Showing all 7Maintenance man was removing the output shaft on the front of the Caterpillar engine that drives the hydraulic motor for the dredge. In the process of removing the shaft, the vibration dampener that the shaft was bolted to fell off and hit the maintenance man's right middle finger causing break to the right middle finger.
Employee was grinding down dredge cutter head shaft when the cup style grinding wheel on the 7" grinder came apart and struck his arm causing lacerations and an arm fracture. Employee was wearing PPE.
