Showing all 4IE and another EE were removing the steels from a Fast 65 hole. The bottom steel didn't come loose and when it turned the wrench hit IE in the left ankle.
EE was moving wire when another employee attempted to start the wire saw twisting EE's finger. EE said the accident happened on 9-16-2013 but did not report the accident to anyone until 12-16-2013, 3 months later. He was at work everyday since 9-16-2013 and continues to work up to the time of this report. EE went to the doctor on and they said he might need surgery.
EE was using a jackhammer when the jackhammer spun around and the handle hit EE in the chin.
