The employee was helping a mechanic repair a piece of equipment. While holding the nut on one side of a bolt, the mechanic tightened the bolt from the opposite end. The employee did not maintain a secure grip on the bolt, resulting in the bolt slicing the side of their right middle finger.

A conveyor belt had been set on the ground by lifting equipment, the employee went onto the belt of the conveyor to unhook the lifting chains, the lifting equipment subsequently bumped the conveyor while the employee was till on the conveyor belt, and the employee fell onto the ground.
