Showing all 16The injured was drilling hole in roof to install roof bolt. While drilling hole, a burr on the steel caught his glove and fractured his left ring finger.
The injured is a roof bolter, he said he had a bolt stuck in the hole in roof when he pulled the bolt out of roof he mashed his litte finger between the bolt head & the wrench on bolter.
The injured said he had drilled a hole in the mine roof, to install a roof bolt when the top section of drill steel hung in the roof - The drill steel then fell out of the roof, striking him on the left hand.
The injured was roof bolting, after putting bolt in roof, he let his foot off before letting the canopy down, when he did this the boom and canopy came down 3-4", hit him in the head, he had too much pressure on foot jack and canopy.

EMPLOYEE WAS STANDING BESIDE THE ROOF BOLTER MAKING UP BOLTS. HE HAD HIS FOOT IN FRONT OF THE TIRE WHEN THE OPERATOR PULLED BOLTER UP TO PUT IN ANOTHER RUN OF BOLTS AND RAN OVER HIS FOOT.
EE SAID HE WAS PUTTING FOOT JACK DOWN ON BOOM OF ROOF BOLTER, WHICH RAISES BOOM ASSEMBLY AND CANOPY, WHEN THE BOOM ASSEMBLY AND CANOPY CAME BACK DOWN 2-3" HITTING HIM ON HEAD, THE FOOT JACK WA S SETTING ON A ROCK OR CHUNK OF COAL, WHEN THE COAL OR ROCK BUSTED, THE BOOM ASS. AND CANOPY DROPPED.
EMPLOYEE WAS SITTING ON REAR BUMPER OF ROOF BOLTER WHEN OPERATOR BACKED BOLTER UP AND CAUGHT HIS LEFT FOOT UNDER BUMPER.
EE TOLD SUPERVISOR HE HAD HURT HIS SHOULDER AT PREVIOUS PLACE OF EMPLOYMENT. THEN SAID HE HAD RE-INJURED IT PULLING HIS DRILL STEEL OUT OF THE ROOF.
INJURED EE WAS STARTING A STARTER STEEL TO DRILL HOLE FOR ROOF BOLT WHEN PIECE OF DRAW ROCK FELL STRIKING EE ON TOP OF RIGHT HAND.
CANOPY JACK ON ROOF BOLTER REPLACED - NEW CANOPY JACK LOAD LOCK MALFUNCTIONED. CANOPY COLLAPSED STRIKING EMPLOYEE ON TOP OF HEAD.
EE WAS BOLTING & LET BOOM DOWN ON RIGHT LEG
