a roof bolt and during the rotation process a small slither of metal off either the bolt or the bolt plate became airborne and struck the employee in the right eye.

While grinding and welding beneath a ramcar tailgate and lying on his back he raised his welding hood and foreign material fell into his left eye. He finished his shift and worked through the next day before continued irritation prompted him to seek medical attention. A piece of material was removed from the eye; he was prescribed medication and then released to unrestricted duty.

Employee received a laceration to his left knee while cutting a tree down. He had notched one side of the tree and making the back cut, the tree started to and he was moving to a safe area when the saw hit his left knee.

While roof bolting a drill steel fell against the side of his neck. The steel was hot from use and caused a burn extending behind his right ear. He continued working and finished his shift before seeking medical attention, the wound was cleaned and he was issued a full release. The application of prescription medication made this a reportable injury.

While roof bolting and adding a drill extension the extension bowed, sprung free and struck his right hand near the base of the index finger. X-rays revealed a fracture and he was placed on a 'no use of right hand' restriction.

The Mechanic and a co-worker were in process of installing cutting edges on #1014, D11 bulldozer. While lifting a cutting edge with the boom and chain from the mechanic's truck, the eyelet on the cutting edge suddenly broke loose causing it and the chain to fly up and strike him on his hand. He sustained a laceration to his left hand as a result.

Injured was sawing a timber when the blade became stuck. He forcefully tried to pull it back towards him. The blade came out of the timber unexpectedly and struck him on the left forearm, causing a laceration that required 10 stitches to close.

While roof bolting and installing an 8' bolt pinched his left index finger between the roof and the plate causing a laceration to the fingertip. Transported to Springfield Memorial ER where X-rays revealed a fracture to the fingertip and the wound was closed with sutures. He was released to unrestricted duty and reported to work on his next scheduled shift.

Employee stated while working in the shop he sustained 'welding flash' to both eyes due to a nearby co-worker's welding activities.

Drill steel got stuck in top while drilling hole, trying to spin the drill steel down, when the head went up and caught hand between steel and the top. Had five stitches in right thumb.

While operating a continuous miner and cutting the slab side of a straight cut a rock slid off the miner and struck his left foot leaving a laceration. Transported by ambulance to Memorial Medical ER Springfield where X-rays also revealed a fracture to the foot.

The injured was installing a roof bolt, he was spinning the the drill chuck and bolt counter clockwise so that the bolt head would seat down into the drill chuck prior to pushing the bolt up. He was holding the bolt as it spun; his glove was snagged by the bolt which jerked his hand/thumb around the bolt.

While bolting, allowed his left hand to rest on the drill pot where it was struck by a drill steel that had been left in the roof, causing a crushing injury to the fingertip and nail. X-rays revealed a fracture to the fingertip and treatment included removing the nail and suturing to close the wound.

While lowering the drill steel. The steel came out of the chuck and struck the individual on the end of the left thumb. The individual was seen at the hopital and diagnosed with a fracture to the left thumb.

The injured was installing the outside roof bolt with a curtain hook on top of the plate. As he pushed the bolt up, he said he left the plate around 6-8 inches from the roof. He then tried to adjust the direction of the curtain hook. The unused re-coil from the up pressure broke the jacket of glue, pinching his left index finger.

On 01/18/10 EE's wife reported that the EE had injured himself while operating his dozer on 01/14/10 and not reported the injury. Now on 01/18/10 he was in pain and needed to go to the Dr. This accident is under investigation because it was not reported before leaving mine property and pending results of the investigation, this accident report may be requested to be withdrawn.

Injured was tightening wheel lugs on a DBT Coal Hauler. The bolt he was tightening broke, causing him to fall forward towards the hauler. Injured struck his mouth on the hauler, causing a large cut on his lower lip that required 2 stitches to close.

EE was bolting top, he had put extension on the steel and was raising it into the hole when the steel pressurized in the hole and it sprung into the drill chuck hitting his hand.

