Over-exertion (Not Elsewhere Classified)

Employee was working in a quench pit and got boot stuck in 2"" of mud. When the employee tried to free the foot the employee twisted / strained knee.

Flash burns (welding)

Employee was exposed to arc flash from air arc process, while performing confined space attendant duties. Employees were removing structure holding a CIL screen. Employee had eye pain that evening and returned to work the next morning. Reported to leadman that their eyes hurt from the previous days work. Was taken to doctor and prescribed antibiotic eye drops for infection.

Fall to the walkway or working surface

Employee was accessing the cooling pond to retrieve level measurement. The employee stepped on the cooling pond liner with right foot and EE's leg slid out from under EE's body. The slip caused EE to do the ""splits"" resulting in an injury to their hip and thigh. Employee was assessed at NNRH and was put on restricted duty for a hip/thigh injury.

While changing out the slides & guides on Classifier Bucket Elevator, the East Guide was removed without incident. Fellow workers trying to remove the west guide got it out of alignment to remove easily. Injured worker had taken bolts out of the guides on both sides. After removing guide on East side, worker went to west side to assist the other when incident occurred.

Inhalation of radiations, caustics, toxic and noxious substances

Employee was changing out a chlorine tank at the gas handling circuit and after the tank was removed, they went to go get a tool to see if there were any leaks, and employee was exposed to a cloud of chlorine gas.

Employee was crushing samples in sample prep at the lab. The employee's left finger was smashed when the mobile crusher lid closed. The employee was placing a sample bag down when the lid inadvertently smashed employee's finger. The employee sustained a fractured finger as a result of the injury.

Employee was working on a ladder when another employee brought forklift forks down onto the employee pinning EE to the motor. When freed the employee then stepped off the ladder landing on one leg with the other hung up in the ladder twisting EE's back. 7000-1 created 02/09/2022 due to Workers Compensation authorizing physical therapy 02/07/2022 and setting it up 02/09/2022.

Over-exertion in pulling or pushing objects

When pushing a reject cart on the pad the cart struck a hole in the ground and stopped suddenly, muscle soreness was noticeable after returning to heavy physical activity.

The employee was going to dump mop water outside of the Security building when employee slipped on a patch of ice from a water drip from the awning and landed on left side injuring left elbow and wrist.

While the employee was walking to the scrubber containment unit by the FL room the employee slipped on mud on the concrete and fell back onto the right wrist twisting back during the fall.

The employee was attempting to clean the front headlight on the right hand side of the loader 42. The employee locked down the equipment and articulated it for easier access. The employee could not reach the light from the lower railing and decided to lean over the railing on top. As the employee was retracting they felt a pop on the right hand side of the ribs.

The employee was removing glass from a window in the lab when the window broke causing a laceration to the left wrist.

While cleaning CIL containment the employee attempted to lift a wash down hose that was buried in the mud and strained EE's lower back. A full duty release was received at the initial Dr. visit. On July 1, 2021 EE attended a follow up appt receiving a light duty release hence making this injury a medical reportable injury.

The employee was breaking core with a standard rock pick when an employee next to EE was moving a box of core. As this employee moved the box of core EE inadvertently made contact with EE's arm mid-swing causing EE to miss the core and hit EE's thumb.

An employee was helping another employee weld a steel plate in the bag house. this area was tight and the employee had some exposure to weld flash.

While the employee was moving a 3 inch sump line, on one end of the line was a rope, that the employees used to assist with moving the line across the tank. The line fell, causing the rope that the employee was holding on to pull and jerk on the employees shoulder.

Employee was walking backwards in the maintenance shop. While doing so EE tripped over a pallet and landed on the part that was on the pallet and hitting EE's tail bone.

An employee was working at the primary crusher. A propane bottle was knocked over and fell from the upper deck striking the employee on the lower back. on 1/25/2017 the employee arrived at work complaining of pain in lower back, the employee was transported to see a doctor.

While lifting something up with a pry bar the employee felt something like a shock go through into their fingers. After every time they squeezed their hand it would hurt. After eating lunch the pain was still bothering them and they reported it.

While the employee was descending a ladder, EE let go with one hand still maintaining three points of contact to look to see how far away EE was from the landing when EE's hand slipped off of the rung causing EE to fall roughly three feet onto a hand rail resulting in a broken rib.

The employee had put a hook on a rock bolt coming down the apron feeder for the Primary crushing circuit and while pulling the metal out his left hand was on the metal liner for the apron feeder; upon loosening the ore by removing the rock bolt a rock on top shifted and rolled on to his left hand.

