EE WAS ATTEMPTING TO CHECK THE PUMPS IN THE BOTTOM OF THE PIT. THE WATER WAS GETTING TOO HIGH. AS HE STEPPED OFF OF THE STAIRS THE WATER WAS HIGHER THAT HIS BOOT TOPS. THE WATER WAS VERY HOT AND BURNT HIS LEFT ANKLE.

EE HAD PULLED A 15 PASSENGER VAN INTO THE SHOP TO CHANGE A FLAT TIRE ON THE LEFT REAR. HE CRAWLED UNDER THE VAN AND RAISED IT WITH A SMALL HYDRAULIC JACK. AS HE CRAWLED UNDER IT AGAIN TO RAISE IT HIGHER, IT ROLLED OFF OF THE JACK AND CRUSHED HIM.

WHILE RETRIEVING THE CORE FROM A 45 DEGREE UPHOLE, EMPLOYEE UNSCREWED THE STUFFING BOX. THE OVERSHOT & STUFFING BOX CAME DOWN W/SOME FORCE & PINCHED HIS RT. HAND TO THE ROD SADDLE. THE BONES BETWEEN THE RT. THUMB & WRIST ARE SPLINTERED.

EMPLOYEE WAS USING A PIECE OF WIRE TO UNPLUG ANFO FROM AUGER. THE WIRE APPARENTLY CAUGHT IN THE MOVING AUGER AND PULLED THE END OF HIS FINGER OFF.