While operating a grinder felt right eye irritation. Followed up next day with a visit to his personal physician, the eye was irrigated and a piece of dark material was removed. The irritation remained and a subsequent visit removed an additional piece of imbedded material. Released to unrestricted duty and continued to report to work for all his scheduled shifts.

Employee was sitting in a 375 Komatsu dozer at the beginning of shift. The other equipment was parked in line behind him. A D11 dozer was parked directly behind the fill dozer and in the process of getting out of line and to the work area, his blade hit the ripper shank on the 375 dozer. At 7:49 pm the employee notified the foreman that his neck was hurting and he was taken to ER.

The employee was operating a dual boom Fletcher roof bolter when a piece of roof rock fell, striking the drill pot causing it to fly up, striking the indivual in the mouth. The laceration required 6 sutures to close the wound.

Injured was tramming bolter into #4 Entry to bolt 4 right Crosscut. He moved from deck to "inch" tram controls. As machine pivoted around, a curtain board on machine caught on a rib and sprang towards operator, stiking him in the mouth. Busted upper lip and possible fracture of facial board.

While roof bolting and drilling a hole he lowered the drill pot to remove steel and the chuck adapter came out of the pot. While re-inserting adapter the steel fell from the roof striking the left fingertip resulting in a laceration and fracture. Transported to St. John's Springfield for sutures and released to restricted duty.

Employee stated the dozer he was operting got on a rock and slid sideways when the tracks got off the rock it came to a sudden stop causing a jar to his shoulder. He continued to work until 4/16/09 when he went to dr and was referred to farily dr for an MRI. He was placed off work on 4/21/09.

The D11 dozer track jumped segment sprockets and jarred operator's back. Foreman made note of complaint by employee, but accident was not reported to company until 08/14/2009. After investigation it was determined to be a reportable on 08/21/2009.

While replacing cutting edges on a D11R dozer, employee was using an air hammer to remove the dirt and other material from bolt heads. A foreign object was blown behind his safety glasses and into his eye. He went home and used some eye wash Friday and Saturday night. The pain increased on Sunday and he went to ER.

Employee stated that he was grading the haul road, when the blade of the Grader struck a tree that was imbeded in the berm of the road way. This caused the grader to stall, resulting in a sudden stop, which caused pain to his lower back. EE refused medical treatment, but went to his family doctor after work and was placed off work.

Employee was installing a cable bolt, when drilling the hole his drill steel stopped up. He laid his left hand over the drill steel that was still inserted in drill head checking the suction on the steel, when another piece of drill steel fell out of the hole hitting the tip of index finger on his left hand.

Adding new drill steel to one of the company's drills. The new steel was put in place by using the boom on the mechanic's truck with a strap around the steel. After several attempts of lining up the steel and jogging the head to thread the steel, EE got his hand caught in the strap. When the head was jogged it wrapped his arm around the steel.

Employee was assembling a new 993 wheel loader. Employee was removing bolts that secured rollover protective structure to shipping pallet. When employee cut the first bolt the ROPS shifted breaking the other bolts and fell on employee's foot.

EE was operating a 1500 Komatsu Haul truck being loaded by an O&K 340 shovel and stated he received a jar from a rock that was being loaded. EE continued to work until 01/16/09 when he went to Dr. and was placed off work with a strain to the back.

While spinning a bolt during installation, a small piece of shale fell and was struck by the spinning plate, causing it to strike the operator on the right side of the face leaving a laceration. Transported by ambulance where the wound was cleaned and sutured. Advised not to return to work until his risk of infection is reduced, minimum 1 to 2 weeks.

The injured was drilling a hole for a belt anchor, when he let the head of the bolt machine down the drill steel stuck in the mine roof, momentarily, and then fell out of the drilled hole and struck the injured on the chin resulting in a laceration which required 2 sutures to close.
