1 of 3 drive belts broke, injured shut motor down, dismounted the crusher and approached the large pulley driving the crusher in the hopper. Pulley had slowed down but had not totally stopped rotating yet. Reached around from behind the guard to grab the broken belt from the back when the remaining 2 belts caught his arm between them and the pulley causing a fracture.

While hauling overburden, the injured was backing up for the shovel to load him and the shovel bucket hit the bed of his end dump truck, jarring and straining the injured's neck. He missed one full shift and went home early during another shift.

While steam cleaning a piece of equipment, the injured accidently squeezed the trigger of the wand and hit the right side of his right foot. The outside of the foot received 1st and 2nd degree burns.

WHILE WELDING AND GRINDING ON A 773 END DUMP TRUCK FRAME, A PIECE OF METAL LODGED IN HIS EYE. HE WAS UNAWARE UNTIL 1-20 WHEN IRRITATION SENT HIM FOR MEDICAL HELP TO REMOVE A PIECE OF METAL. RE CEIVED A PRESCRIPTION FOR PAIN PILLS.

WHILE BACKING HIS DOZER, THE INJURED BACKED OVER A RISE AT THE REAR CAUSING HIM TO HIT HIS HEAD ON THE BACK OF THE DOZER AND HIS CHEST. HE RECEIVED STITCHES IN HIS HEAD AND BRUISED HIS CHEST.

WHILE HELPING TO MOVE POWER CABLE, THE INJURED GOT HIS FOOT CAUGHT IN A LOOP OF CABLE BEING PULLED BY A BULLDOZER CAUSING A SPRAIN TO HIS RIGHTANKLE.
