EE was operating a chain saw cutting trees. A tree kicked back sideways and struck EE on left knee. MRI shown torn ligament. Surgery is being scheduled to repair ligament.

EE had a piece of metal on the bumper of his welding truck and was using a cutting torch to cut a piece for the blade of. Bulldozer when the metal fell striking him on the left foot.
Employee was operating D-11 dozer in an area with some large boulders. He got on top of one of the boulders and it shifted and moved. The bulldozer slid about 6 ft to one side and when it stopped, it jerked employee to the side straining a muscle in his neck.
Mine owner was operating a Caterpiller 245 Excavator and was cleaning the highwall when a rock rolled down and struck the cab. Owner had laceration to left hand and two broken fingers. Returned to work the same day.
EE was climbing out of Ingensol Rand Drill, when the step broke off where it has been welded. EE fell about 4' down and landed on his feet. His back started hurting later on and he went to the doctor on 2/5/07. He was put off work for 1 week for rest.
Employee was helping cut and weld on loader frame on 966 F endloader when a small grease fire started in a hole on the frame. Another ee attempted to use an air hose to blow put the fire and blew hot grease on ee causing minor burns to face and neck
Employee was side cutting a road down the side of mountain when a large rock rolled out of the cut and hit the ripper of the back of the bulldozer causing it to jerk sideways. Employee complained of pain in his back. X-rays & MRIs were negative. Has appointment with another doctor on 8/24/06.
EE was operating a D-11R bulldozer when a rock or rocks came off the highwall, striking the bulldozer on the ripper. A piece of rock bounced off the ripper and came thru the back glass and struck EE on the right shoulder, causing a dislocation of the shoulder. Outpatient surgery was performed to put shoulder back in place and EE went home.
EE WAS ATTEMPTING TO INSTALL A CUTTING EDGE ON A D-11R DOZER. AS THE EDGE WAS BEING PUT INTO PLACE THE LIFTING STRAP SUDDENLY BROKE AND THE EDGE STRUCK EE ON LEFT HAND, CATCHING HAND BETWEEN EDGE AND DOZER BLADE. (AMPUTATION: DISTAL PHALANGE - RING AND MIDDLE FINGERS.)
EE WAS CUTTING & WELDING ON BUCKET FOR 992G ENDLOADER WHEN FOREIGN OBJECT GOT INTO RIGHT EYE. EE WAS USING WELDING HOOD & SAFETY GLASSES. EYE DOCTOR REMOVED A SMALL PIECE OF METAL AND TOLD EE TO STAY HOME NEXT NIGHT.
EE WAS ATTEMPTING TO FLIP A LARGE ROCK OVER WITH EXCAVATOR WHEN THE BUCKET TEETH SUDDENLY CAME OFF THE ROCK RESULTING IN THE EXCAVATOR TURNING OVER BACKWARDS DOWN THE SLOPE. LOCAL FIELD OFFICE WAS NOTIFIED THE DAY OF THE ACCIDENT BECAUSE EXTENT OF INJURIES WERE NOT KNOWN AT THAT TIME.
EMPLOYEE WAS CUTTING BROKEN TRACK BOLT OFF D9N AND A FOREIGN OBJECT GOT INTO LEFT EYE. EE WAS WEARING SAFETY GLASSES.
EE WENT HOME AT 2:30 AM ON WEDNESDAY NIGHT 8/15/02. THOUGHT HE HAD FLASH BURNED HIS EYE. MISSED WORK THURSDAY NITE 8/15-16/02. WENT TO DOCTOR 8/16/02 & HAD PIECE OF METAL REMOVED FROM RIGHT EY E.
EE HAD REMOVED THE INSIDE COVER FROM THE ALE UNIT ON #6 INGERSOL RAND DRILL & PUT THE COVER IN THE CORNER OF THE CAB. WHEN HE STARTED THE DRILL TO CHECK THE SYSTEM THE COVER FELL, STRIKING HIM ON THE RIGHT FOREARM CAUSING A LACERATION THAT REQUIRED 3 STITCHES.
EE WAS PULLING UP ON FUEL PEDAL ON 16G GRADER TO SHUT DOWN ENGINE AND FELT PAIN IN RIGHT SHOULDER DID NOT REPAY IT TO CO TIL 3-22-02. WENT TO DR AND WAS TOLD TO BE OFF WORK FOR 2-3 WEEKS.
EMPLOYEE HAD BEEN WELDING AND USING A GRINDER AND A FOREIGN OBJECT GOT INTO LEFT EYE.
WHILE CUTTING AND WELDING, A FOREIGN OBJECT GOT INTO EYE. DIDN'T BOTHER EMPLOYEE UNTIL 4/18. WENT TO EYE DOCTOR AND HAD OBJECT REMOVED AND RECEIVED PRESCRIPTION FOR EYE DROPS.
EE HAD BEEN WELDING ON A HOSE BRACKET ON D-10N DOZER & WAS USING A GRINDER TO SMOOTH IT DOWN WHEN A PIECE OF THE GRINDING WHEEL CAME OFF & STRICK THE CLEAR SAFETY LENS ON WELDING HOOD SHATTER ING THE LENS & PIECES OF SAFETY GLASS & DISC ENTERED THE EYE. OBJECTS WERE REMOVED BY A OPTHAMOLOGISTY
EE WAS BENT OVER USING A CUTTING TORCH TO REMOVEA PIECE OF METAL PLATE FROM SCREEN & WHEN HE STOOD UP HE FELT A SHARP PAIN IN HIS BACK. EE KEPT WORKING & DID NOT GO TO DR UNTIL 10/17/00. NO LO SS DAYS & NO DR VISITS UNTIL 10/17/00
