Employee was on a Vermeer walk behind machine performing clean up tasks in the wet plant area of Utica 3. EE was pulling material out near the surge bin when a finger became wedged between a pipe and the toggle on the machine controls. EE sustained a fracture on the left hand ring finger tip.

Employee was replacing a vibrator on Dryer. After replacing the vibrator employee looked up to see if vibrator was operating, when a small amount of water with very small amount of acid dripped on employee's forehead and running down into the left eye.

While using a grinder, a maintenance mechanic received a laceration on the thumb when the disc caught the bolt being cut and kicked back into EE's left hand, cutting through the leather glove EE was wearing.

Employee ran into parked railcars while operating a trackmobile. EE was thrown from the seat which resulted in a fractured nose and fractured sternum. Contributing factors to the incident were inattention and the employee failed to wear the seatbelt.

Employee was thawing out bridged material above BC-107 and a large piece broke off. Employee pulled the torch back that EE was running and in doing so twisted shoulder. Required no medical attention at the time. 8 months after the incident EE was having issues with the shoulder and went to the doctor for an examination.

Maintenance employee was fabricating mounting bracket for oxy/acy. cylinders to be mounted upright in new Ford shop truck. A pipe hanger was being modified into a mount when the S.S. Disk on grinder became entangled with uniform shirt pulling itself into their lower right abdomen, resulting in a laceration needing 8 sutures at ER.

An Employee was drilling a bracket with left hand and a drill with right hand when the drill bit snagged spinning the bracket around into Employee's palm. The spinning action resulted in the latch ripping through Employee's leather glove and into the tissue in Employee's palm. The Employee was taken to the hospital to get checked and ended up receiving 3 stitches in hand.

While hoisting an unloaded electric chain hoist to move it out of the way, the hoist hook briefly caught/bound on a piece of machinery, popped loose, and struck the worker in the lip causing a laceration. Both upper front teeth were also chipped along the inner edge.

Employee was grinding with hand held grinder wearing safety glasses and face shield. The next day employee's eye felt irritated employee went to the clinic and they removed a small piece of metal, employee received ointment prescription for eye. Employee did not miss any time.

Employee was operating the scraper when ee hit a pot hole and felt a sharp pain in back. Employee called the supervisor and notified ee of the incident. The employee was taken to the clinic where ee was diagnosed with acute lower back pain. Employee was given prescription medication and restrictions.

While using an angle grinder the grinder kicked back hard enough to come loose from the operators left hand and the cut off wheel nicked left thumb as it passed. This resulted in a minor cut between the first and second knuckles of the left thumb which was repaired with 3 stitches. The operator returned to work and finished shift with no restrictions from the doctor.

Employee was holding angle for welder to tack up in hopper. From 11:30PM to 6AM. Employee was closing eyes as welder was welding. Employee went home and woke up with burning eyes.

Employee was cutting steel with angle grinder. Steel was not supported properly and pinched cutting wheel. Cutting wheel disintegrated with debris reaching employee's eye despite dual eye PPE. Debris was removed by doctor and employee was returned to regular work

Employee was welding and grinding and felt irritation in his right eye. He was wearing all necessary PPE but conditions were windy and work was occurring on the floor above. A small piece of metal was removed by the eye doctor and a prescription antibiotic was given.

Employee was cutting a door on the RTO in half to create a safer access into the exhaust plenum. While cutting the door using a 4" grinder, the grinding wheel got bound up and kicked back striking the employee in the left forearm. The employee suffered a laceration on his arm which required 11 stitches to close.

Employee tried to clean shavings off of a drill bit that was in a running drill press. The drill bit then got tangled with his glove and wrapped his left hand up in the drill bit thus, dislocating his left index finger.

EE was using a 4 1/2" grinder with wearing gloves, the grinder also did not have a guard on it and he was not wearing the required PPE to protect his eyes. He ended up injuring his finger / requiring stitches - he was suspended for 3 days.

