Showing all 4Cement silo was not flowing properly, ee shot air into silo to increase flow. EE put hand inside silo and paddle wheel caught fingers on left hand. Index finger was removed by doctor and middle and ring finger were broken but were saved.
EE started a/c unit and a piece of debris flew out and hit him in right eye. Due to the fact the EE was in an enclosed cab, he was not required to wear safety glasses.
I was digging a perimeter drainage ditch in the new pit area. There is some standing water (mabye 2ft deep). This limited the visibility of the actual ditch sides. I apparently got too close to the edge of the ditch and the machine tipped into the ditch, cab side first. The cab strted to fill with water and I (in a panic) kicked out the front window of the cab and crawled through.
