While installing pipe in the shaft, a small piece of metal entered a worker's right eye, causing irritation and redness. Employee was taken to an optometrist, the debris was removed and the employee was prescribed eye drops. At the time of the incident, the worker was wearing safety glasses.

The injured employee was handling the crosshead hook on the #2 bucket for inspections on the bell line. As the employee was walking backward with the crosshead hook, their right boot caught on a "lip" of the collar doors. This caused the employee's knee to hyperextend.

Two employees were placing a section of steel brattice onto the Shaft 10 auxiliary conveyance. While positioning the brattice to allow the conveyance door to close, the injured employee lifted the material and tore their right bicep.

A crew was conducting auger drilling operations. The assistant operator went to retrieve a 4.25" auger from the storage rack and was struck in the left hand when auger shifted in the rack, pinching EE's hand between the moving and a stationary auger. Impact resistant gloves were worn at the time of the injury. The injury was a laceration to the outer left hand requiring 8 sutures.

The employee caught a boot on the top of a short retaining wall and tripped and fell. The injured struck forehead on the forks of forklift causing a laceration that required three sutures to close.

The employee's finger was pinched between the sub-collar gate latching mechanism when a second employee assisted with opening the gate unexpectedly. The employee received stiches to close the wound.

Injured employee was assisting another employee tighten a drill water swivel when the pipe wrench slipped off the swivel and caused the employee to lose EE's balance and swing the wrench backward toward injured employee, striking the EE on the left cheek with the end of the pipe wrench handle. This caused a facial laceration that required wound closing sutures.

The main hoist would not pass the start of shift brake test, upon investigation the pinion brake disk was found to have oil on it. Oil was determined to have come from the adjacent hoist bearing.

At 10:51 the #9 hoist went down with a high temp reading (140° F) on #3 bearing. Employees were evacuated from the shaft using the Auxiliary hoist. The bearing cap was pulled at that time for inspection, during this time it was noticed that the oil level site glass was reading higher levels than what was in the gear box and it seemed to be air locked.

The No. 9 shaft auxiliary cage dog spring was found to be defective during the pre-shift inspection. Underground work was suspended until the new egress cage was commissioned on 5/5/2020.

TPL2 No. 9 shaft east drift pulled out existing ground support and fell. There were no persons in the area at the time the ground fell. 8' resin bolts, 6' splits sets and wire mesh failed due to working ground that started in the newly blasted face. The area had been barricaded previously due to the spalling unsupported ground.

The employee was drilling with a Jackleg when the drill steel shanked/broke. The employee received a laceration to the L-wrist when employee contacted the end of the broken steel that remained in the hole.

While working in the shaft the miner fell from height while attempting to access the bench to reattach the bail on the muck bucket. The miner landed on both feet on the muck pile and then rolled. Injuries included a hairline fracture to the left Tibia and a sprained ankle. The miner was released to light duty.

The mechanic's left thumb was pinched between the VSM bucket and the cylinder while working to install the retaining pin. The injury required sutures to close the wound.

The employee was welding in the shaft when employee noticed irritation in right eye. The employee received on site first aid and was taken to the Emergency room and diagnosed with a corneal abrasion. The employee received a prescription of antibiotics as a precaution to prevent a possible infection.

The #9 Service Hoist had a #1 motor tachometer failure that resulted in the loss of the hoist operation. There were no miners underground when this occurred. The Hoist was out of service for approximately 4.5 hours.

Janitorial contractor felt dizzy and nausea and rested in restroom. When standing up, EE fainted and struck head on wall. Based on this, initial 15 minute report made to MSHA Ref 01780594. Transported and treated at hospital for heat illness and head bump.

The #9 shaft Auxiliary hoist (secondary hoist) was not functioning properly due to electrical issues. The employees were hoisted out of the shaft using the primary hoist. Once repairs were completed on the Axillary hoist, the crew returned to work in the shaft.

The dywidag bolt driver fell from the recently installed bolt, deflected off of the drill, and hit the employee on the right side of the mouth. The drill helper sustained several broken teeth as a result of the impact.

Shotcrete dust entered the employee's right eye behind the foam lined safety glasses, while chipping dried shotcrete out of a concrete bucket. The employee was treated and released to full duty.

The employee received flash burns to both eyes while working in the vicinity of plasma cutting that was taking place in the shaft. The exact time of exposure was not known. The employee returned to full duty work, post treatment by an eye physician.

On the morning of 3/7/2018 the employee reported that the employee thought something was in eye. Although employee was not sure when the incident occurred employee believed it happened while working in the Shaft on 3/06/2018. The employee was treated and released by a local eye doctor.

While at the kitchen sink, an employee squashed a bee against a window pane with a sponge. The window pane broke and the employee cut the back of EE's left hand. EMT was immediately notified and evaluated the employee. Employee was transported to the hospital and received three stitches and a tetanus shot.

The injured ee was assisting the Jackleg driller by helping pull the machine out of previously drilled hole they were cleaning. The wrist of the helpers glove was caught momentarily in the rotating drill steel, pulling the glove off the L-hand. Tip of the middle finger was amputated as the glove wrapped around the drill steel and the injured pulled back to free his hand.

Employee was holding a length of 3/4" all thread with his left hand & operating the grinder with his right hand. The grinder kicked back and the cutting wheel contacted his left thumb, causing a 1/2 inch cut & severed the tendon on the back of his thumb.

Worker stepped into a sump hole filled with water. Ankle sprain was diagnosed on 08/17/11. On 09/01/11 an avulsion fracture of the ankle was diagnosed rending the injury Reportable. This is medical only. No restricted duty or lost time.

The crew was in the process untying and moving the canopy when a sudden gust of wind violently blew the canopy over. The frame of the canopy struck EE in the left shoulder. He was taken to urgent care in gold canyon by a supervisor. EE was treated and released to return to work on light duty. He was diagnosed with a dislocated shoulder and a cracked rib.

A drill crew was doing a wet trip. The helper was using a bucket to prevent the mud from spraying out of the pipe. The bucket was positioned on the pipe that was hanging in the elevators. As the helper pushed the rod back to direct the mud away, he cut his arm on the pipe that was still in the foot clamps.

Worker lifted the handle to dump the trash hopper. The weight of the trash caused the hopper to dump quickly. The finger was pinched between the bottom of the hopper as it rotated past the handle.

Employee's right hand middle finger tip was pinched between the verticle shaft mucker and the tower frame corner angle iron. Resulted in smashing his finger tip which required suturing.

Worker opened a 4 inch valve on a charged airline to bleed it off. The force of the air releasing out of an 11 degree elbow caused the end of the pipe to move and it struck the worker on the left side of the nose. The edge of the safety glass lens caused a lacertation to the nose.
