Employee was using a Belt winder to wind old belt that was replaced at the quarry. During the process the employee was using their foot to "guide" the belt in place while the winding process was occurring. Employee slipped and ankle become entangled between the belt and guide roller. The employee received a fracture to right ankle.

When EE was swinging the excavator bit from a telehandler to loosen the head nut dust cover, EE realized its path was misaligned. EE attempted to hold the bit back mid-swing. EE was unable to hold it back and the bit slipped off the welded plate causing EE's right ring finger to contact the plate.

Associate was welding on skid steer bucket. Another associate came over to assist to complete the job. Both were welding back to back resulting in light reflecting on the inside of the welding shield. Associate noticed eye irritation the next day when the associate awoke.

We believe that when EE was tightening the side rails on the screen with the impact gun that the metal had fallen into EE's eyes behind EE's safety glasses. The next day, EE woke up with eye pain and went to urgent care to get checked out and they found metal in EE's eyes which they were able to remove.

Employee was removing cutting edge modules from a 988k loader bucket. While loosening the bolt on the module, the bolt started to spin. Employee reached under and pushed up on the bolt, then engaged the air gun on the nut. This caused the bolt to slip and start spinning again. The bolt head tore through employee's glove into the palm, cutting EE's right hand.

DRILL TECHNCIAN WAS DEMO'ING A NEW DRILL. WHILE ATTEMPTING TO ADD STEEL LENGTH TO THE DRILL, TECHNICIAN'S HAND BECAME ENTANGLED IN THE LIFTING SLING. AS THE STEEL PIPE WAS LIFTED THE SLING PINCHED THE SMALL FINGER OF THE RIGHT HAND. TECHNCICAN REQUIRED MEDICAL CARE BEYOND FIRST AID.

EE was assisting for the third time at greasing the plant. While the grease fitting was attached to the button on the grease line, pressure built up and as EE removed the line off the button, grease under pressure was injected into EE's finger. EE was wearing only latex gloves. The two associates decided not to report the injury to management.

When moving a crane from work area, our associate began to raise the crane unaware that one of the hooks from a two-way chain had become lodged under the flange of a beam. This created tension on the chain until it dislodged and flew upward. The chain hook struck the associate in the forehead.

WHILE REMOVING THE CUTTING EDGES FROM THE BUCKET OF THE 990 LOADER, ONE BOLT DROPPED CAUSING SPINNING. AS EE REACHED UNDER THE BUCKET, PUSHED THE BOLT BACK INTO THE SLOT & PULLED THE TRIGGER ON THE 1 INCH AIR IMPACT WRENCH, WHICH RESULTED IN CUTTING THROUGH EE'S GLOVE & LACERATING EE'S LEFT INDEX FINGER.

Employee was changing cutting edges on 990 loader. While using 1" impact gun, left hand slipped off the handle & middle finger came in contact with the socket & nut causing employee to get cut. Employee was taken to the hospital where employee received 10 stitches. Employee was wearing gloves.

While holding a wrench on a bolt to be removed on front suspension cylinder of 769C water truck, another associate was running an impact wrench. The nut loosened, retaining ring moved, & the socket flew-striking employee in the bridge of the nose.

While installing the bolts that connect the top shell to the bottom shell of the crusher, EE was holding the head of the bolt from the bottom & using 2 fingers to support the center of the bolt. Another associate began to tighten the nut, the bolt spun, grabbing EE's fingers and spinning them across the steel plate deck. The steel deck sliced the middle finger.

Associate was removing ground engagement tool (wear plate) from the bottom, leading edge of a CAT 990 front end loader bucket. While pressing the bolt up to re-engage the square shoulder the associate triggered the impact wrench causing the bolt to spin and the sharp edges of the bolt to cut through the glove and cut his left index finger.

A crew member was using a 5" Dewalt grinder to cut off excess angle from a mid rail. The employee was holding the angle with his left hand and grinder in right. There was no handle on the grinder and when it got in a bind it jumped into the employee standing one step down, striking him in the Left Arm just below the elbow.

Associate cut thumb nail while trying to drill a pilot hole. He was holding a small clamp when the drill slipped and the attached hole cutter cut his left thumb nail.

EE was preparing to use a cutting torch. His cutting goggles were on his hard hat. As he was pulling them down to put over his eyes, he got a foreign object in his eye. *This is being reported to MSHA late because it was not officially reported internally to Luck Stone until 5/24/07.

While reworking a steel screen feed box EE cut a steel plate loose without securing it and it turned over on his arm. The plate cracked the bone in his right arm just above the wrist.

Associate was installing middle wedge on stationary liners at C145 jaw crusher. He slipped and accidently hit the release tab on the safety bar holding liner plate. Plate then tipped forward pinning associate's legs between the stationary liner and the swing jaw.

EE was assisting with excavation of new area for test drilling. Coworker was operating excavator and hit a 35' pine tree, breaking it near ground. Tree fell, striking EE on hard hat & knocking him unconscience. Coworkers called for assistance; EE regained conscienceness & was airlifted to hospital for further observation. EE was released from hospital at noon on 3/10/05.

Employee peeled loose paint from side of machine when radiator fan blew paint or other debris into his right eye. Safety glasses were on, but were pulled down for better visibility. Employee was treated for corneal abrasion and released with 2 prescriptions.

ASSOCIATE WAS TAKING FEED BOX TO THE CONVEYOR W/CRANE. THE BOX WAS BEING PUT ON THE GROUND. WHEN CLOSE TO THE GROUND, HE PUT HIS LEFT HAND ON IT TO STEADY THE LANDING. HIS HAND WAS IN THE WRON G PLACE. WHEN BOX LANDED IT SHIFTED. HIS HAND WAS PINNED BETWEEN BOX AND BEAM.
