A hoisted dragline drag blower motor was being lowered down while suspended, the load shifted and made contact with the employee.

The cutoff wheel fractured during grinding, and a fragment struck the employee's right arm, causing a laceration that required sutures to close. The disc had the proper RPM rating for the grinder, and the employee was wearing all necessary PPE at the time of the incident.

EE was pushing dirt off a coal fender to bury warm coal when the fender gave out and the dozer slide off sideways. The dozer overturned landing upright on the tracks. EE sustained a 1 inch laceration on their scalp requiring a suture to close.

While tightening the chuck on a lathe for a bogie wheel shaft, the lathe slipped, causing the worker's finger to be caught and crushed between the chuck handle and the lathe.

Employee was holding onto a combination wrench while another employee used an impact wrench to tighten bolts on an 18" valve. While tightening the bolts, the combination wrench unexpectedly moved causing EE's right index finger to become wedge between the wrench and valve flange, resulting in lacerations.

EE was greasing a pin with a pneumatic grease gun when the adapter slipped off the fitting and injected a small amount of grease into the EE's LH index finger. Debridement and stiches were required to treat the wound.

EE was cutting a piece of wear plate off the dragline shoe. When it fell to the ground it cut through EE's boot causing a small laceration on two toes. Stitches were required to close the wound.

EE was using a hydraulic tool to remove a wheel lock ring on a RTD. The tool slipped off of it's position landing on the EEs Rh index finger causing a laceration that required stitches.

Employee was preparing a coal sample by pulverizing it in the pulverizer. When pushing down on the lever with left hand to lower the plate to replace drawer with right hand employee felt a "pop" in left wrist.

Employee was changing cutting edges on 24H grader, while using impact to remove hardware, a nut came out of socket, bounced off the ground and hit employee in the lower lip resulting in a chipped tooth.

While loosening lug nuts on a haul truck tire, an employee pinched their finger between a rad gun reaction arm and the tire rim. The employee was wearing gloves at the time of the injury.
