The employee was drilling with a Jackleg when the drill steel shanked/broke. The employee received a laceration to the L-wrist when employee contacted the end of the broken steel that remained in the hole.

The employee was welding in the shaft when employee noticed irritation in right eye. The employee received on site first aid and was taken to the Emergency room and diagnosed with a corneal abrasion. The employee received a prescription of antibiotics as a precaution to prevent a possible infection.

The dywidag bolt driver fell from the recently installed bolt, deflected off of the drill, and hit the employee on the right side of the mouth. The drill helper sustained several broken teeth as a result of the impact.

Shotcrete dust entered the employee's right eye behind the foam lined safety glasses, while chipping dried shotcrete out of a concrete bucket. The employee was treated and released to full duty.

The employee received flash burns to both eyes while working in the vicinity of plasma cutting that was taking place in the shaft. The exact time of exposure was not known. The employee returned to full duty work, post treatment by an eye physician.

The injured ee was assisting the Jackleg driller by helping pull the machine out of previously drilled hole they were cleaning. The wrist of the helpers glove was caught momentarily in the rotating drill steel, pulling the glove off the L-hand. Tip of the middle finger was amputated as the glove wrapped around the drill steel and the injured pulled back to free his hand.

Employee was holding a length of 3/4" all thread with his left hand & operating the grinder with his right hand. The grinder kicked back and the cutting wheel contacted his left thumb, causing a 1/2 inch cut & severed the tendon on the back of his thumb.

Employee's right hand middle finger tip was pinched between the verticle shaft mucker and the tower frame corner angle iron. Resulted in smashing his finger tip which required suturing.

Worker was drilling with a jackleg drill installing ground support. The air line gooseneck detached from the machine and struck the worker on the right side of the chin.

Worker was operating small air tugger to hoist hoses from shaft bench. Hoses hung up on shaft form. Worker placed hand on cable 6 inches in front of drum and shook it to free hoses. The cable took up slack and rotated slowly pinching worker's index finger between cable and drum.

Worker was attempting to drive a split set bolt in the back. The jackleg drill slipped and the worker lost his balance and his forehead contacted the rockbolt plate hanging on the split set bolt. Laceration requiring sutures resulted.

Worker was drilling with jackleg drill to install rock bolts. While pulling drill back out of a hole, the pressurized air line disconnected from the drill and struck the worker on the forehead.

Driller helper was attempting to tighten water swivel on rod string when driller engaged lever with incorrect selector switch setting. Driller rotated rods, forcing 36" pipe wrench into helpers hand and compressing forearm between wrench handle and mast support strut. Report is late due to discussion with MSHA regarding proper jurisdiction over drill site.
