EE HAD PULLED A 15 PASSENGER VAN INTO THE SHOP TO CHANGE A FLAT TIRE ON THE LEFT REAR. HE CRAWLED UNDER THE VAN AND RAISED IT WITH A SMALL HYDRAULIC JACK. AS HE CRAWLED UNDER IT AGAIN TO RAISE IT HIGHER, IT ROLLED OFF OF THE JACK AND CRUSHED HIM.
Slate was being lowered with an overhead crane. Slate slipped and hit his hand. EE suffered deep laceration to right pinky finger. He was treated at hospital and released.
WHILE RETRIEVING THE CORE FROM A 45 DEGREE UPHOLE, EMPLOYEE UNSCREWED THE STUFFING BOX. THE OVERSHOT & STUFFING BOX CAME DOWN W/SOME FORCE & PINCHED HIS RT. HAND TO THE ROD SADDLE. THE BONES BETWEEN THE RT. THUMB & WRIST ARE SPLINTERED.
While working on drill press, drill bit caught causing a pice of iron to be pulled out of the clamp. Iron spun around causing a laceration to his left hand.
EE WAS SITTING IN THE TAMROCK BOLTER WHEN A LARGE BOULDER FELL OUT OF THE BACK AND CRUSHED THE CAB DOWN ONTO HIM.
CORDLESS DRILL WAS PLACED ON BOOM OF BOLTER. WHILE DRIVING IN A PIN THE DRILL FELL OFF BOOM AND THE DRILL BIT PUNCTURED RIGHT FOOT.
EMPLOYEE WAS USING A PIECE OF WIRE TO UNPLUG ANFO FROM AUGER. THE WIRE APPARENTLY CAUGHT IN THE MOVING AUGER AND PULLED THE END OF HIS FINGER OFF.
