Injured employee was holding angle iron while another drilled a hole in it in order to mount a sign. The drill bit pierced the metal, caught employee's glove, and caused a puncture wound in hand. Injured employee driven to the office where the hand was washed and bandages applied. Employee was taken to the emergency room where they received 8 stitches and was released to go home.

Showing all 3Employee was operating 336 excavator near surge pile. Weather was clear & dry. Employee loaded rock truck with 2 buckets of material. As EE turned to get third bucket, rocks & boulders slid down and hit excavator. A boulder crashed through side window, hitting employee's shoulder and showering EE with broken glass. Employee bruised shoulder and had minor lacerations from glass.
EE broke a bone in foot after a rock from the crusher fell on it. EE went to close a side door on the crusher without properly shutting it down, first. EE was told by the plant manager to shut it down first, but EE did not. The plant manager saw EE approaching the machine while still running, and pressed the e-stop, but EE had already been hit by the rock.
