Mechanic was using impact gun to remove cutting edges from dozer. The nut got hung-up on the bolt and the bolt head became unseated from the recessed hole of cutting edge. The spinning head contacted the mechanic's gloved left hand causing a laceration requiring four sutures.

Employee was assisting a miner installing split-set bolts. The miner turned on the feed pressure to the pneumatic stopper leg to drive in a bolt. The bolt driver and the split-set were out of alignment and the driver caught on the edge of the bolt. The helper bumped the driver with his hand and the bolt driver went into the bolt, pinching the palm of his left hand.

Employee was using a six ton bottle jack to lift concrete form in place, when employee tried jacking up the jack, it came apart at the base and sprayed out hydraulic oil into the employees face. No RWC; No DAFW; employee was released same day with antibiotic.

An employee was using a small band saw in the shop to cut off the end of a pipe nipple. Employee was using his gloved hand to saw off the end of the nipple when his hand slipped and went into the blade cutting through the glove and into the end of his right thumb.

After completing a sanding job on a piece of equipment, employee removed facial PPE, and felt irritation to his right eye. No days missed, no restricted work. Final diagnosis given during follow up evaluation on 5-24-11.

The employee was spot bolting when the drill steel was approximately three feet in the hole, and it struck another bolt causing the machine to rotate jerking the employee's right arm. The employee was able to shut off the machine in a timely manner, and continued working. After final evaluation and determination by physician, surgery was scheduled and became a DAFW on 1-26-12.

Employee was standing on a 14" step stool to weld on a loader fender. He lost his balance. While attempting to regain balance, his right hand knocked off his welding hood and the tip of the welding rod made contact with his right eye. No lost time.

Employee slipped and fell while drilling with jackleg. The jackleg fell towards the rib and pinched his finger against the rib. As result of incident, employee's last digit of pinky finger on left hand was amputated.

Employee was preparing to drill a hole in the overburden. When applying down pressure, the pressure regulator apparently did not work and created full down pressure on the drill steel. This action lifted the drill off its jacks causing the drill to tip over.

Employee was installing a self tapping screw with a hand held drill. The drill was equipped with a threaded hole for a side handle which was not installed. When the screw "grabbed", the torque twisted his arm and shoulder. He continued normal maintenance activity until the day he went for shoulder surgery.

Employee was bolting a heading with split sets. The split set driver momentarily stuck inside the set allowing the air to bleed off causing the Jack Leg to drop quickly. He tried to catch the falling Jack Leg. He felt a "popping" sensation in his left shoulder. Employee had surgery on his shoulder on 1/19/07.

EE was operating a dozer 0-10 pushing and ripping overburden. after his shift was over he noticed his back being sore, which the ee alleges was caused from the roughness of the dozer and type of work he had done. the following day he went to the dr and received a cortizone shot to relieve the pan. the dr did not restrict the ee from any work.

Employee was placing walking cam lube bags on the 1370-W walking cam frame while the dragline was walking. While placing a bag on the cam frame, he slipped and the roller passed over his right hand. The right hand was partially, traumatically amputated from the tip of the right index finger to the knuckle of the pinky finger.

Employee stated he was roof bolting in the face area and when starting the drill steel a piece of scale rock fell, striking him on the left knee. EE's first lost work day did not occur until 6/21/04. EE LOST AN ADDITIONAL 17 DAYS FROM 11/18/04 UNTIL 12/13/04 DUE TO A 2ND MINOR SURGERY ON HIS LEFT KNEE.

EE STATED HE WAS MOVING CONTINUOUS MINER AND HAD STOPPED TO HANG CABLE ON MINER. HE STATED A DEFLECTION PLATE HAD BEEN ADDED THE SCRUBBER DISCHARGE AND WHILE STANDING AT THE REAR OF THE MINER. HE BEGAN TO MOVE IT AND THE MINER SLOACHED CATCHING HIS LEFT FOOT BETWEEN IT AND THE FOOTWALL.
