Showing all 14Employee was cutting grass with zero turn lawn mower. Tire dropped in ditch causing mower to turn on its side. Roll bar prevented mower from turning completely over. Employee stuck arm out to stop roll over and strained his shoulder.
Mechanic was cutting a 2" steel pipe with a Metabo and metal cutting disc. The Metabo kicked back and cut employee on left arm, 3" above elbow. Employee suffered a 3" laceration requiring 7 stitches to close wound.
Reinstalling belly pans on machine, using impact to install bolts in belly pan. Air blew sand behind safety glasses, into his left eye.
While changing edge and trying to get bolts out with torch, the bolt snapped causing the edge to pivot and swing down, hitting his leg. Bruising and swelling right leg above the knee.
DOZER OPERATOR SUSTAINED A NECK STRAIN WHILE BACKING OVER A 2' LIFT. OPERATING IN REVERSE, THE D9R BACKED OVER A LIFT THEREBY JAARRING THE OPERATOR'S UPPER TORSO. EE INDICATED HE WAS EXPERIENCING DISCOMFORT AND WAS TRANSPORTED TO THE PHYSICIAN FOR EXAMINATION. DIAGNOSIS WAS A NECK STRAIN.
While mounting the D7 bulldozer, mobile equipment operator slipped from his hand hold (3-points of contact) and fell backwardard striking a HPDE pipe. He sustained a sprained lower back.
Employee wrenched his lower back while conducting a start up of a rotary dryer in the process plant. Employee is presently under doctor's care and no work status.
DOZER OPERATOR BACKING OVER UNEVEN GROUND AT HIGH SPEED RAN OVER A TREE STUP & JOLTED THE DOZER. OPERATOR SUFFERED PAIN IN LOWER BACK ON THE FOLLOWING DAY. NO DAMAGE TO DOZER WAS SUSTAINED.

EE REVERSING CAT, D6 TRACTOR IN RECLAMATION AREA. RIGHT TRACK STRUCK LARGE STUMP. EE BRUISED RIGHT HIP. EE RECEIVED MEDICAL CARE. SEAT IN TRACTOR DETERMINED TO BE WORN TO BE REPLACED.
WHILE LIFTING PUMP IMPPAR USING 5-TON BRIDGE CRANE HOIST, EE HAD LEFT HAND PLACED BETWEEN AUXILIARY HOOK ATTACHED TO IMPELLAR AND CRANE HOOK TO GUID IMPELLAR FOR INSTALLATION. CRANE HOOK SLIPP ED INSIDE EYELET OF AUXILIARY AND PINCHED TIP OFLEFT INDEX FINGER AGAINST IMPELLAR VEIN.
EMPLOYEE LEANING OVER DREDGE DISCHARGE LINE REMOVING BOLTS WITH IMPACT AIR WRENCH. STRAINED MUSCLE IN MIDDLE BACK. EMPLOYEE RECEIVED MEDICAL ATTENTION, PRESCRIBED MEDICATION, RETURNED TO DUTIE S WITH RESTRICTIONS FOR ONE WEEK.
OPERATOR REPORTED HIS BACK STARTING HURTING WHILE HE WAS OPERATING BACK HOE LOADING OFF ROAD TRUCKS IN HARD PAN. OPERATOR CONTINUED TO LOAD TRUCKS UNTIL THE END OF HIS SHIFT. AT THIS TIME, OPE RATOR COULD HARDLY EXIT EQUIPMENT. WITH ASSISTANCE OF SUPERVISOR, OPERATOR EXITED EQUIPMENT AND WAS TAKEN FOR MEDICAL TREATMENT.
