Over-exertion in lifting objects

Employee along with two other employees were sampling a slurry pond. When the equipment that they were using started to sink and got stuck in the muddy coal. They had to unload the trailer full of equipment and pack it across the muddy slurry pond. Then they had to dig out the trailer and push it off the pond. He pulled a muscle in is back.

EXAMINER FOUND FALL IN NO. 6 ENTRY OF THE MAIN NORTH ENTRIES AT +7081 (4-WAY INTERSECTION - NEUTRAL AIR). FALL IS APPROX. 38' LONG X 19' WIDE X 6' HIGH. APPROACHES TO FALL HAVE BEEN TIMBERED - FALL WILL NOT BE CLEANED.

Examiner found fall in No. 2 entry in the intake station+2010 of the first east off the main north entries. Fall is approx. 40' long x 18'wide x 7' high. Fall will not be cleaned.

Struck by falling object

While being task trained on pinner, EE put drill steel on top causing a large rock to fall out, hitting him on the heel of his left foot driving his toes into the floor of the mine.

Examiner found roof fall in #2 entry of 1st E/main north at +1945. Fall is in primary escapeway - intake - and will not be cleaned. Escapeway set over to #1 entry. Approaches to fall have been timbered. Fall is approx 6 feet 7 inches high x 18 feet 8 inches wide x 40 feet long.

Examiner found roof fall in #2 entry of the main north entry fall is approx. 40' long 17-18"" wide 6-7' high fall is in intake between 32 to 33 xcut 2580 to 2660-will clean up around fall in adjacent entries to have passable travel wall. Fall will not be cleaned up timber or cribs will be set on ends of fall.

Found roof fall on the 1A road at 8121' to 8041' of the north entries. Roof fall is 90' to 95' long, 17-18' wide, 7 to 8' high. Also it fell across 1A belt roadway. Had additional support previously installed (timber and cribs). Primary roof support was 6' tension rebar. Fall will not be cleaned up.

Examiner found a roof fall at the 88 Xcut(7081') to just inby 15' of 89 Xcut (7161') in #6 entry of the main north headings. Fall is approx 90-95' long X 7-8' high X 17-18' wide. Primary roof support was 6' tension rebar. Fall was timbered and cribbed on north end of fall. All approaches will be timbered.

Roof fall was found by examiner at start of shift fall is at 4841-4921 between got 61 x cut on main north travelway fall is 65 to 70' long 18 1/2' wide 8-9' high fall is in entry between intersecitons primary roof support was 6' double rock with glue intersection at 4841 had been resupported with 12' cable bolts fall will not be cleaned up.

Primary roof support was 5' double lock. Fall has been timbered on both ends. Additional support has been installed on roadway. Fall will not be cleaned up.

Employee was replacing guard on belt at hoist. Guard was resting on bolt head when it slipped, falling down and cutting right little finger on hand.

Roof fall was found when examining entries in 1st east. Fall is at the 320' of the 1st east entries between #1 & #2 entry. Fall is 45-50' long, 17-18' wide 7-8' high. Fall is in pillar line. Fall has been timbered off & will not be cleaned up. Primary roof support was a 5' double lock.

Roof fall was found at the 5001' of the #3 entry north at 2:10pm. Fall was in a 4 way intersection. Fall is 35-40' long, 7-8' high and 18-19' wide. Fall has return for #2 entry blocked. Fall will be cleaned up. Areas have been timbered off and cribs built on south end of fall. 6' tension rebar was primary roof support.

Roof fall was found at the 2530' of #6 entry in the 1st East @ 1:30 pm. Fall was in a 4-way intersection. Fall was 45'L, 17-18'W, 7-8'H. Primary roof support was 5' double lock bolts. Fall will not be cleaned up. Areas have been timbered off & additional roof support has been installed (cribs).

Examiner was making weekly examination of return entry and found roof fall in #8 entry between the 1940' + 2020' X cut was in entry between the intersections. Fall is 60' long, 6' to 7' high and 17' to 18' long. Area has been timbered off. Fall will not be cleaned up. Primary roof support was 6' tension rebar.

EE was bolting at face. He was in process of putting bolt in hole and pushing bolt up into glue, when power was lost on pinner. EE then pulled bolt out of hole and struck finger on drill pot when bolt came out of hole.

Struck against a moving object

Employee was using water line on unit to water haulage roads. When pulling on waterline, it hung on a rock and pulled him backwards, twisting his knee. At end of shift when riding out in mantrip, a spare tire on mantrip caught on a roof bolt causing mantrip to stop suddenly and employee bruised his tailbone.

Roof fall at 1780' mark in xcut between road & belt in 1A east entries. Fall is 55' long x 5-6' high x 18' wide. Intersection on roadway was previously cable bolted when area was developed. Roadway & belt line has been cribbed off. Primary roof support was 5' double lock bolts.

