Employee states that he and his bolt partner were backing the roof bolter out of the #5 entry. As he pulled up the cable slack back, the bolter turned and began to roll up the cable to the spool. The cable had a loop in it and as it tightened up, his foot was in the loop. This caused a fracture to his (L) ankle.

Employee states that while adjusting the leveling chain on the pot he had placed his (R) hand on the boom. His (R) middle finger got caught between short arm and pot arm. This caused a laceration to his (R) middle finger. Employee was wearing leather gloves at the time of the incident.

Man was installing a roof bolt after drilling the hole and got his pointer finger on his right hand caught between the wrench and the washer on the bolt, he missed the hole and put pressure on the bolt against the roof.
Employee was running the continuous miner, when he was struck above the right eye by a piece of metal. He went to the hospital where they removed the metal object. The metal was approximately 1/2" x 3/16" x 1/8". The cut required two stitches.
The employee was removing a wheel from a fuel truck with an impact wrench when the socket broke causing a piece of the socket to strike him in the abdomen. He treated the wound himself and continued working until May 30, 2011. On that date, he was admitted to the hospital with infection in the wound.
Employee states that he was bolting top in the #4 entry when the drill steel hung in the top. It came down and caught my finger between the steel and the pot.
Employee was putting up screen wire in our Main West travel road with a double boom roof bolter. He missed the hole when trying to insert the roof bolt and the bolt bent. He then accidentally hit his rotation, instead of the lowering lever, causing the bent bolt to swing around and hit his right wrist.
Employee states that as he engaged the drill pot on the bolter, his (R) hand glove was caught between the drill steel and pot and was twisted by the rotation, it removed the glove from his hand.
Employee states that while bolting top, the drill steel got stuck in the top. As he was attempting to remove the stuck steel, the wrench contacted his hand.
Employee states he was bolting the #30 wall entry, a bolt was not fully up. He attempted to bend the bolt over by using the drill pot on the roof bolter. As he was bending the bolt, his right index finger was caught between the moving parts on the canopy.
EE was putting a roof bolt in a trimed off corner of an intersection. He bent over to pick up the roof bolt after drilling his hole and felt a pain in his left shoulder and neck. He continued to work from the date of the accident until Monday, 10/19/09.
Backing the miner out of a cut, he stopped the miner to put a loop of a cable onto the tail, the cable strap was to short to reach the hook on the tail, he swung the tail over, smashing his right forearm between the coal rib and the miner tail.

Slid Dozer off end wall to the pit floor about 10'. He was offered transportation to a hospital, he refused. Then in Dec. 07 started complaining of shoulder and neck pain, said was caused from acccident in 3/07. He started missing work and had surgery on neck and shoulder on 3/10/08. This was reported to me on 3/25/08.
Employee was helping hang bolter cable the reel on the bolter was full of cables slack and became fouled the bolts was traming at this sametime as he were hanging the cable the cable got pulled down and struck ee in back op neck and knock-him to the ground.
We were pulling pump hose back into the N.E. boxcut using 1250 backhoe and nylon straps. I was between pump and 1250 bucket. The bucket accidentally swung into the pump, catching my left arm between pump and bucket, resulting in a fracture of the lower left arm.
