While an employee was standing up from their workstation, they hit their elbow on the corner of the table. After a short period of time, the affected area began to swell up and the employee was evaluated at the local hospital where they were prescribed days off of work.

While using a wrench to loosen a nut, the wrench slipped off the nut and the maintenance worker struck their hand on a bracket injuring their hand.

The employee was exiting the administration building through the East door with a bag of garbage over their shoulder. When stepping down the steps they took them both in one stride and landed on the side of their left foot, resulting in a strain to the left ankle. Employee stated that they were holding the handrail at the time.

Employee was moving an empty 55 gallon barrel that was being used to collect material being cleaned up in the mill when they felt a muscle pull in their groin area.

Fall onto or against objects

Two chipped teeth when falling forward after a railcar mover made impact with a rail car. The employee was standing up in the railcar mover looking out the window to see how close they were to the staged railcar. The impact resulted in the employee losing their balance and they fell forward into the window chipping their two front teeth.

Low back injury when lifting pipe into an elevated hangar. Physical therapy was prescribed at later date.

There was ice that was discharged in the area where our loaders are parked. After EE's shift, EE was getting down off of the loader, and EE's right foot slipped out and to the right. EE's left foot slipped off the ladder, but EE held on to the ladder and did not fall completely.

Struck by flying object

Loader operator turned on the air conditioning in a front end loader and proceeded to open the vent when dust was blown into their right eye. The eye was flushed but irritation persisted. After a medical evaluation the employee was prescribed antibiotic eye drops for the irritation and to prevent the possibility of infection. The operator was wearing glasses.

An employee was washing their hands in a bathroom sink. When the employee reached for the faucet the employees right hand contacted the bottom edge of the soap dish that was mounted near the sink faucet causing a one inch long laceration on the employees right knuckle. Four sutures were received.

While reinstalling a gear box Employee strained groin.

An employee was performing routine seated work in the security building. The employee experienced an instant pinch and sharp pain in the right shoulder when twisting their upper body and raising their arm. The employee was evaluated at UP Health System-Bell and received work restrictions and a sling. The employee also sought physical therapy as recommended by the physician.

Employee was attempting to remove an impeller from a slurry pump. Impeller broke free from the shaft allowing it to spin counter clockwise. Shaft continued to rotate pinching employees index finger.

Struck by falling object

Employee was moving a ladder which contacted a light fixture causing the fixture to break. The broken fixture struck the employee on the nose and upper lip. Employee received 8 stitches.

Fall to the walkway or working surface

Employee was helping to pull an electrical cable by hand with a pulling rope. While pulling the rope broke and the three employees fell at ground level. When they fell, their legs got tangled and caused a hairline fracture in the employees right ankle.

Employee was air lancing a chute in the crushing circuit when the Chicago fitting separated and struck employee in lip. Employee was taken to ER where 4 nylon sutures were administered.

Flash burns (electric)

Electrician received partial thickness burns to right hand when the lugs on the MCC became energized while doing maintenance work.

Employee hit his left thumb while dumping a hopper.

Two employees were working on replacing the packing on the nickel Filter feed standby pump, they were installing the packing gland and while one employee was pushing the gland in with a pry bar, the pry bar slipped and struck the second employee's hand while was securing the fasteners.

An operator was cleaning underneath the copper thickener using a water hose and while squatting the employee felt a pain in the left knee. Employee was evaluated by on-site medical care and then taken for further evaluation to Bell Hospital. The employee received work restrictions.

Employee was terminating cables inside a junction box and installing tags. He attempted to adjust the tag with a knife. The knife slipped and allegedly cut his left index finger

EE sweeping on top of transfer conveyor and experienced irritation in rt eye. A small piece of debris was observed & flushing was attempted onsite. He was seen at the local ER on 4/23/14 at shift end. He continued to have irritation & requested to be seen again on 4/28/14. Returned to full duty following both visits. Was prescribed medicine (eye drops) at 4/28/14 visit.

Employee was working with another employee to roll a 12' beam from the H position to the I position. The employee pinched his right thumb between the beam he was handling and a beam they already had standing in the I position. His thumb was fractured and stitches required.
