A miner was struck in the ribs with a drill steel that was hidden under the muck in a horizontal position. When the backhoe operator raised EE's outrigger it grabbed the steel causing it to twist up out of the muck striking the miner (right side ribs).
Employee was center bolting when EE was reaching for plate and EE's finger got caught in drill guide. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Employee was running shearer when a rock came off the drum and struck EE's hand. Mining systems was not a factor, compliance with rules and regulations was not a factor. PPE was a factor.
Employee was installing center bolts using the Fletcher Roof Bolter. Had drilled EE's starter hole and just changed drill steels and when EE started drilling again, a piece of rock fell from the top and bounced off the drill head and struck EE's left forearm. Suffered a contusion with swelling. Missed 3 days of work. Has training and experience and was wearing required PPE.
Employee got EE's left hand caught between the auger and wrench while rib pinning, resulting in a laceration to EE's left hand. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.
Employee was drilled hole for rib pin and turned around to grab strap when piece of rib hit EE's back and shoulder. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Employee was helping use cobra drill when drill was collapsing, they strained their shoulder. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor. Employee did not start missing until April 6th.
While mining inby crosscut 23 of the #1 entry in 8 West injured was standing by rib bolting controls when a rock fell out from in between the roof straps and struck them in the back causing a closed fracture of spinous process of thoracic vertebra. No problems with equipment or compliance issues, training or PPE.
Injured employee hooked a scoop winch hook to a shield on a 30-Ton car to center up the shield on the car. When the scoop operator tightened up the winch rope, the hook slipped off the shield and struck employee's right leg above the knee. They suffered a contusion/bruising, pain & swelling. Employee failed to position themselves in a safe location.
Employee was rib pinning when vent tube fell and knocked them down, causing them to injure a hand, PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Injured was using 28 center bolter in St Leo Mains 3-2 at 148 block. They had a drill steel laying on the tray above the drill controls and it was struck by the boom, flipping up and hitting them in chest resulting in a contusion. No problems with equipment or compliance issues, training or PPE.
Employee had finished bolting on EE's side and went to assist EE's bolting partner who was struggling with setting EE's drill steels out of the top. EE asked EE's bolting partner to lower the drill pod a little bit but instead EE brought it down all the way on top of the employees right foot.
Injured holding up airline and cables at 97 blk 5 belt while another employee was using grinder to cut of belt structure nuts when the grinder caught and kicked striking EE's left index finger and causing a laceration that required 5 sutures. No problems with equipment or compliance issues, training or PPE.
Employee was using a grinder to cut a bolt when the grinder disk came apart and struck EE in the left hand resulting in a laceration. The injury resulted in 5 sutures.
Employee was bolting off miner when steels fell out of top and struck their knee. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Employee was working on a section fan that was just unloaded at the end of the track. A Trip of cars was being moved when it struck a motor and dolly at the end of the track, causing the dolly to come off track and hit fan. The fan moved sideways, causing the fan to pinch miner between coal rib and fan.
Employee was assisting installing J-screws into rib. When the bit on the drill became lodged and stuck, employee attempted to free it by pulling on the drill. Employee's hand slipped onto the bit, which spun and injured the employee's hand. Injury was not the result of a training or compliance issue.
Employee was lining and leveling the belt when a the jack kicked out and struck their left thumb, resulting in a fracture. Compliance with rules/regs, PPE, mining equip/systems not a factor. Miner proficiency may be a factor.

While guiding roof bolt sections (#6 bolter Sugar Run Rehab) together (with bolt head in chuck) the drill head rose slightly when the other drill removed up pressure causing their left ring finger to get pinched between bolt sections resulting in an open wound and fracture to distal phalange. No problems with equipment or compliance issues, training or PPE

Injured employee was roof bolting on the left side of the continuous miner. Employee was in the process of installing a rib bolt when a piece of coal/rock fell from the brow behind them and struck them on the left shoulder. Was diagnosed with a deep tissue contusion and given time off. No rules or regulations were violated.
Employee was z drilling on belt line when EE squeezed trigger and felt pop in wrist. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Injured operating fletcher center bolter in motor barn rehab area between #3 and #4 seal installing a bolt in a bacon strap and had left hand resting on drill mast. As mast went up it pinched the tip of EE's middle finger in the mast resulting in a crushing injury that required 6 sutures to close the wound. No problems with equipment or compliance issues, training or PPE.

Employee was engaged in the mucking phase of shaft construction. A bucket of muck was sent out of the shaft when a rock fell striking them in the hardhat. This resulted in the employee having a small laceration which needed 2 staples and a mild concussion.

Employee was z drilling and went to bring the bolt up, when the spin got caught. The drill head and cylinder snapped off and hit their right hand.
Injured was roof bolting on the 36 1/2 block main west roof fall with an air drill when some head coal around an adjacent roof bolt fell out and allowed a bowling ball sized rock to slide out and strike them in the upper back, resulting in a possible muscle strain/sprain (no bruising, x-ray and CT scan negative). No problems with equipment or compliance issues, training or PPE.
Employee was installing a rib bolt when the rib strap swung down hitting their hand and cutting a finger through their glove. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Employee was operating a hammer drill and struck themselves in the mouth causing them to chip their right front tooth.

Injured was assisting panline bolter operator near headgate of 5W longwall recovery face, attempting to install a bolt wrench in the bolter head when their right foot slipped and went under bolter head while it was lowering, pinching their foot between bolter head and mine floor, resulting in a contusion. No problems with equipment or compliance issues, training or PPE.
Injured installing rib bolt with 3' metal rib strap off right side of miner 41 block 1-2 crosscut 7W when the strap rotated from vertical towards horizontal and while trying to catch it, it struck EE's right wrist resulting in a laceration. Bolt was not rotating at time. No problem with equipment or compliance issues, training or PPE.
Employee was center bolting when EE's bolt wrench broke and hit EE on EE's forearm. PPE was not a factor, mining system was not a factor, compliance with rules and regulations was not a factor.

Employee was bolting off the miner when they leaned down and was struck by a piece of slate rock on the back of the head. PPE was not a factor, mining system was not a factor, compliance with rules and regulations was not a factor. Body positioning was a factor.
Injured employee and co-worker were using a small crane mounted on a mantrip to lift and load a shuttle car reel on the end of the mantrip. As the reel was suspended it became stuck on the edge of the mantrip platform. The employees were attempting to free it up when the reel slipped off the hoist hook and fell on injured employee's right foot. Suffered a contusion and bone bruise.

Employee was bolting off the miner when the vent slider hit them on top of the head. PPE was not a factor. Mining systems was not a factor. Compliance with rules and regulations was not a factor.
Employee was trimming off a piece of regulator beam when grinder kicked back and caused abrasion on EE's leg. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.
Employee was bolting off the right side of the miner when EE was pushing a bolt up and the plate shifted. EE then reached up to adjust the plate but was still pushing the bolt up causing EE to pin EE's hand between the plate and the strap. EE had skin abrasions on EE's hand.
Employee was using an angle grinder to cut a piece of metal angle. The grinder slipped while cutting and contacted their left upper leg causing a cut. Employee received 4 stitches to close the wound. Returned to work their next shift. Employee failed to maintain control of the grinder. No rules or regulations were violated and employee has training and experience.
