THE INJURED INFORMED HIS FOREMAN THAT WHILE HE WAS TRAMMING A DOZER, HE RAN OVER A ROCK AND SLIGHTLY STRAINED HIS BACK. HE SAID IT WAS NOT SERIOUS. UPON TRYING TO CONTACT THE INJURED, HE REFUS ED TO GIVE ANY INFORMATION. THE COMPANY WAS NOT AWARE THAT HE WENT TO A DOCTOR UNTIL A REPORT WAS RECEIVED FROM THE DOCTOR.
