Employee stated was tramming a bolter when the right rear tire slipped off the track causing the operator to be thrown up into the operator's deck canopy injuring neck and back. This section is currently moving across the mine. Employee sought medical treatment and was taken off work due to neck and back strain.

Employee stated was operating the Joy 14/15 C/M machine on the Unit 1 Section. In the process of operating the machine cleaning up some gob a rock flew off of the C/M head and struck the employee in the mouth. This caused EE's tooth to be chipped. NOTE: This employee didn't seek medical treatment until 4/8/2019 and that is why it is now being reported.

Employee stated ee was operating a roof bolting machine on the Unit 2 Section. EE was in the process of installing a roof bolt. In the process of installing the bolt, the pie pan plate came through the head of the bolt striking the employee in the nose.

Employee was removing lug nuts from a 789 rock truck wheel using an impact wrench. Wrench kicked back and struck employee in the mouth causing damage to several of their teeth.

Employee was installing a roof bolt and inadvertently engaged rotation while hand was on bolt. The bolt caught on glove and ee cut hand and fractured right thumb. EE started missing work on 11/2/2018.

Employee stated that ee was operating the 30 ton crane on the top floor of the prep plant that was hooked to the metal bin. In the process of operating the crane to empty the metal bin, it swung and caught arm between the separator and the bin.

Employee was operating a continuous miner when EE was struck by a roof bolter trailing cable that was knocked down by a rock that was coming out of the conveyor chain of the miner which resulted in a laceration that required 5 sutures. No lost or restricted work days.

The employee stated ee was in the number three entry and in the process of swinging back in after installing a rib bolt when right knee made a pop sound. The employee was taken via by ambulance to hospital where ee was diagnosed what a torn ACL. This incident is being reported as a Lost Time Accident.

Employee was operating a roof bolter when ee tried to catch drill steel that was falling out of hole and struck left hand against the drill pot causing left pinky finger to become dislocated. This did not result in any lost or restricted days.

Employee stated was backing up with the grader and did not see the rock truck due to the standard inside the cab of the grader. The front of the rock truck caught the ripper arm of the grader spinning the grader around and hitting the back tire of the rock truck. This caused EE to fracture 3 ribs.

Employee was drilling a test hole with stack steel, when removed the bottom piece of steel the top piece fell out of hole striking ee left ring finger resulting in a laceration that required 10 sutures. No lost work days.

Employee stated that EE was attempting to tighten a huck bolt using a huck gun in the de-slime screen beam. When the employee pressed the trigger to tighten the bolt EE didn't have the gun all the way on the bolt so instead of the gun drawing up on the bolt, it shoved down smashing the fingers of EE's right hand between the gun and the 8" I-beam below.

Employee stated was attempting to install a roof bolt that was partially bent, when employee activated the rotation to line the bolt up in the drill chuck, the corner of the bearing plate struck the employee in the right forearm causing a laceration. The employee received medical treatment (3 sutures) and returned to the mine for next scheduled shift.

The Employee stated was standing in a cross cut as the bolter was backing up EE went to reach for the cable and toes of left foot were ran over by the bolter. Employee finished the shift, and on 11/13/2017, Employee sought medical treatment and was taken off work. This incident is reported as a Lost Time Accident.

Employee was tightening lug nut on the front wheel of a rock truck. EE was holding the wheel cleat with one hand and tightening nut with an impact wrench using the other hand. The wheel cleat slipped and crushed EE's finger.

The employee stated EE was bolting in the number 6 entry on 2-Section when a piece of steel fell out of the hole causing a laceration to EE's left middle finger. The employee was transported to CAMC Memorial where EE receives two sutures to EE's middle finger and returns to work next scheduled shift. This is a reportable medical incident.

Employee stated EE was installing roof support when the bolt would not go in the top. Employee stated EE placed the bolt onto the side of the pot to assist the bolt into the hole. The bolt jammed and slipped off the pot, pinching EE's right middle finger, resulting in a laceration that required stitches. No lost work days.

Employee stated that while pushing refuse with a D9N Dozer, the dozer trammed over an offset in the material causing employee's neck to jar. this resulted in pain in employee's neck and right shoulder. The employee missed one scheduled work day and returned to work with a full release.

Employee stated was using a battery powered leaf blower to blow dust out of #3 D-Box and felt a small piece of dust or unknown object enter right corner of right eye. The employee went to optometrist after work and was prescribed prescription eye drops. This incident is being reported as a medical reportable due to the prescription eye drops.

Employee was operating a roof bolter on the l Left Section. EE had drilled 5ft of a 12ft hole and added a pusher to complete the hole. EE started rotation and the kevlar sleeve became entangled on the drill chuck wrapping EE's arm around the drill steel resulting in a broken Humerus bone in left arm.

Employee stated EE was installing roof bolts on the No. 2 Section in the No. 5 Entry. EE drilled the hole, inserted the glue and was placing the bolt head into the wrench. EE's left hand was pinched between the bolt/wrench, and the rotation was engaged. This resulted in the amputation of the tip of the left pinky finger, and dislocation and laceration of the left ring finger.

The employee stated that as EE was lowering drill pot on the roof bolt machine. EE started the rotation and the bolt wrench came out and struck right wrist. The employee sought medical attention and diagnosed with a contusion, however the incident became reportable on 6/30/2016 we EE did not return to work. The mine was idle on the 1st and 4th.

The employee stated that EE had been cutting and welding on a new rock box for the new 3 South belt head on the 5th. EE stated EE felt fine at the end of the shift and went home and had pain in EE's eyes when EE woke up the next day. EE sought medical attention and was diagnosed with burns due to viewing welding arcs and was placed off work for 1 week.

The employee stated EE was installing belt hangers when EE reached for EE's wrench that had fell over on the drill pot when EE's drill steel fell down striking the tip of right ring finger. The steel fractured the tip of EE's finger and caused a laceration that required sutures. EE returned to work EE's next shift. This is a medical reportable incident.

As employee dropped mast the bolt fell over and the mast kicked it back towards employee causing the plate to lacerate left jaw. Employee sought medical attention and received 6 sutures and returned to work at normal job.
