Contractor employee traveled on UTV to load the drill core on service truck. On the travel back to drill, the UTV began sliding out of control, becoming sideways, catching the edge of the road and tipping the UTV on the side causing the contractor to hit abdomen area on roll cage bar.

Maintenance employee was replacing bull gear on rod mill. While transferring the old gear from the rod mill to the designated location, one of the dollies that was holding up one of the two chain hoists being used came off of beam causing the gear to slightly shift positions and come in contact with employees right leg.

Employee was adjusting the water on a cyclone to put more water in the sump. When employee was stepping away from the sump employee's right foot slipped causing employee to lose footing and fall on left side and strike left elbow off of the floor. Employee was experiencing some pain and swelling and was taken to local hospital for treatment.

It is right elbow or the muscle that attaches to the elbow. EE stated that it hurts when the arm moves. EE said that sometimes EE has pain like they are being stabbed and other times just pain. EE was asked to demonstrate what movements cause the pain and showed paper bagging as well as putting on a glove.

Employee was bagging 50 lb paper bags and felt a "burning" pain in right shoulder. Employee continued to bag the rest of shift. EE taken to doctor on 12/22/2020 and placed on modified duty.

At roughly 3:30 PM on Tuesday December 1 the 2nd shift Wet Process Operator stepped on a rock getting out of vehicle and sprained left knee. Employee was taken to the ER. The result was a sprained left knee and ankle. Employee is to return to work in 3 days.

Miner used a forklift to move hopper filled with hot sand from a kiln system. Once at the tipping location, miner lifted hopper to shoulder height, got out of the forklift and released the lever to tip the load. The hot sand spread out radially and ran up EE's pant leg, burning EE's skin above the boot and sock line. Diagnosis was partial thickness burn of lower right leg.

The employee was setting out caps and boosters to load the shot. EE stopped at the bulk truck and told EE's co-worker that EE felt funny. EE continued to layout caps and boosters but was unable to walk correctly and EE's right side was numb. EE finally sat down and tried to call EE's spouse but was unable to due to a lack of motor skills on one side.

Employee was investigating intruders near Pond. While walking from pond (no established walkway), walked through washed out area with uneven terrain & foliage. Employee slipped, fell to the knees and felt pain in L hamstring. Employee was offered to go to dr, but refused until 21 Sept. Dr appt on 23 Sept put EE on modified duty, ordered an MRI. No diagnosis available.

Employee was attempting to clean out the sump hole at the west end of the reclaim building. When EE tried to lift the sump pump from the hole EE felt a pop in EE's right forearm. Employee reported the incident to EE's supervisor and finished EE's shift. The following Monday (9/14/20) the employee visited a doctor who placed EE on modified duty and suggested an MRI.

The worker was striking the head of a bolt with a hammer to unplug a 1 ¼ X 4" pipe nipple. When the material inside the pipe nipple broke loose the impact of the bolt pinched the thumb of the left hand against the pipe nipple. The worker was transported to the local emergency room and received 5 stitches to the thumb.

A pipe line had been taken apart, flushed out, and bolted back together. Pump PU-03 was started up to flush the pipe line and water hammer occurred. It ruptured the poly pipe line, the pipe struck one mechanic on the back of EE's right lower leg. The injured employee was medically evaluated and placed on temporary restricted duties.

Employee stepped out of man basket at it's lowest position after completing a decel screen cleaning task. The employee's left knee "popped" as they stepped out of the basket. After a period time during shift the employee felt pain and discomfort. The employee was taken to the local hospital, given ten days off and prescribed a knee brace.

Thursday April 30th while installing dryer flights at the rotary dryer (DR-01) the maintenance employee strained back. EE did not want to go to the doctor and finished the shift. Friday May 1st the employee was able to come to work, stretch it out and make it thru the day. Thursday May 28th the employee stated EE needed to go to the doctor because back was injured.

The employee reported feeling pain in left shoulder towards the end of shift. The next day the employee stated that the pain was a little better and wanted to continue to monitor it. On 4/22/20 EE reported pain to the Plant Superintendent once again and asked to seek medical attention. The employee sought medical attention on 4/22/20 and was put on modified duty.

