The employee was helping a mechanic repair a piece of equipment. While holding the nut on one side of a bolt, the mechanic tightened the bolt from the opposite end. The employee did not maintain a secure grip on the bolt, resulting in the bolt slicing the side of their right middle finger.

Struck against stationary object

While doing maintenance on an Excavator and trying to unscrew a bolt, employee's left arm caught onto a piece of metal causing a 6-8 cm laceration to the left distal forearm.

Fall from machine

Employee was assisting a team of 5 with moving of conveyor belts to a different area within our property. Employee climbed onto conveyor belt #2 to guide the operator with conveyor #1. While moving, Conveyor #1 got caught on a rock and swung back hitting conveyor #2, causing Employee to fall.

The employee jumped off of a feeder hopper into the rocks and twisted and bruised his knee. No bones were broke and nothing was tore in the knee, just bad bruising.

Fall from machine

A conveyor belt had been set on the ground by lifting equipment, the employee went onto the belt of the conveyor to unhook the lifting chains, the lifting equipment subsequently bumped the conveyor while the employee was till on the conveyor belt, and the employee fell onto the ground.
