Employee was getting onto a D9N dozer, his foot slipped and he hit his left knee on the push arm of the dozer.
Employee was climbing ladder and slipped catching himself with his left arm causing pain in his left shoulder.

Backed up on a rock with a dozer and hit it hard with track. Jarred him and said he hurt his neck. Did not wish to go to doctor and didn't want medical attention. Went to see his family doctor after work and said he had a pulled muscle in his back.

Employee was welding and thought a piece of dirt or something flew behind his face shield and into his eye. He worked the rest of the shift and did not wish to go to the doctor. He worked the following day and after the shift went to doctor on 8/28/14. Doctor found a small a metal shaving in eye.
Using a combination wrench to remove a hose from a hydraulic pump. He was in a squatting and bending position pulling on the wrench when he felt a sharp pain across his belt line area.

Employee was operating Caterpillar 665 challenger tractor with a box scraper attached to the back of it. When he backed up to unload the box scraper he ran over a rock that caused the machine to jerk sideways causing his head to jerk to the side. Doctor wants MRI before returning to duty.

Moving a piece of metal with pry bar the metal slipped and caused the bar to flip back striking EE on right hand causing a fracture on small bone between finger knuckle and wrist. Surgery sch. to put pin in bone.
EE was driving a pin back in the blade of cat D11N Bulldozer with sledge hammer when a piece of metal came off hammer and struck EE in abdomen causing a laceration. EE was given a tetanus shot and antibiotics and RTW.
EE came in on 4/19/12 said he had been to Dr. and that the Dr. wanted to do surgery on back as the result of a fall on 3/1/12.EE missed work on 3/12 & 3/26 but did not report it due to an injury. On 3/1/12 EE stepped in a hole and fell backwards but did not miss any work or go to a doctor at that time. EE worked until 4/19/12. W.C.reviewing

Employee was assisting welder lift a piece of metal out of the back of truck. Metal weighed approx. 150 lbs. Felt sharp pain in lower back just as metal got to the ground.

EE was using a chain saw to cut a tree approx 18-20"" in diameter. When the tree fell, a grapevine that was unseen in the top of the tree pulled a dead tree approx 6-8"" in diameter down and struck EE, knocking him to the ground. When EE hit the ground, he fell on a rock that caused a laceration over right eye, requiring 7 stitches.
Employee was laying a 2"" cable to pull a tooth onto a 994 bucket with the help of the customer's mechanic. The customer's mechanic let go of the cable without warning, causing the cable to fall across employee's finger, cutting the finger.
EE was using a pry bar to remove a fiber bearing from a control arm on 16G Road Grader, when bearing abruptly came out, striking EE in the mouth, causing 4 stitches and a temporary bridge on 2 teeth.

Employee was installing a muffler system on CAT 245 Excavator when he strained a muscle in his left shoulder. MRI is scheduled.
Employee developed carpal tunnel in both wrists due to operating bulldozers over several years. Had surgery on 8/10/09.
Employee was cutting a piece of beltline to make a mud flap for grader when knife slipped and cut left hand, requiring 4 stitches. Employee returned to work.
Because of storm damage the day prior, employee was cutting metal supports on door of building when it caught fire damaging the building and several pieces of mining equipment.

Employee was helping install a bed on water truck. He was helping guide bed down into place and the bed shifted and he sprained his wrist while trying to keep bed in place.
Mechanic was removing D10 engine. The nose guard had been rolled over and 2 hydraulic lines were still attached and under tension. When 1 line was removed the remaining line could not hold the load and pulled loose striking mechanic on the hand.
EE was climbing out of Ingensol Rand Drill, when the step broke off where it has been welded. EE fell about 4' down and landed on his feet. His back started hurting later on and he went to the doctor on 2/5/07. He was put off work for 1 week for rest.

Employee was repairing a Mack 600 fuel truck. When he stepped off the truck he sprained his ankle when he stepped on a rock.
EE was diagnosed with carpul tunnel syndrome and had surgery to correct it on 3/2/06.
Employee has filed a black lung claim.

Employee was climbing up on a Caterpillar 785 Rock Truck to perform some mechanical duties and as he pulled himself up on the ladder he felt a pain in the lower part of his back. He is scheduled to take physical therapy for a strained muscle in the lower back and is not released to come back to work yet.

EE WAS CUTTING & WELDING ON BUCKET FOR 992G ENDLOADER WHEN FOREIGN OBJECT GOT INTO RIGHT EYE. EE WAS USING WELDING HOOD & SAFETY GLASSES. EYE DOCTOR REMOVED A SMALL PIECE OF METAL AND TOLD EE TO STAY HOME NEXT NIGHT.
EE WAS ATTEMPTING TO FLIP A LARGE ROCK OVER WITH EXCAVATOR WHEN THE BUCKET TEETH SUDDENLY CAME OFF THE ROCK RESULTING IN THE EXCAVATOR TURNING OVER BACKWARDS DOWN THE SLOPE. LOCAL FIELD OFFICE WAS NOTIFIED THE DAY OF THE ACCIDENT BECAUSE EXTENT OF INJURIES WERE NOT KNOWN AT THAT TIME.

EE HAD REMOVED THE INSIDE COVER FROM THE ALE UNIT ON #6 INGERSOL RAND DRILL & PUT THE COVER IN THE CORNER OF THE CAB. WHEN HE STARTED THE DRILL TO CHECK THE SYSTEM THE COVER FELL, STRIKING HIM ON THE RIGHT FOREARM CAUSING A LACERATION THAT REQUIRED 3 STITCHES.
EE WAS USING DEAD BLOW HAMMER TO JAR ANFO LOOSE IN BIN AND FELT SHARP PAIN IN BACK AND LEFT ARM AS HE WAS STRIKING BIN WITH HAMMER.
EE WAS PULLING UP ON FUEL PEDAL ON 16G GRADER TO SHUT DOWN ENGINE AND FELT PAIN IN RIGHT SHOULDER DID NOT REPAY IT TO CO TIL 3-22-02. WENT TO DR AND WAS TOLD TO BE OFF WORK FOR 2-3 WEEKS.

EE WALKED AROUND THE BACK WELDING TRUCK & DIDN'TSEE METAL BARS THAT WERE EXTENDING APPROX 18"" PAST THE BUMPER & WALKED INTO THEM. THE BARS WERE ANGLED UP & HIS NOSE HIT THE BARS CUTTING HIM REQUIRING 6 STITCHES.

EMPLOYEE WAS SPLICING A WIRING HARNESS ON A D-10BULLDOZER WHEN HIS KNIFE SLIPPED CUTTING HIM ON THE LEFT LEG.
EE WAS INSTALLING A BELT SCRAPER RUBBER ON BELT LINE AND WAS LINING THE BOLT HOLES UP WHEN HIS HAND SLIPPED AND HIT A PIECE OF TING COVERING THE BELT LINE, CAUSING A SEVERE CUT TO THE TOP OF T HE RIGHT HAND.
