Lull overturned in stockpile. Operator egressed through rear window. Operator was diagnosed with contusion in shoulder. Restricted to no heavy lifting.

Employee hooked lifting strap to water pump. Injured Employee was still standing on tongue to pump. Strap connected to excavator. Excavator operator rolled the bucket of the excavator so other employee to remove chain from bucket. This exerted tension on strap which caused pump to become un-hooked. Pump spun, Injured operator slipped between A-Frame causing cut on shin.

Employee was directing adjustment placing of pipe on a pipe welding machine. Pipe made a sudden movement thinking it was going to fall the employee made an effort to move. He placed his hand on a part of the machine which was not in use. This was on the lower half of a clamp. The upper half dropped hitting the middle finger on the left hand. Finger was fractured

Employee was bringing tools to another Employee and slipped on concrete floor which had "flock" on it. Employee's elbow hit an anchor bolt.

Operator was performing pre shift on Front End Loader. When accessing the top compartment to check coolant level, the access door fell forward and landed on his hand, breaking his middle finger.

While cutting a 16" HDPE pipe with a chainsaw, the pipe snapped midway through the cut and fell onto the employee's left foot, causing a bruise to his ankle.

While stepping over a 14" HDPE pipe, the employee placed his foot on slightly uneven ground, which caused his ankle to turn.

Employee was observing the discharge of a sanded line. When the slug broke loose, the line pulled free and sprayed material. The employee was turning to get out of the line of fire and hit his head on a piece of fixed equipment.

An employee was assisting the operators mechanic with a routine inspection on a piece of plant equipment. When prying the piece of equipment with a pry bar, the pry bar slipped and struck the employee in the lower lip. The employee went to seek medical treatment and was given 2 stitches to seal the sustained injury.

Pond 1135 experienced a failure of the west wall with approximately 9 inches of tailings and water leaving the pond, traveling through a stormwater conveyance ditch, filling a sediment basin and leaving site through the rip rap outfall.

Employee was walking in mining field flagging power cable when his left heel stepped on something (unknown) and employee fell with left leg underneath him. Employee said it burned and he reported incident to his supervisor. On 8/2 employee asked to see Dr. and was diagnosed with strained calf and given 2 weeks light duty.

Employee was working in top of plant. Became hot and went to cool down. Was transported to hospital for evaluation only as precaution. Diagnosed with heat stress and discharged within 2 hours. No loss of consciousness or orientation occurred.

Employee stepped inside CAT generator trailer with heel of boot landing on lip of catch pan causing employee to fall forward and burn arm on diesel engine turbo. Employee received a "dollar bill" size second degree burn to left forearm.

Employee picked up a 2 gallon bucket of sand with his right arm and felt a sharp pain in his right shoulder. Seen at local ER and diagnosed with separated AC joint in right shoulder.

Employee was dumping oversize material with FEL when right front tire sunk in mud. Employee's hand slipped off steering and was caught by spoke resulting in his wrist twisting & causing slight pain. On June 15 employee's wrist still hurt & he was seen by company doctor and was diagnosed with hairline fracture of bone in his hand.

Employee was cutting grass with zero turn lawn mower. Tire dropped in ditch causing mower to turn on its side. Roll bar prevented mower from turning completely over. Employee stuck arm out to stop roll over and strained his shoulder.

Employee was attempting to line up 2 pipe flanges so they could be bolted together. As he pulled up on pry bar to align bolt holes, he felt something pull in right shoulder blade area. He did not report this but @ 1530 hours his back began cramping & he notified the Foreman. He was seen in local ER and diagnosed with a muscle strain. Pain med prescribed.

Employee was walking on a slight slope, slipped and twisted left ankle. Seen in local ER and diagnosed with sprained ankle. Returned to work on restricted duty.

Employee was walking on flat, level ground and left knee began to hurt. Went to lunch and noticed swelling. No slip/trip/fall/bump/twist occurred prior to pain. Seen by Dr. who diagnosed swelling on left knee - gave ice pack, ace wrap, prescription pain med & 3 days light duty. Employee does not believe it is work related.

Employee was walking toward pipe valves, slipped and fell in mud, striking right shin just below the knee on bolted metal pipe flange. Employee was seen in ER, diagnosed with large hematoma below knee, given 2 days of restricted duty, pain medicine and anti-imflammatories.

Ind. walked up sandy slope and fell in loose sand spraining left knee and dislocating left patella. Traveled to medical clinic - x-rays were neg. Follow-up has included an MRI and Orthopedist visit. Orthopedist indicated that MRI showed bone bruising and torn ligament associated with left patella. Suggested rehabilitation on own and follow-up visit in 2 weeks.

Employee was tightening a flanged suction pipe connection to a diesel pump when the open ended wrench slipped and he fell backwards landing on the frozen ground. Employee suffered a contusion to his buttocks. Seen by Dr who provided prescription and 2 shots as well as 2 weeks "restricted duty."

Mechanic was cutting a 2" steel pipe with a Metabo and metal cutting disc. The Metabo kicked back and cut employee on left arm, 3" above elbow. Employee suffered a 3" laceration requiring 7 stitches to close wound.

Employee was dumping a bag of lime in process pond weir. Wet lime splashed on his face. He removed his hard hat & safety glasses to wipe lime off face and caused it to fall into his eye. Employee immediately flushed his eye with water but suffered a small chemical burn under top eye lid. Employee taken to Occupational MD and given prescription eye drops for irritation.

Electrician & mechanic were changing junction box on motor. Electrician threaded 2 of 4 mounting bolts & was having difficulty starting threads on 3rd bolt. As he started threads, mechanic tapped top left corner of box with hammer. This was at the same time the electrician placed his gloved hand on top left corner of box and was struck by hammer.