Employee was riding out in mantrip at the end of shift. There was a piece of belt in roadway. When mantrip drove over belt the belt caught his foot pulling leg sideways.

Employee was tramming scoop down 1st north travel way & ran over rock in roadway causing him to hit his head, neck & shoulder on canopy.

Employee was using a cutting torch to remove a broken leaf spring on a golf cart. He was holding spring with left hand and moved cutting torch over left hand. Employee was wearing gloves and cutting goggles.

Xcut north at 1920' on 1A road (roof fall) intersection on inby & outby end of fall had previously been cable bolted roof fall 6' high 17-18' wide 35-40' long area that fell was bolted with 5' double lock bolts.

Fall from machine

A GOLF CART BROKE DOWN & WAS SENT TO SURFACE TO BE REPAIRED. AN ENDLOADER WAS USED TO PICK CART OFF LOWBOY - WHEN LOAD WAS PICKED UP IT WAS NOT CENTERED. THE MINE FOREMAN STOOD ON END OF CART TO BALANCE LOAD. WHEN LOAD WAS MOVED, CART SHIFTED ON FORK OF ENDLOADR CAUSING CART TO FLIP MINE FOREMAN IN AIR AND FALL TO GROUND ON HIS BACK.

HAD A ROOF FALL ON THE 2ND NORTH BELT @ +4601 INTERSECTION ON #4 ENTRY NORTH BELT LINE WAS TIMBERED UP TO THIS AREA. APPROACHES ARE TIMBERED FALL WILL BE CLEANED UP. BELT & STRUCTURE AND WATER LINE UNDER FALL. FALL IS APPROX 40'L, 18' W AND 7-9' HIGH. AREA HAD BEEN BOLTD WITH 5' TENSION BOLTS.

Struck by flying object

EE was using a cutting torch and cutting the bolts out of a cyclone in prep plant cyclone was suspended by a chain hoist. When EE hit cyclone with hammerto knock bolts out of the cyclone it broke free & swung around striking EE on left forearm.

EE is a belt maintenance man. As he was watching belt run, he saw a roofbolt laying on the belt. He reached to pull bolt off the belt and his right middle finger got caught in between the moving belt and roller.

EE was installing rof bolts at Face # 3 entry was getting drill steels out of drill chuck. He was going to turn on the rotation but pushed the wrong lever & drill pot went up and pinned his left index finger to the roof.

EMPLOYEE STATED HE WAS DRIVING GOLF CART BETWEEN COAL HAULER & RIB. A COAL HAULER TIRE WAS LEANING AGAINST THE RIB WHEN EE DROVE THE CART BY RIB. THE CART STRUCK THE TIRE LEANING AGAINST RIB, AS IT WAS FALLING OVER EE REACHED OUT TO CATCH THE TIRE PULLING HIS LEFT ARM BACKWARDS.

EMPLOYEE WAS LIFTING BELT STRUCTURE FROM THE MINE FLOOR & LOADING STRUCTURE ON SUPPLY CAR.

AT ABOUT 8:00 AM, 2 GOLF CARTS WERE PUT ON CHARGE AT SHIFT CHANGE. AT ABOUT 9:55 AM, 2 MINERS SMELLED SMOKE ON TRAVEL WAY. THE MINE FOREMAN WAS NOTIFIED & TRAVELED TO SLOPE BOTTOM & FOUND 2 GOLF CARTS ON FIRE. THE FIRE WAS PUT OUT ABOUT 10:15 AM. CAUSE OF FIRE IS UNDETERMINED (INVESTIGATION ONGOING). ALL ELECTRICAL CIRCUITS SUPPLYING POWER TO GOLF CARTS FUNCTIONED OK.

In the #5 entry north supply road at 5721 to 5801 in pillar line was developed about 8/10/04. The roof was pinned with 5' tension rebar bolt. the fall was approx. 44' long 16' wide, 8' high.

The injured is a roof bolter, he was throwing rockdust on the rib, the upper of machine was setting over, while doing this he ran over the injured EE, right foot, the injured had his back to machine.

THE FALL WAS IN THE #6 NORTH ENTRY (SUPPLY ROAD) AT +5321 TO +5241 IN PILLAR LINE. WAS DEVELOPED APPROX 8/2/04. THE ROOF WAS PINNED WITH 5' TENSION REBAR BOLTS. THE FALL WAS APPROX 50' LONG, 18' WIDE, 5' HIGH.

Employee said he had put Ram battery on charge & when he jerked on battery lid his back started hurting, he worked until 11/30/05, went to Dr.

HAD FALL IN INTERSECTION, BELT ENTRY, 2 X-CUTS INBY FEEDER ON #2 UNIT OF 650'WEST PANEL. FALL MEASURED APPROX. 49' X 42' X 6'HIGH. PULLED 4' DOUBLE LOCK ROOF BOLTS. FALL IS BEING CLEANED UP.