While working on the skid steer, noticed a large rock on the #1 and #4 belt. The employee went over to the cone to look inside and placed his right hand on the screen to pull himself up and a large rock hit his finger.

The employee was attempting to reposition sections of metal in the metal storage rack. The employee was using a metal clamp with his left hand at the same time as he was holding the second sheet of metal in his right hand when the metal shifted backwards pinching his right little finger. The employee was walking out of the Wet Mill MCC going down some stairs in dim lighting when he missed a step causing him to step down wrong resulting in a twisted ankle.

CP-50B, a centrifugal oxygen compressor, tripped suddenly. Less than 1 minute after the trip, there was a rapid ignition of the compressor. Because oxygen at 99% purity was lined up, the fire grew quickly in size and heat. The cost to repair or replace the compressor is currently estimated at $100000. Downtime is greater than a week.

Miner was using a hand held grinder to cut welds off of a metal project. The grinder ""walked"" away from the operator and went across the gloved left hand lacerating the skin on the left thumb knuckle.

Miner was working on a caustic line. He was wearing full PPE+ for the job that was assigned to him, including a full face respirator. Miner showered off with all PPE in place first. While removing respirator, he got a small amount of caustic in his left eye.

Miner was a passenger in a truck. Truck was parked, parking brake set, and chocked. Miner was not seat belted in as truck was parked. While vehicle was dumping, it tipped over onto the left side. Miner fell approx. 3' injuring elbow.

Miner was assisting in hosing down plant with water. Backed against a hand railing that had a 120v extension cord wrapped on it. Miner experienced an electrical shock. Miner was released light duty for 12-22-2013. He then goes off on RDO, and will be back to full duty.

Miner was assisting another miner carry a piece of pipe up an inclined roadway. Recent snow had fallen on the road covering previous snow and icy conditions. Miner slipped and fell on his left side. Miner complained of pain to his left leg and ankle.

Miner was engaged with guzzling out a vessel in the strip area. Miner received some very minor caustic burns on the neck and his face. Miner rinsed off for several minutes. Taken to hospital flushed more. Released full duty. On 12/22/2013 miner returned to hospital was given three days off by ER doctor.

Employees was doing a PM in a MCC and cleaning. While brushing out a breaker enclosure that was bottom feed it caused an arc flash.

Employee complaint of chest pain and tightness, care flighted to local hospital from mine site. Employee was tested, no heart condition found, diagnosed with muscle spasms of the chest due to coughing. Employee released from care. This is a non recordable event but was called in on the Immediately Reportable Hot line due to possible heart attack at the time.

Miner was walking on the ground level of the fine crusher area. A second employee was carrying a roll of liner material up some stairs. The employee lost his grip on the liner material and the liner fell from above, striking the miner on the side of the head and shoulder area.

Injury reported on 5-12-2013, no medical wanted by employee until 5-23-2013. Put on restricted duty until 6-6-2013, still hasn't seen by chiropractor. This is a reported repetitive motion injury. Miner is lifting 30# bags of ore samples and putting into small crushers in the assay lab.

Miner was shoveling fine ore under the conveyor belt, while doing this he experienced back pain from his activities. Miner reported a prior Non work related injury to the supervisor after the incident.

Miner was stepping off of a 950 loader after parking it. Miner stepped onto uneven ground under the step and twisted his ankle.

EE was wire wheeling silicon off of Fan Access Door. Re-positioned door and a sharp edge cut his upper right leg. Safety was notified. Incident was managed by Veris Safety and Veris Opps. Traveled to hospital for 15 stitches and was given permission to return to work same day. No medication prescribed.

Miner was climbing down a ladderway. Caught his ring (left Ring finger) on a handrail as he stepped off ladderway. Miner was wearing gloves, however ring had to be cut away by responding medical personnel and he was transported to hospital for sutures. Miner returned to work next scheduled shift with no restrictions.

Miner was seated in a chair. Miner stood up and took a single step on a relatively flat concrete floor inside of a well lit building. The miner was unaware that his foot had went to sleep, and the miner rolled his right ankle causing the tendons to strain.

Miner was using an air lance to clean out shoot. The air lance separated from the hose the hose end came up and caught the miner under the right eye, just below his safety glasses.

Miner was walking through an area on the mine site. The miner tripped and fell forward, striking his face against an object. The object caused minor facial injuries. Miner was treated for first aid at the mine site, transported to town to the doctor's office for follow-up treatment. The injury was stitched and miner returned for full duty.

Miner working in the refinery, noticed that the drain to a sump, located under a vessel, was not fully closed. The miner, not realizing the depth of the hot solution, stepped into the sump and the water went over his knee high boots and burned his left leg.

