Employee was cutting belting with battery operated carpet knife and it slipped and came back hitting EE's right foot cutting through boot/sock and into the ball of foot behind the big toe. EE had steel toe boots on. - Laceration was cleaned at the clinic and Steri Strips were applied.

Showing all 3While using a cutting torch a piece of steel came around his safety glasses and into his left eye. He was able to remove the metal from his eyelid and then went to a clinic to make sure it was clear. Scratched cornea reported and return to work same day.
THIS INJURY WAS UNCOVERED DURING AN INTERNAL AUDIT AND WAS REPORTED BECAUSE I OVERLOOKED IT EE SMASHED RIGHT INDEX FINGER WHEN STUCK ROCK IN JAW MOVED AND CAUGHT HIS FINGER BETWEEN THE PRY BAR AND SIDE OF THE JAW. EE HAD MEDICAL ATTENTION INCLUDING THE APPLICATION OF A FINGER SPLINT PROTECTOR AND WAS OFF 2 1/2 DYAS BEFORE RETURNING TO WORK.