Plant operator was pulling a rolling ladder across the floor when one of the legs caught the toe plate causing the ladder to come to an abrupt stop. This caused a sudden twisting to his lower back resulting in a muscle strain.

Supertindent was walking to parking lot attempted to step over silt fence and got r foot caught in fence. Fell to ground breaking both lower leg bones, Dislocating ankle and tearing ligament. Required surgery (6 pins/plat) to repair fracture.

Employee went to step over 18" pipe in mining field. As he stepped on the pipe, his foot slipped off, causing him to fall to the ground. He stuck his right arm down to break his fall & jammed his shoulder. At that time he refused medical examination & said he was fine. On 6/11/09 he was diagnosed with torn humeral ligament & given cortisone injection & 4 weeks of physical therapy.

Employee was clearing blockage from mining unit pump suction. He slipped & fell from a standing position, landing on his left hand. Unknown object punctured his left palm. Seen by company Occ. Med Center & sent to hand surgery specialist for stitches.

Employee was leaning over pipe flange in order to line up bolt holes with a spud bar for connection. As he stood up, he felt a sharp pain and cramping in his mid-lower back.

Employee states while splitting sand he hurt his neck. He has stated that while he was standing with his arms fully in front of him at chest level, his pain started in his neck.

Tailings pond #823 failed along the eastern wall from decant water piping through a deep linear crack. All water flowed into pond #827. Pond had just been repaired from previous failure.

EE was clearing blockage on distributor pot of coronastat. Rubber and metal end cap came off pipe and landed on hand, causing laceration to right middle finger. Seen in ER and given 4 stitches to close wound. Returned to work, no restrictions.

Tailings pond #823 developed a failure in the eastern berm wall from decant water piping thru a deep linear surface crack into pond #827. No persons or equipment were in danger at any point. Cause is shrinkage of clay material from 3 month drought.

EE was lifting laboratory sample and felt sharp pain in lower back. Seen by company's occupational physician, who diagnosed lower back muscle strain and gave prescription meds X3 (muscle relaxer, analgesic cream and 800 mg Motrin). Lifting restriction X 1 week, did not affect ability to perform primary duties or tasks.

Employee was using maul and brass punch to drive out stuffing box on 2" X 1.5" pump. Missed punch with maul and struck top of left hand causing large hematoma.

Employee opened flush water valve on 3" X 2" pump and hot sand immediately sprayed on right arm, resulting in second degree burns to upper and lower right arm.

Employee was dismounting machine, had both feet on the ground. When he turned to leave the machine his feet slipped on muddy ground. The slippage caused employee to fall to his knees causing a stress fractur in the lower leg.

Employee was carrying tool bag to truck. He noticed itching and swollen red spot on his left hand that appeared to be a bite/sting from an unknown insect. Seen by company doctor for localized swelling of hand. Anti-histamine & anti-inflamitory prescribed.

Reinstalling belly pans on machine, using impact to install bolts in belly pan. Air blew sand behind safety glasses, into his left eye.

EE was walking through parking lot to shop bldg. EE fell to ground suffering abrasions to arms & hands and sprained rt. ankle. EE has history of sprained ankles as this is his 4th in 3 years.

Employee threw a metal pipe patch approx. 10' into a tool box. As he released it from his hand the patch cut his right forearm 2" long x 1/2" deep requiring 12 stitches and a tetanus shot. Returned to full duty immediately following Dr's visit.

Unloading trash/boxes from back of service truck into dumpster. Footing slipped & the employee fell from the truck. He hit the back of his head on the dumpster & his buttocks hit (dirt) the ground.

EE was stripping electrical cable jacket to facilitate termination in junction box. Knife blade slipped out of rubber jacket, stabbing EE in the left pointer finger. EE was cutting towards himself with knife. Required 4 stitches to close laceration.

Employee was walking down stairs to office trailer while carrying out trash bag. He slipped, grabbed handrail and reportedly injured wrist. He made notification to supv. but stated he was ok. On 5/31/06, he was diagnosed w/torn ulnar collateral ligament by MRI. Surgery scheduled on 06/07/06.

EE was ground man working with cat 320 to set. diesel pump in place to remove excess water from pond. EE states his R foot slipped while walking on a slope and his knee "buckled". EE did not report injury until next day 1/31/06. Surgery to repair on 3/29/06 torn cartilage.

ROTARY KILN IN ZFP @ MINERAL SEPARATION PLANT EXPERIENCED MINOR RAPID COMBUSTION/EXPLOSION DURING SHUT DOWN PROCEDURE. MINOR DAMAGE TO DISCHARGE EXHAUST COWLING, DUCT WORK & SUPPORT PIERS.

EE bent over to adjust valve on overflowing sump. Cascading water knocked off hard hat while bent over. EE stepped back away from valve-stood up and struck top of head on drain plug of another valve causing laceration. V shape 4" long.

EMPLOYEE WAS ATTEMPTING TO CLIMB DOWN FROM A 5TH WHEEL UTILITY TRAILER APPROX. 3'FROM THE GROUND SURFACE. HE PLACED HIS LEFT FOOT IN THE APPROPRIATE STEP & THEN PLACED HIS RT. FOOT ON TOP OF A PIPE RESULTING IN HIS RT. FOOT SLIPPING OFF PIPE & EE FALLING THE REMAINING 12"TO THE GROUND.

While changing edge and trying to get bolts out with torch, the bolt snapped causing the edge to pivot and swing down, hitting his leg. Bruising and swelling right leg above the knee.