Employee was helping two welder repair crusher during a schedule shutdown. The EE was exposed to the light from the welding operation that was going. The EE had the access to the correct PPE but chose not to wear it. The EE was seen by the local Dr on 11/22/13 and giving perspiration medicine for his eyes. The EE is doing fine and returned to work the same day.

Injured employee was pushing bottom of a bolt up with hand during removal of a loader bucket cutting edge. Bolt was not fully inserted into the square catch of the cutting edge. A second employee using an impact wrench on the nut started and the bolt turned, cutting the right hand and finger of injured employee. Employee required 15 stitches to close the wound.

Employee was using a grinder to cut a 12" piece of angled steel, when the angle started to move he removed his left hand from the handle of the grinder and proceeded to use his hand to hold the angle in place instead of a vise or clamp. At this point the grinder got bound up causing the grinder to jump and hit the employee on his left hand/thumb area leaving a laceration.

On 04/17/2012 at approximately 8:15 am. EE was allegedly lifting up on the diffuser bar on the VS302 and he felt pain and a pop in his right shoulder. He was taken to the hospital for medical evaluation and was given an X-Ray that was negative for breaks and other signs of trauma.He had a medical procedure on 05/30/2012 that made this incident MSHA reportable.

Work was being done to remove a section of the railroad track, two excavators were in use. Excavator #1 moved into position and was directed to stop. Excavator #2 mirrored movement of excavator #1 without direction and because it did not stop as it should have, it pulled the rail towards itself and struck the leg of employee.

The employee was using a table saw to cut pieces of PVC blocks. With the saw running, he reached under it, and his right middle finger was struck by the rotating blade. He sustained a laceration and damaged distal phalanx bone. Prior to the task the supervisor had a meeting with the three employees that were assigned to do the task about the safety issues with the job.

EE was cleaning clogged kaolin out the #6 roller mill feed bin. As he was removing the clay from the interior wall, he mashed his finger against the door of the feed hopper. This resulted in a laceration and a broken finger tip on his left ring finger.

While lifting a jaw crusher toggle block, an electric hoist chain swung and mashed a maintenance mechanic's left thumb against a nearby beam of the crusher frame. The mechanic was guiding the block through the frame when the block cleared the jaw and swung free. The incident resulted in a laceration requiring 9 stitches, loss of thumbnail and broken tip of the bone.

Employee was working on 220 screen firing building when the torch he was working with failed. The acetylene hose separated from fitting and ignited. A fire fall burned the screen, the fire was controlled immediable by other employee's working around. However the flash caused a burning in right face side and neck which required a medical attention/prescription.

A Front End Loader Operator was greasing his loader at the end of his shift. He was at the grease shed using a pneumatic grease gun to do this. The grease gun slipped off the zerk and the grease came out at extremely high pressure which cut into his left thumb.

The employee was replacing a sleeve on a rotating screen when he cut his hand on the feed cone. The feed cone had become very sharp due to normal ware.

The miner was attempting to loosen the bar bolts on a chainsaw when the bar wrench handle broke and the miners' hand struck the chainsaw chain which cut through his glove and cut the middle finger on his left hand.

BUCKET ELEVATOR BELT BROKE AND FELL. A CRANE WAS USED TO LIFT THE BELTING AND RE-LOOP IT OVER THE HEAD PULLEY INTO HOUSING. EMPLOYEE DIRECTING THE CRANE USED NYLON STRAP AS A CHOKER. WHEN THE BELT HAD BEEN LIFTED 30 FT THE NYLON STRAP PINCHED THROUGH BELT CAUSING IT TO FALL. THE EMPLOYEE GUIDING THE BELT WAS STRUCK IN THE HARD HAT AND BACK BY THE BELTING AND BUCKETS.

The employee was attempting to break a rock that was plugging the front of a feeder going into the primary crusher using a jackhammer. When the jackhammer broke through the rock, the operator lost control of the hammer causing the hammer to pinch his thumb between itself and the guardrail.