Miner was cutting wire ties off from a line on a water truck. As miner was running his hand up the line his hand came in contact with an old metal flange the was not seen by the miner. The miner cut his index finger on this flange. Miner was taken to the doctors office for stitches to the tip of this finger.

Washed hands at end of shift and felt a burning sensation on both forearms. Was treated at NNRH and was a very mild case of an unknown chemical burn and was released back to full duty work.

Mobile maintenance technician installing windshield in a downed piece of equipment. While tightening bolt with a wrench, the wrench slipped off the head of the bolt. Miner received a laceration on hand.

Miner was pulling a portable ladder out of a van, slipped on ice and fell backwards. Miner put out his hand to catch himself, when he struck ground, injured his wrist.

Fall from ladders

Miner was standing on a tied off ladder approx. 18"" off of floor. Miner was unthawing a frozen, or obstructed, water line. A loose union fitting uncoupled and the water in the line swept the miner off of the ladder. The miner fell to the floor injuring his right ankle and foot.

Two miners working to remove a guard from a apron feeder for repairs/clean up. One miner was attempting pry off the guard, while other miner was using an 8 lbs. sledge to strike on the main frame. The injured miner moved to get a better position. The second miner, using the hammer, the hammer glanced off of the frame and stuck the second miner on the right knee cap.

Miner was repairing a splitter 4"" valve (water). While they were trying to disconnect the valve from the water hoses, The last hose came loose and the valve dropped onto the ground, with the valve handle pinching the miner' s right ring finger between the valve and the ground. Miner was wearing proper PPE for this job task.

Miner was shoveling snow out of a walkway using a hand shovel. The shovel struck a concrete pole base. Shoulder injury was reported next shift to his supervisor. Miner was withdrawn from his normal duties and medical treatment was sought.

Miner was checking ore samples just inside of a recently opened bay door that had ice in the vicinity of the threshold of the door. When the miner fell on the floor she landed on her left kneecap.

Assisting a rig welder, employee was operating a forklift while the welder was cutting off random lengths off of the transition (cap for the hood). Employee placed his foot on flange and as he released the flange the flange struck his fingers and caused a laceration.

The employee was being pushed up hill with another 657E Scraper. The incident occurred when the second scraper made contact with the lead scraper, resulting in a slight jarring motion. The employee indicated he was not injured and completed the remainder of his current shift and the following days shift before compalining his back hurt.

Worker was in a lime silo and was washing it out when lime on his boots came into contact with water causing a chemical reaction and eating through his boots to his feet causing burns.

Employee was on the van getting ready to return home at the end of shift. A co-worker left the van door open and it was very cold outside. EE went to close the van door and stepped to the edge of door. She lost her footing & slipped out of the van face first, landing in the parking lot.

Employee was exposed to spilled Hg while cleaning a spill around scrubber unit. Biologic tests showed a high amount of Hg in this miner's system. Miner was immediately removed from area. Placed under medical care to reduce Hg levels further, she was removed from site by doctors advice and given medical to help assist in the reduction Hg levels in her system.

Struck by falling object

EE's failure to get sufficient grip & clamping force on tire he was off loading. Equipment didn't allow securement of tire. E#E's failure to ""Stand Down"" & wait for the mine to provide him with larger equipment. Truck driver's attitude in getting quickly unloaded so he could head home. 15 below zero icy conditions. Tire fell & struck EE, fracturing his right leg.

Non employee was riding as a passenger in a privately owned vehicle on the access road approaching the main gate, the driver lost control of the vehicle, left the road, rolling the vehicle and damaging the vehicle, our fence and seepage well pump. There are no reported injuries to passenger or driver of the vehicle.

Employee was replacing a screen and pinched his hand between the screen and a ladder guard. Hand was bruised and employee was prescribed nabumetone for pain and swelling, and instructed to use an ace bandage and ice for 48 hours. Light duty was prescribed for 48 hours, and was provided at work site. After light duty employee returned to full duty for next scheduled shift.

Employee was going to shut down haul truck at the shop and slipped on ice and fell.

Driving on the haulpak road going 25-30 mph. He heard a bang and the truck tipped on its left side and slid into the berm.

Employee was finishing repair of preg filter in refinery, reached for a come-a-long handle and made contact with a cable tray and cut his arm on a burr. Laceration was about 1.5 inches long and required stitches. Employee returned for his next shift.

Employee was attempting to remove an air hose from cyclone at roaster to install it on another cyclone. When removing the hose, the isolation valve came off and hot fine ore blew out and hit employees hand and thumb.

