Employee was cranking a cable winch to pull the dust stacker conveyor sideways. The cable guide bar on the winch failed. The broken bar flew up striking the employee on the right side of EE's face causing a laceration, fracture and right eye injury.
Employee was operating a Porta Power hydraulic jack. The employee's right hand little finger got caught in the pinch point at the base of the jack handle.
Employee was operating a 7" portable hand grinder. The grinder kicked to the left causing a laceration on the employees left forearm.
