EE WAS WELDING A PLATE ON TOPOF THE 3-DECK SCREEN. AT ABOUT 1855 HE SCREAMED. COWORKER WAS WORKING BELOW HIM AND HE RAN TO SEE WHAT WAS WRONG. HE IN TURN YELLED FOR THE SUPERVISOR. THEY FOUND EE LYING ON THE SCREEN WITH THE WELDING STINGER ON TOP OF HIM. COWORKER THREW THE STINGER OFF, RAN AND TURNED THE WELDER OFF. HE CAME BACK, CHECKED AND FOUND A PULSE. HE THEN RAN AND CALLED 91

Struck against a moving object

Coming off hill with loader, hit a pothole with loader, bucket went into ground causing EE to be thrown forward and hit the steering wheel.

Fall to the walkway or working surface

EE was walking across the drag line bench. EE slipped down. EE states that when EE stood up EE felt pain in the groin. EE was seen at DR the next day. No treatment. EE reports no improvement. Dr listed restrictions that could not be met on 04/12/2018. EE missed first shift on 04/16/2018. Follow up with general surgeon on 04/24/2018.

Contact with heat

Employee became overheated at the job site. His supervisor offered him hydration and put him in the truck to cool down. It became apparent that he may need a doctor because of pre-existing medical conditions and the fact that he was not taking his blood pressure medications. The hospital treated for the heat stroke, but kept him for other diagnostics.

Employee placed hand on hot metal, burning first finger on right hand.

Over-exertion in lifting objects

Employee's were relocating 55-gallon drums from the ""grease rack' area to a storage trailer. They were using a dolly to move the drums. A drum must be moved from the dolly into the trailer, which sits 6"" off the ground. When the employees lifted a drum, EE felt something pull in his upper left side area.

Struck against a moving object

I was digging a perimeter drainage ditch in the new pit area. There is some standing water (mabye 2ft deep). This limited the visibility of the actual ditch sides. I apparently got too close to the edge of the ditch and the machine tipped into the ditch, cab side first. The cab strted to fill with water and I (in a panic) kicked out the front window of the cab and crawled through.

Accident type, without injuries

Employee was pulling around some ruts in the pit to get to the dragline for greasing. When he was finished, he started backing up. He thought he was following the same tracks he used coming in, but instead backed up roughly 60 ft staright into the pit. Another employee was able to get him out of the truck, but as soon as he was out, the truck went into the pit and was submerged.

Employee was lifting (with a truck-mounted hoist) a piece of cutting edge to be installed on the bucket of the Komatsu WA-500 endloader. When the cutting edge was nearly in place, the pin fell out of the lifting chain, causing the cutting edge to scrape along his thigh and shin.

Employee stated that he was removing rocks from the hopper and felt a snap in his side, he continued to work telling a couple of co-workers, but failed to tell his supervisor untill the following week, EE had been to Dr and thay are stating that he has abd. hernia.

Employee fell after stepping off roadbase catwalk; looked to see how area around tail section had been washed away to allow unit to turn, as he hopped down with his left foot, ground gave away. He turned to his right trying not to fall on conveyor, he fell to the ground on left side. Said that his left lower back was in pain.

Employee was on catwalk and hit his left knee. Knee became swollen and hurting.

Employee was swinging sledge hammer, missed and hit finger. Finger required stitches and end of finger fractured (3rd finger left hand).

Employee was swingin a sledge hammer at the primary plant. He started getting light-headed & pains in his chest. He went over to the belt, sat down, and then passed out. One of other employees went to check on him and called 911.

EE was inside the cage for cone crusher. The floor is wet and full of rock and mud. He slipped and fell against oil tank and hit neck and left shoulder. I finished the day but on Saturday, I could not move my head. On Monday, I was in pain and went to the emergency room.

I was in the process of routine maintenence. I was began greasing the bearings on the conveyor tail pulley's when I slipped in a puddle of mud. I lost my balance and landed on my right foot. I heard a pop sound and a sharp pain shot thru my ankle. As I tried to stand up the pain became unbearable. I fell back to the ground and asked for help. 2 men came & put me into control room.

Struck by flying object

Employee states: On Friday July 7th approx 8pm while working on a broken cable I was swinging a sledge hammer to knock out the wedge to remove the cable and I felt something explode in my right arm. I thought I pulled a muscle so I continued to work. Then 1 hour later I took off my rain jacket and then I seen all the blood and the hole through my jacket and both t-shirts.

The miner fell getting off the dragline. He tried to push himself away from the tracks and got his right hand caught in hand rail.

Employee was standing behind the feed belt to the stacker, washing off the concrete. He went to climb up the belt when his arm starting burning. He snatched his arm back to see a snake fall to his feet and run away. Was taken to the ER, treated and released.

I was walking on the clay belt catwalk, when I walked up to the clay deck. I had my hand on the rail. I slipped on the step and my armpit got caught on the hand rail and twisted my shoulder & back when I fell.

Was getting off dragline at the end of the day, stepped in some grease on the track and slipped and fell on the ground. Fell on the ground but my head hit on the left side.