The injured said he was getting out of cab on 488 scoop when he put his foot on the ground he heard a pop in his right knee.

Struck by powered moving object

THE INJURED WAS BUILDING A BRATTICE ON #1 UNIT. HE DID NOT HAVE THE CURTAIN DOWN AND NO FLAG WAS HANGING TO CURTAIN CAR DRIVER. A RAM CAR RAN THRU CURTAIN, HIT THE TRAILER OF BLOCK, HITTING THE INJURED ON LEFT LEG. THE CAR DRIVER DID SOUND HORN AND FLASH HIS LIGHT BEFORE GOING THRU CURTAIN.

The injured was drilling hole in roof to install roof bolt. While drilling hole, a burr on the steel caught his glove and fractured his left ring finger.

The injured said he was picking up a 5' long 6X6' timber when he felt a sharp pain in his right groin area.

THE INJURED SAID HE WAS HELPING MOVE BELT UP. HE WAS LIFTING BOTTOM BELT ROLLERS UP AND HANGING IT ON ROPE, WHEN HIS BACK STARTED HURTING.

The fall was int he #2 (N. intake) entry, which blocked escapeway from the #1 unit no work will be done on #1 unit until escapeway is sate to travel. The fall was 70' long, 5' high, 18' wide. The fall will be cleaned up before work is rejumed on the affected #1 unit.

The injured is a roof bolter, he said he had a bolt stuck in the hole in roof when he pulled the bolt out of roof he mashed his litte finger between the bolt head & the wrench on bolter.

Had roof fall on the 2nd M. North belt line, at +4681, intersection #4 entry. Belt line was timbered up to this area, approaches are timbered. Fall will be cleaned up. Belt, structure and water line, under fall. The fall is appx 70' long, 4 to 6' high, 18' wide, had been supported with 5' tension roof bolts.

THE 5 HP MOTOR ON PRESSURE TANK WENT TO GROUND. HAD TO CHANGE MOTOR. THIS HELPS OPERATE BRAKES. THE MEN UNDERGROUND WERE NOTIFIED & PRODUCTION WAS STOPPED. THE EMERGENCY HOIST IN SHAFT WAS SENT TO BOTTOM. THE HOIST WAS RUNNING AGAIN AT 11:00 AM. IT WAS DOWN 1 HR 45 MIN.

THE INJURED SAID HE WAS WALKING OVER TO GET THE SCOOP WHEN HE RAN INTO A ROOF BOLT, FELL DOWN TO HIS KNEES, AND HEARD SOMETHING POP IN RIGHT KNEE. HIS KNEE WAS NOT HURTING THAT BAD. HE FINISHED WORKING THE REST OF SHIFT.

The injured said he had drilled a hole in the mine roof, to install a roof bolt when the top section of drill steel hung in the roof - The drill steel then fell out of the roof, striking him on the left hand.

Fall from machine

THE INJURED TOOK RAM CAR TO BATTERY BARN TO CHANGE BATTERIES. WHEN HE WAS GETTING OFF OF THE CAB OF RAM CAR, HE SLIPPED, FELL & HURT HIS LOWER BACK.

Fall to the walkway or working surface

The injured took the ram car out to the battery barn to change the batteries, he said he tripped and fell down and injured his right wrist.

the injured said he was pulling ont he miner cable when his back started hurting. he said he heard something pop in his lower back.

The injured was climbing up and down a ladder when he slipped and hit his right knee against a step on the ladder.

The injured is a miner oper. He was walking behind the wing curtain, out to intersection, when a piece of rock appx. 2' x 3' x 2"" fell from roof, striking him in back.

The injured was driving a ram car, from batt. barn to unit, when a 2x4x8' was bolted to roof and isolation curtain was nailed to it. The 2x4 had been knocked loose from roof bolt & was hanging down, as he drove the ram car thru curtain, the 2x4 caught under canopy of car, came in cab, striking him on right knee.

The rock fall was in xcut between belt entry and neutral entry at 520 east panel, fall was approx 5' high 19' wide and 40' long. This was supported with 5' tension rebar roof bolts. The 2 approaches have been cribbed off. The unit at 1170' working out intake rooms. The fall will not be cleaned up.

The fall was in the #2 main north entry, intake at +5721. The fall was in partial intersection & mostly inbetween the pillars. The fall will be cleaned up; in the process of setting timbers in ajoining entries.

Had partial rock fall in South return panel, #8 entry. A slip was running through this area. The fall was approximately 25' long, 16' wide & 6' to 8' high. This area was bolted with 5' tension rebar roof bolts and was reinforced with some 8' pins. Fall will not be cleaned up. It is cribbed & timbered at approaches.

