Employee reached into bag machine to remove jam when the belt grabbed EE's glove and pulled it into the belt pulley. The injury occurred when the employee reacted by pulling EE's hand out.

Employee was changing v-belts on hummer screen when the counterweight moved. The employee's glove was caught by the v-belt causing a finger to be pinched between the belt and the belt pulley.

Loading 29IN face with #8 powder rig, employee was loading the top right corner of the face. The basket tilt cylinder rod snapped causing the basket to rotate forward freely 90 degrees. causing a laceration on right knee

Employee was operating a Fletcher Man Up Bolter at 6L 75 Block. When loading the steel back into the mast the employee had hand on the steel and closed the finger clamps on thumb. Employee fractured the tip of thumb and ruptured the nail. Employee returned to work with restrictions.
