Over-exertion (Not Elsewhere Classified)

Employee reported having back pain to EE's supervisor. EE was about to grease EE's loader, but hadn't started yet, when EE began to feel pain. EE does not know what caused it. EE was not doing anything when the pain started. Axis care line was called. Nurse advised rest, OTC pain meds, & Ice therapy. Employee was sent home at 12:40 to rest and follow nurses care plan.

Struck against a moving object

EE texted Plant supervisor that the excavator operator was dropping large rocks too hard in EE's haul truck. EE did not report and injury at that time but after EE's shift EE called HR and said EE's back hurt. EE to Texas med clinic and the ER and was diagnosed with possible muscle and vertebrae injuries.

Fall from machine

EE was tightening a bolt on the Inertia Hawk Crusher with a wrench and ratchet. The ratchet slipped, EE lost balance and fell to the ground injuring left leg. EMS was called. EE was taken to the hospital.

Contact with heat

EE came to the office feeling sick- dizziness and muscle spasms. EE was unable to drive home. EMS Came to check EE. EE refused the hospital so EMT suggested Urgent care. EE was seen by Urgent Care doctor and diagnosed with dehydration & low electrolytes. All tests including Covid & Flu came back negative. Doctor gave EE a steroid shot, told EE to rest & drink lots of fluids.

Injured employee was holding angle iron while another drilled a hole in it in order to mount a sign. The drill bit pierced the metal, caught employee's glove, and caused a puncture wound in hand. Injured employee driven to the office where the hand was washed and bandages applied. Employee was taken to the emergency room where they received 8 stitches and was released to go home.

Employee was operating 336 excavator near surge pile. Weather was clear & dry. Employee loaded rock truck with 2 buckets of material. As EE turned to get third bucket, rocks & boulders slid down and hit excavator. A boulder crashed through side window, hitting employee's shoulder and showering EE with broken glass. Employee bruised shoulder and had minor lacerations from glass.

Struck by falling object

EE broke a bone in foot after a rock from the crusher fell on it. EE went to close a side door on the crusher without properly shutting it down, first. EE was told by the plant manager to shut it down first, but EE did not. The plant manager saw EE approaching the machine while still running, and pressed the e-stop, but EE had already been hit by the rock.
