Employee was using a grinder on a handrail, when the grinder caught and was jerked out of the employee's hand. The grinder fell and struck the employee's left boot where it cut through the boot and into the foot.

EE was operating a 627G scraper when he hit a rough spot in the road and strained his lower back. Taken to hospital via pickup truck.

EE was cutting metal with an abrasive saw when a piece of metal passed by his face shield and glasses and embedded in his eye. He thought it might extract itself but he eventually went into emergency room to have it removed the next day. It was a mild embedding and was quickly extracted by the doctor. There will be no lost time from this accident.

EE attempted to dislodge and remove a rock from a slicer screw by entering the housing from the top. As the screw continued to rotate toward his hand, he pulled it back but was not quick enough to avoid having his left index finger caught between the blade and the rock. The finger had 3/4 of an inch amputated. No lockout was in effect at the time.

EE was helping change hammers in a hammer-mill when a hammer got caught and caused the rotor to change directions. It spun backwards and caught the pry bar the EE was holding onto, causing it to swing and strike him in the lower chin area. This action resulted in many teeth being fractured and three stitches in the lower jaw.

EE was cutting hinge off right loader door. He was cleaning weld off with grinder when it kicked back and deeply cut left arm several inches above the wrist. He went to emergency room to have it cleaned and stitched up.

EE ran over a large rock while operating a scraper which caused him to pull groin muscle in right leg. He expects to return to work on 02/23/3009.

Employee was grinding inside of fire-box when a piece of metal by-passed his protective eyewear and lodged in his right eye. He went to doctor the following morning and had it extracted, causing him to miss one day of work.

Employee was running dozer in reverse, resting right arm on rest when a rock fell in behind track, causing jolt to right shoulder as machine raised then came down hard. He thought shoulder would get better, waited a couple of weeks, a torn rotator cuff was verified-requiring surgery on July 9, 2007.

EE failed to do a proper lockout procedure, reached in and attempted to change a dust collector belt while the operator was manipulating the control button. This was due to miscommunication between the two and caused the EE having EE's middle finger tip pulled through the pulley, which required two stitches.

While running a push-pull scraper operation in pit, the operator of the pull scraper pulled up a rock that the push scraper hit, causing the operator of the push scraper to be bounced, hitting his head on the roof of the machine.

HOT SLAG BURNED A HOLE IN 02 LINE WHEN CUTTING METAL WITH OXYACETYLENE UNIT ABOUT 6" BELOW FITTINGS, RESULTING IN 1ST & 2ND DEGREE BURNS TO LOWER RIGHT ABDOMEN AT BELT LINE. THE 2 X 3" AREA OCCURED THROUGH 5 LAYERS OF CLOTHING, 4 OF WHICH WERE COTTON OR PARTLY COTTON. E.R. ADMINISTERED CARE & HAD HIM TAKE ONE DAY OFF.

EE WAS USING A DIE GRINDER TO ENLARGE BOLT HOLES ON A BEARING WHEN A SMALL PIECE OF METAL GOT PAST HIS EYE GLASSES, STRIKING HIM IN THE RIGHT EYE. HE MISSED WORK THE FOLLOWING DAY AS HE SPEN T TWO HOURS IN THE DOCTOR'S OFFICE TO HAVE THE PIECE REMOVED.

EE WAS WELDING AN I-BEAM, HELD BY CLAMPS, TO A NEW HOPPER LID. HE ATTEMPTED TO TURN THE BEAM AND THE TACK WELD BROKE CAUSING THE BEAM TO FALL ON HIS UPPER & LOWER RIGHT ARM. THE INJURY REQUIRE D 4 DAYS OF LOST TIME TO ALLOW THE BRUISING AND PAIN TO SUBSIDE.
