Employee was standing near main shaker screen observing operations. A piece of oversized rock was ejected out from main shaker and fell approximately 60 feet striking employee on crown of head causing laceration. Laceration was closed with glue.

Showing all 3Employee was helping install discharge pipe on sand pump. Trackhoe operator raised pipe so gasket could be installed between flanges. Employee was holding a bolt in place. The pipe shifted pushing the bolt between EE's ring and middle fingers on EE's left hand. Hand was lacerated and required two stitches.
Employee attempted to do a spot weld without properly positioning welding helmet. EE suffered irritation to both eyes. EE was welding on a gravel trailer.