Employee was engaged in lining drainage pond and while changing a roll of the liner material a support pipe suspending the roll fell on him, striking left shoulder, arm and side. Employee suffered some bruising and returned to work for one day on light duty.

Subject exited side door of van and closed door. Dust/dirt on weather stripping was blown off weather stripping into subjects right eye. Right cornea was scratched. Eye doctor at the Total Eye Care in Elko suggested full healing in 48 hours. Subject returned to work.

Subject was performing janitorial work and was exposed to a cleanser she had a reaction to. Treating physician described the reaction as Chemical Bronchitis. She suffered cough, runny eyes, some difficulty breathing. No continuing effects. Exposure started on 10/19/2009 and continued to 11/04/2009 when the problem was reported.

EE was positioning steel with a spud wrench to weld steel into place. He pulled down on wrench abnormally, causing shoulder pain.

He was getting in the lube truck and went to close the door and cut his left index finger on the inside door handle.

Coworker was removing a PVC pipe from saddle it was secured in, when the pipe buckle from the tension it was under and struck subject in the right side of his face. Subject suffered contusion and laceration, was treated and returned for his next shift.

There are several factors that contributed to the accident, it happened to be a curve next to a pond where the evaporation causes the road to become very slippery and the fact that it had rained the night before changing into snow early in the morning; the rear tires in the van should have been changed and finally the fact that even though he was driving at the speed limit.

Working in lime slaker cleaning up when he walked around & stepped into a pile of lime it went up over his boot & went inside of his boot. Didn't bother him at first & he didn't take off his boot & clean it off inside. So it was 2 days later it started bothering him we took him to the ER then. He was released to go to work his next work day that was February 22, 2008

The employee was cleaning out around the Zero Speed hose, while the belt was running. He opened up the guard and stuck his Right hand in, past bearing for the tail pulley, with a pair of Channel locks to clean. When he lost his footing, slipped and fell into the belt, the tail pulley grabbed his channel locks and pulled him into the tail pulley.

When tripping out HQ rods, I woke up the next morning and felt a burning sensation on the right side of my groin. It could have been from mixing or hauling gel bags.

Employee was hooking up cable to a crusher cone gear wheel, while the wheel was still turning and got his left finger pinched between the hand rail and cable mashed his finger and fractured the tip of his finger.

This accident is supposed to have happened on 10/26/07, but never told the company until 12/1/07 at the start of that shift. At this time he was seen by a doctor, but was never put on restricted duty until 2/4/08 by the doctor. He was grabbing either a fire extingusher or a wheel chock when he pulled his shoulder.

Haul Truck driver lost his brakes coming down a grade. His left rear duals caught fire. He steered the truck into the berm and went off the road and tipped his truck over on to its right side. The driver was injured and taken to the ER room and was released about 4 hours later.

Was checking on a pressure tap seal in the Roaster. While stepping onto a grating to turn ball valve the grating bent down at a 45 degree angle and his foot slipped into the hole and fell to his knee. He caught himself against a hot cyclone and burned his forearm and hands.

Prying on grating when he hyperextended his left knee. Employee was placed on restricted duty.

EE was working on the stand for the ball mill, when a cloud of dust fell from above, striking EE on the head, resulting in a strain to his neck and left shoulder. EE was taken off work on 6/17/07 and returned to light duty on 6/18/07.

Employee was attempting to move a plate of steel, when he lost his footing on loose rock and fell to the ground with the plate landing on his left forearm causing the bone to fracture in two places. Employee did not work the second day of injury. He was placed on restricted duty. Project was complete on April 20, and employee was laid off.

While tracing piping at cooling tower, slipped on snow and hit knee on hugger bolt. Caused small laceration on knee, requiring 4 stitches.

EE WAS ATTEMPTING TO CHECK THE PUMPS IN THE BOTTOM OF THE PIT. THE WATER WAS GETTING TOO HIGH. AS HE STEPPED OFF OF THE STAIRS THE WATER WAS HIGHER THAT HIS BOOT TOPS. THE WATER WAS VERY HOT AND BURNT HIS LEFT ANKLE.

EE WAS ASSISTING THE MAINTENANCE GROUP WORKING ON A CONE CRUSHER. ONE OF THE HYDRAULIC ARMS FELL. HE TRIED TO CATCH THE ARM WITH HIS RIGHT HAND AND WAS STRUCK BY THE ARM AND FINGER WAS PINCHED AGAINST THE END OF THE SHAFT.

Slate was being lowered with an overhead crane. Slate slipped and hit his hand. EE suffered deep laceration to right pinky finger. He was treated at hospital and released.
