EMPLOYEE WAS CUTTING A ZIP TIE AND CUT ARM. EMPLOYEE RECEIVED 5 STAPLES
Underground mechanic was traversing from the continuous miner onto a 4 foot tall step ladder. The mechanic had EE's left foot on the top step and was placing EE's right foot on the next step. When EE put EE's weight on EEs right foot, the ladder shifted and EE fell approximately four feet to the ground, landing on EEs right shoulder. This injury resulted in a fractured collar bone.
Office cleaning contractor sat in their truck to take a break and was found approximately one hour later deceased. Originally, a report was submitted to MSHA on 11/20/2025.
While erecting the C-3 take-up structure two employees were holding up an 8'x 8' column to have 2 bolts installed by another employee on the ground when they lost their hold on the column. It fell over and struck a support beam that was laying on the ground causing the column to bounce up and the corner of the column hit the employee in the face as EE tried to move out of the way.
Employee was changing v-belts on hummer screen when the counterweight moved. The employee's glove was caught by the v-belt causing a finger to be pinched between the belt and the belt pulley.
The injured was putting the thimble on the new skip and noticed that it needed a couple more shims. While pushing the pin out, thumb entered the hole and was injured between the thimble and the draw bar.
Employee reported neck pain while using an LHD to clean up faces for drills and augers. Upon evaluation, the employee was diagnosed with a fracture. Work relatedness determination was made on 12/29/2025.

Production skip components failed causing the hoist to be unavailable for egress.

Tracking and arcing developed on the MMT-3 transformer, faulting the transformer phase to phase. The vacuum breaker at MMT-1 failed to open to disconnect power to the underground power system. Resulted in a fire on the Neutral Ground Resistor on MMT-1 and a failed 12.47kV cable in the mine.
Miner hit another miner with EE's fist fracturing EE's jaw
Employee was walking to job site, slipped on salt, injuring their left quadricep. Diagnosed with a tear of the left distal quadricep tendon.
Employee was getting off the vertical drill and slipped off the last rung of the entry ladder. Employee fell on employee's right side and arm. Employee was taken to the ER as a precautionary method and x-rays were negative for rib injury.
Removing drill motor when it fell on foot of the miner.
Hydraulic hoses on the ground made a slip trip and fall. After reviewing by doctor a torn Achilles tendon was found.
Twisted while working and strained back.
Blasted and gas hit AMS system and knocked out power. Around 4:30AM we found it was gas and not a malfunction on the AMS system.
Fire was found once machine killed and lost all power. Fire Suppression and Fire Extinguisher was not used from investigation with the size of the fire and mine was evacuated.

The employee was walking near the maintenance shop when EE tripped over a flume cover and fell to the ground. EE put hand down to catch self and felt pain in the left wrist almost immediately. EE had a wrist x-ray showing a strain.
A small fire occurred inside the compressor housing on AL2 powder rig. The fire was able to be extinguished at the time it occurred.
Shaft crew identified a gap between the Capel and skip attachment plates.
During a routine inspection, the shaft crew found three broken wires on the North rope.
