EE WAS CHANGING PIPE. HE WAS TIGHTENING BOLTS WITH AN IMPACT WRENCH. A PIECE OF METAL CAME OFF OF THE BOLT & HIT THE EE'S RIGHT EYE. EE STATES THAT HE WAS WEARING SAFETY GLASSES AT THE TIME OF ACCIDENT.
EMPLOYEE WAS SPINNING ON IMPELLER ONTO A PUMP BY HAND USING DRIVE BOLTS. THE RING FINGER ON HIS LEFT HAND WAS CAUGHT BETWEEN A SHIVE AND A BELT.
