While using a one-handed reciprocating saw, the blade hopped out of the rubber liner that was being cut and punctured a maintenance employee's hand.

While cleaning cement out of haul truck bed, employee was struck on the eye by an air hose that came away from the air chipper. Employee received first aid by security and was taken to the hospital for further treatment; employee was prescribed medication and days away from work.

Driller Assistant was pulling the wireline so it could be cut and re-crimped. EE could not pull any more slack so driller pushed forward on controls to let out more slack and the wireline abruptly retracted catching assistant's left hand between the stuffing box and the sheave wheel of the drill.

The Grader operator was pushing material to the right rib at roughly 3mph when a rock measuring 1'X 1.5'X 1.5' rolled over top of the blade and then under right rear tires of the Grader. When this occurred the seat-belted operator's right elbow hit the armrest forcefully, causing a sprain to the employee's right shoulder. The full extent of the injury is currently unknown.

The drill helper was holding an 8' drill steel while attempting to install it into the chuck of the Jackleg drill. The operator applied air to the leg to assist with the connection and the employees left hand was pinched between the steel and the machine.

The employee was using an Angle grinder when the material ee was working on came loose from the clamp and moved. The employee reached to grab the clamp and at the same time the grinder kicked contacting left thumb. The employees left thumb received a small laceration due to the contact.

Employee was air lancing a chute in the crushing circuit when the Chicago fitting separated and struck employee in lip. Employee was taken to ER where 4 nylon sutures were administered.

Employee was working out of Genie boom basket on corner of the building. He reached out to grind and strained a muscle in his left shoulder. No medical attention at time of injury. He is now scheduled for rotator cuff surgery on 4/16/14
