Welding on a Joy 14-9CM when a piece of hot material went into boot causing a burn on the inside of the left calf area causing a burn. Was not reported until 8/18/13.

Bolting with twist steel and it hung up trying to pull steel out of top some how canopy hit him on the back of his head, was nothing wrong with the jacks.
EE was bolting repeat enter was resin roof bolts and his shoulder started hurting.
Miner had backed out of pillar line. MRS jack was being moved when a bolt head broke off letting a rock 67 x 72 x 6 fall and strike MRS operator in the lower back.
The injured employee was operating a remote continuous miner. A piece of line curtain was burried in gob that had been scooped into a fresh cut. When the gob with the curtain came across the end of the conveyor into s.c. it slung coal & mud into miner operators face, filling his eyes with coal.
The witness stated he was bolting not knowing that the cable reel was engaged. His co-worker was at the rear of the bolter at this time and was flipped by the cable when the reel began taking up. The co-worker complained of low back pain.
Letting head down, he had his hand on drill pot. Got fingers between drill pot and drill pot arms.
Bolting top in face when he lowered head on bolter a piece of drill steel fell out of hole and hit him on lower left arm, causing a bruise and contusion.
Miner operator was loading end of 8 rt on main section 1 break in by spad 3748. Miner operator was loading gob and became pinned against rib approx 3' inby standoff. Injury to pelvic area and legs.
Bolter operator bolting up cap coal. When drill steel when through cap coal the coal dislodged fro the mine roof striking the operator on his head & shoulders.
A piece of slate mine roof fell out between he last row of bolts and the ATRS on the operator. The operator complained of pain in his neck, right shoulder and chest.
EE WAS BOLTING TOP - PUTTING UP RIB BOLT AND RIB ROLLED STRIKING HIM ON HIS BACK IN THE CENTER OF HIS BACK IN #8 ENTRY. WHEN HE TURNED AROUND TO SEE WHAT HAPPENED HIS LEFT JAW STRUCK CANOPY POST. HE'S GOING TO SEE A SPECIALIST ABOUT JAW.
WAS SETTING UP MINER TO LOAD, DROPPED PAN & HEAD ON BOTTOM TO PUSH UP LOOSE COAL. I WAS LOOKING STRAIGHT AHEAD & DID NOT REALIZE MY FOOT WAS IN THE PAN OF HEAD A BIT CAUGHT MY BOOT & ROLLED ON MY FOOT BREAKING TWO TOES AND HAVING TO GET STITCHES.
A PIECE OF DRAW ROCK FELL BETWEEN ATRS AND LAST BOLT.

THE MAN WAS PREPARING TO DRILL BOLT HOLE IN LAST ROW OF BOLTS. WHEN HE PUSHED THE DRILL STEEL AGAINST THE ROOF A PIECE OF ROCK 2"THICK 18" X 18" BROKE LOOSE AND FELL STRIKING HIS LEFT HAND WHI CH CUT HIS MIDDLE, RING AND PINKY FINGER.

EE WAS DRILLING TOP WHEN A PIECE OF LOOSE ROCK FELL, STRIKING HIM ON HIS LEFT HAND.
HE CAUGHT GLOVE ON LEFT HAND AT DRILL CHUCK TWISTING LITTLE FINGER.
GLOVE CAUGHT ON ROTATING DRILL STEEL.
ROOF BOLTER MASHED FINGER ON DRILL STEEL.
EE WAS OPERATING THE CONTINUOUS MINER WHEN HE TURNED TO LOOK AT THE INCOMING S/C & CABLE WHE HE FELT PAIN IN HIS KNEE AND IT CONTINUED TO SWELL.
EE HAD WENT INTO FACE OF #4 ENTRY WITH BOLTER OPERATOR & BOLTER AND WAS REMOVING REFLECTOR FROM 2ND ROW OF BOLTS, WHEN A PIECE OF DRAW ROCK FELL, STRIKING EE ON THE HEAD AND DRIVING HIM DOWN T O THE BOTTOM, BREAKING HIS LOWER LEG AND POSSIBLY HIS THIGH. (RT. LEG)