Employee was patching a leaking hose from an extendable ladder which had slipped out from under the employee on a grated steel floor. Employee fell at a height of four feet and struck their head on the flooring. The employee was taken to the hospital ER and received six staples to a laceration on the back of head.

Employee was on a Vermeer walk behind machine performing clean up tasks in the wet plant area of Utica 3. EE was pulling material out near the surge bin when a finger became wedged between a pipe and the toggle on the machine controls. EE sustained a fracture on the left hand ring finger tip.

Employee doing maint work on caustic pump in Flot bldg. EE leaned over the pump, a pin hole leaked on the caustic line of pump, spraying caustic soda 50% membrane under employee's face shield. Caustic hit EE's L side lips and EE's R eye. At the hospital, found with no lasting injuries, returned to duties, with no modifications, but w/ prescription eye drops for irritation.

Employee assisting to feed belt onto conveyor at head pulley. Employee bent over guard at head pulley to pull belt when they felt sudden pain in lower back from reaching across. Employee refused Workers Comp claim/appt 4, 11, 12 Feb. and agreed to claim/appt Feb 17. Appt found lumbar strain; not allowed to lift over 25 lbs, bend, stoop, or twist until follow up.

The employee stepped on the edge of a concrete pad. The sand at the edge of the pad was soft and sank down as part of the employees foot was on the pad and part off. The compressing of the sand created an uneven surface causing EE's left ankle to roll; spraining it.

Employee has seen several specialists throughout months, MRI, X-ray to determine source of pain/issue. 3/18/20: doctor finally placed a hard cast on left wrist and gave cortisone shot.

The employee works in a position where frequent shoveling is required. EE's shoulder has been sore for a while and aggravated by shoveling. Is experiencing tingling in EE's arm when trying to sleep. Went to Dr. who says scar tissue has built up and prescribed 6 weeks of physical therapy.

Employee was taken for an exam on 1/10 and was diagnosed with fractured right thumb w/no restrictions. Doctor visit on 1/16 recommended restriction of use right hand as tolerated until ortho. visit. .

Employee was removing coil from electric valve when the valve busted causing scrubber water to run down behind employees safety glasses into EE's eyes. Took to have eyes checked to ensure no damage. Employee was prescribed antibiotics to prevent infection. No damage reported.

Employee was walking along side of tailings line measuring the height of the line. As they stepped along some brush they stepped in a hole they did not see and felt pain in their ankle. They went to the doctor and were diagnosed with a sprain and were placed on restrictions.

Employee was removing a grating that had fallen in a drainage trough. After task completion, employee felt discomfort in abdominal region. Discomfort was reported on 10/2/19. Discomfort became worse on 10/30/19. Employee was taken to the hospital on 10/30/19 and diagnosed with a potential hernia.

Employee was replacing a troughing roller on BC54 at the head end of the belt. While turning to pick up a bolt to fasten down the new assembly EE hit knee on the torque arm support for the reducer. This caused EE's knee to flex sideways. EE was experiencing some swelling and soreness. Update: Went to doctor on 9-20-19 and scheduled an MRI for 9-25-19.

Party injured left shoulder while trying to lift the lid on one of the sand-trucks. Lid was bound close and the 3ft' pry bar-like tool was used. Employee stated EE may have jerked back EE's arm to hard and felt EE's shoulder pop. Later that night it started causing EE pain.

Employee was cutting weeds and brush at the intersection of the Utica 1 and 2 roads to make better visibility with traffic and they got poison ivy rash on their body. Employee received medical treatment to relieve the itching on 8/23/19. Employee did not miss work or have modified duty as a result of this incident.

Working on bend pulley on overlay conveyor, load shifted and caught maintenance workers index finger. EE was able to pull finger out of the glove, but the tip of the finger was bleeding and first aid did not stop it. Doctor visit confirmed tip of finger was fractured, but did not stitch or glue the finger. A splint was placed, no work restrictions.