Rock fall is in West Intake, #2 Entry, West Panel. Fall was bolted with 5' Tension Rebar Roof bolts. Fall is 5' high, appx 30' long, 18' wide. Approaches are timbered.

The injured strained his back while lifting a battery cell. He was replacing a bad cell in ram car battery.

THE INJURED WAS OPERATING THE SCOOP, HE HAD HIS LEFT HAND ON CANOPY EDGE. WHILE TRAMMING SCOOP, THE CANOPY CAME IN CONTACT WITH THE MINE ROOF, MASHING OFF THE END OF (2) FINGERS ON HIS LT. HAND. INDEX & MIDDLE FINGERS, JUST BEHIND FINGER-NAIL.

The rock fall was in a x cut between the supply road and belt entry. the support in this area is 5' tension rebai roof bolts, both approaches have been cribbed. this was in the best panel at 7425 the face on unit is 2240.

THE DYNAMIC BRAKES WAS NOT WORKING ON HOIST. ELECTRIC PANEL WAS CHECKED AND THE CONTROL TRANS AND 2 DTODE WERE BAD. PARTS WERE CHANGED OUT AND WOIST WAS OPERATING CORRECTLY AT 925 AM.

THE INJURED SAID HE WAS LOADING CRIB TIES (4 X 6 X 30"" WOOD) INTO SCOOP BUCKET WHEN A PIECE OF ROCK FELL FROM ROOF & STRUCK HIM ON RT. ANKLE.

The injured was operating s&s scoop, he was driving scoop on haulage read just inby feeder, when the ram car operator ran into scoop. the injured stepped in haulage road & car driver did not see him.

THE HOIST BROKE DOWN APPROX 40 MIN, 4:00 PM TO 4:40 PM. THE SOUPLING BETWEEN MOTOR & REDUCER CAME APART. A NEW COUPLER WAS INSTALLED. WORK AT FACE WAS STOPPED DURING BREAK DOWN OF HOIST.

HAD ROCK FALL IN THE MAIN NORTH INTAKE, #3 ENTRY, AT 4 WAY INTERSECTION, AT 4344. FALL IS 80' LONG, 5' HIGH, 17' WIDE. WAS SUPPORTED WITH 5' FULLY GROUTED ROOF BOLTS. APPROACHES ARE BEING TIMBERED AND CRIBBED INBY FALL.

Caught in, under or between a moving and a stationary object

EE WAS CHANGING BELTS ON THE #4 BELT DRIVE JUST INSIDE THE WASHER. THE CHUTE AT #4 TAIL WAS PLUGGED. 2 OTHER MEN WERE IN THE AREA, TRYING TO CLEAR TAIL ROLLER & CHUTE. THE THOUGHT EE HAD GONE TO RESTROOM. WHEN THEY JOGGED THE BELT, THE EE GOT HIS 4 LEFT FINGERS IN BETWEEN PULLEYS AND BELT AT DRIVE MOTOR.

EE IS UNIT REPAIRMAN. HE WAS TRYING TO TAKE FITTING OFF OF Y STRAINER ON FEEDER WATER SYSTEM WITH A PIPE WRENCH. WHILE PULLING ON PIPE WRENCH, HE STRAINED LOWER BACK MUSCLE.

Struck against stationary object

Employee was working on the back side of unit belt tail. The belt went off for unknown reason. as he was crawling across top of belt it started up carrying him down the belt for approx. 70'. The area is very low and he hit the mine roof several times.

Struck against a moving object

The injured was dragging with S&S tractor and drag. He ran over a piece of rock in the road, this bounced the tractor and threw him up against the canopy on the tractor, straining his neck muscles.

Flash burns (electric)

THE JOY MINER CABLE HAD BEEN DAMAGED WHICH KNOCKED(TRIPPED) BREAKER TO MINER. INJURED CHECKED CABLE, REPAIRED IT AND CHECKED CABLE AGAIN. AFTER REPAIR, CABLE CHECKED OK. WHEN EE THREW POWER UP TO THIS MINER, THE BREAKER BLEW-UP BURNING HIS RIGHT HAND.

The rock fall is in #2 entry +980, west intake. Fall was 5' high, 90' long, 18' wide, at intersection. #1, 2, 3 entries are all intake, #1, #3 entries are ok. We have cribbed & timbered approaches to fall & cribbed #1, #3 entries. Notified MSHA insp. on 6-12-04 of fall.

Struck by falling object

The injured was roof bolting, after putting bolt in roof, he let his foot off before letting the canopy down, when he did this the boom and canopy came down 3-4"", hit him in the head, he had too much pressure on foot jack and canopy.

A rock fell from mine roof, striking the injured on collarbone arch. The injured was not hurt and did not see a doctor. Over the past months he started having trouble with his left shoulder, he went to doctor and will be off a few days taking therapy. May come back on light duty. He was off work starting 10-19-04.
