Employee was cutting belting with battery operated carpet knife and it slipped and came back hitting EE's right foot cutting through boot/sock and into the ball of foot behind the big toe. EE had steel toe boots on. - Laceration was cleaned at the clinic and Steri Strips were applied.

EE and another ee were trying to get the latch to release on co-worker's truck and when another ee got ee side to release the other side slammed ee hand and pinned Middle and Ring Fingers on right hand. Middle Finger is broken and Ring Finger received stitches.

The plant was shut down for the day and employee went to fix the guard under the feeder. Employee was underneath the feeder and a rock fell from above and hit employee on the head. Employee sought medical attention and employee had staples put into employee's head and had a slight concussion.

While moving the tires to make the stacker conveyor operational to the Neimann Graham South Quarry, the chain used in lifting the stacker to swing the tires out popped and pinched EE's ring finger on left hand between the chain and bucket. There was an injury that resulted in a loss of the top part of EE's finger right above the top knuckle.

Extremely windy and a gust of wind sent a particle of dust into his right eye, he had safety glasses on.
While using a cutting torch a piece of steel came around his safety glasses and into his left eye. He was able to remove the metal from his eyelid and then went to a clinic to make sure it was clear. Scratched cornea reported and return to work same day.
While dismounting from the track backhoe, employee missed a step with his right foot. He twisted his left knee and felt a pop. There wasn't any witnesses. No medical treatment at this time. The knee is sore and may see a doctor is it persists or gets worse.

While replacing flashing on a tail pulley discharge belt on the surge bin Unit #17-14, co-worker was using a 12lb sledge hammer to punch holes with an awl/punch. He swung the hammer, the head came down and hit the top of the awl-slipped off the awl and hit EE's hand, EE was holding the awl in place. EE was driven by the quarry foreman to the ER in West Union for xrays.

He was standing on a ladder using a torch to cut a piece of metal on the jaw, he lost his balance a little and while regaining balance the torch was in his right hand and it went over his left hand (he then regained balance on the ladder again).

WHILE CLEANING UP AROUND PLANT USING A HAND SHOVEL STRAINED LOWER ABDOMEN. THINKING IT WAS JUST A MUSCLE PULL DIDN'T SEEK IMMEDIATE CARE AND WAITED FOR THE PHYSICAL HE HAD SET UP EARLIER TO FI ND OUT WHAT HAD HAPPENED. SUBSEQUENTLY FOUND OUT THAT HE HAD SUSTAINED A HERNIA THAT REQUIRED SURGERY. PHYSICAL TOOK PLACE ON 4/30/03 & SURGERY WAS SCHEDULED FOR 5/15/03. WAS OFF WORK FOR 26

THIS INJURY WAS UNCOVERED DURING AN INTERNAL AUDIT AND WAS REPORTED BECAUSE I OVERLOOKED IT EE SMASHED RIGHT INDEX FINGER WHEN STUCK ROCK IN JAW MOVED AND CAUGHT HIS FINGER BETWEEN THE PRY BAR AND SIDE OF THE JAW. EE HAD MEDICAL ATTENTION INCLUDING THE APPLICATION OF A FINGER SPLINT PROTECTOR AND WAS OFF 2 1/2 DYAS BEFORE RETURNING TO WORK.
